Resumo: Herpes simplex lesions are treated with acyclovir. Despite reducing symptoms, studies show that it does not prevent recurrence of lesions. Photodynamic therapy (aPDT) has advantages such as decreased remission time and absence of recurrence in the same place. Therefore, the objective of this study was to evaluate if photodynamic therapy can decrease the healing time of these lesions and decrease their recurrence. Patients who present with herpes in the vesicle or ulcer phase and are included in the study were randomly divided into two groups: G1- Aciclovir group - aciclovir + aPDT simulation was used for blinding of the study, G2- aPDT group: 0.005% methylene blue laser group irradiated with 660nm, 100mW (Laser Duo, MMOptics), 120J / cm2, 3J per point. Applied to the central part of the lesion, and 30 seconds of irradiation + aciclovir placebo. The main study variable was the lesion resolution time in days. Secondary variables were assessment of recurrence assessed in months, pain assessed by visual analog scale (VAS), local temperature by digital thermometer. The viruses were quantified by PCRq. All evaluations were be performed at baseline and after 3 and 7 days after the application of aPDT or its placebo. The questionnaire to evaluate the impact of laser treatment on quality of life was be applied at baseline and after 7 days for secondary variables. Long-term follow up was be performed at 1 month, 6 months and one year for quality of life and recurrence assessment. Results: There was no difference resolution of the lesions in both studied groups (p=0.308), pain (p=0862 in the first and p=0339 in the third day) and temperature (p=0.488 first day, p=0.528 second day and p=0.071 on the third day). Conclusion: there was no difference regarding the time for lesion resolution in the analyzed groups, also for all the clinical outcomes analyzed.

Apesar de diminuir a sintomatologia, estudos mostram que não previne a recorrência das lesões. Sabe-se que a terapia fotodinâmica antimicrobiana (do inglês Antimicrobial Photodynamic Therapy – aPDT) possui vantagens como diminuição do tempo de remissão e ausência de recorrência no mesmo local. Portanto, o objetivo deste estudo foi avaliar se a terapia fotodinâmica antimicrobiana foi capaz de diminuir o tempo de remissão dessas lesões e diminuir a sua recorrência. Os indivíduos que se apresentaram com herpes na fase de vesícula ou úlcera foram incluídos no estudo e divididos aleatoriamente em dois grupos: G1- Grupo ACV – Foi aplicado o creme aciclovir + simulação da aPDT para cegamento do estudo. G2- Grupo aPDT - azul de metileno a 0,005% irradiados com comprimento de onda de 660nm, potência de 100mW (Laser Duo, MMOptics), exposição radiante de 120J/cm2 e energia de 3J por ponto. Foi aplicado na porção central da lesão, com 30 segundos de irradiação + creme placebo do aciclovir. A variável principal do estudo foi o tempo de resolução da lesão em dias. As variáveis secundárias foram avaliação da recorrência avaliada em meses, a dor avaliada por meio da escala visual analógica (EVA), temperatura local por meio de termômetro digital. Os vírus HSV-1 foram quantificados por meio de PCRq. Todas as avaliações foram realizadas no baseline e após 3 e 7 dias após a aplicação da aPDT ou seu placebo. O questionário Ohip-14 (Oral Health Impact Profile) foi utilizado para avaliar o impacto da saúde bucal após o tratamento. O acompanhamento foi realizado em 1 mês, 6 meses e um ano para o questionário Ohip-14-14 e avaliação da recorrência. Resultados: Não houve diferença no tempo de resolução das lesões em ambos os grupos estudados (p=0,308), na diminuição da dor (p=0862 no primeiro e p=0339 no terceiro dia) e na diminuição da temperatura (p=0,488 no primeiro, p=0,528 no segundo e p= 0,071 no terceiro dia).
