Resumo: Advances in the field of health over the last decades have caused profound changes in the world’s population, which gets older year by year. This demands integrated actions aimed at assisting the elderly who, as they age, become more vulnerable to incapacitating chronic degenerative illnesses. As they become more incapacitated, they need more assistance, making them more dependent on technical and human resources that intend to offer quality of life, given that the aging process cannot be reversed. Thus, geriatric palliative care looks forward to observing illnesses developed in this stage of life as a whole, with comfort and wellbeing as the main focus of its intervention, including family members from the diagnosis until after the death of the elderly patient. With ever smaller families and ever older elderlies, some other actors have been invited to greater involvement, among them the grandchildren. Objective: The objective of this thesis is to comprehend the relationship established between grandparents in palliative care and their caretaker grandchildren. Method: This research is exploratory in nature, using a qualitative approach. It was carried out with seven pairs of grandparents in palliative care and their caretaker grandchildren. The instruments used were a semi-structured interview, sociodemographic survey data, and a field diary. The data were analysed using Bematic Content Analysis with systemic perspective. Results: The research indicated that 1) the relationships among the pairs were already satisfactory since the grandchildren’s childhood, but grew even closer after the grandchildren became caretakers; 2) the tensions due to the hierarchical inversion were smaller than the satisfaction shown in the exchange and intergenerational care; 3) spirituality and social support networks (friends, romantic partners, relatives, professionals) were sought by grandparents and grandchildren to deal with the stressful situation provoked by the processes of aging, illness, and palliative care; 4) the parents’ generation offered support and stimulated these relationships, except in families with triangulation and coalitions; 5) there was reciprocity in care between the two generations in analysis, such that caring for their grandparents was an important factor for the grandchildren. Final Considerations: Whether due to affinity, proximity, gratitude, solidarity, reciprocity, or convenience, the grandchildren show satisfaction in the task of taking care of their grandparents, and also felt taken care of by them. The intermediate generation appeared to favour the grandchildren becoming closer to their grandparents due to the special condition of caregivers. Even though loss of autonomy appears to be the main stress factor experienced by the grandparents, the opportunity of being beside their grandchildren was shown to be more important than the hierarchical inversion provoked by the change in caretaking roles. Therefore, both for the grandparents and for the grandchildren, the positive aspects were more prominent, reinforcing that it is, in fact, affection that sustains these intergenerational relations.

Tal constatação demanda ações integradas que visem a melhor assistir aos idosos que, à medida que envelhecem, tornam-se mais vulneráveis ao acometimento de doenças crônico-degenerativas incapacitantes. Mais debilitados necessitam, cada vez mais, de assistência, ampliando as relações de dependência de recursos técnicos e humanos que tenham por objetivo oferecer qualidade de vida ainda que o processo deletério de envelhecimento funcional não possa ser revertido. Dessa forma, os cuidados paliativos em Geriatria buscam possibilitar que as doenças decorrentes dessa fase da vida sejam olhadas em sua integralidade, tendo o conforto e bem-estar como foco primordial de sua intervenção, integrando familiares desde a fase diagnóstica até após o óbito. Com famílias cada vez menores e idosos casa vez mais longevos, outros atores têm sido convidados a participar de forma mais ativa nesse cuidado, dentre eles, os netos. Objetivo: Esta tese teve como objetivo compreender as relações de cuidado estabelecidas entre avós, em paliação, e seus netos cuidadores. Método: Trata-se de pesquisa de caráter exploratório e abordagem qualitativa, realizada com sete pares de avós em paliação e seus netos cuidadores. Os instrumentos utilizados foram uma entrevista semiestruturada, o questionário dos dados sociodemográficos e o diário de campo. Os dados foram analisados pela técnica da Análise de Conteúdo Temática à luz da perspectiva sistêmica. Resultados: constatou-se que: 1) as relações entre as díades já eram satisfatórias desde a infância, mas puderam ser estreitadas após a assunção dos cuidados; 2) as tensões decorrentes da inversão hierárquica foram menores que a satisfação evidenciada na troca e solidariedade intergeracional do cuidado; 3) a espiritualidade e as redes de suporte social (amigos, namorados, parentes, profissionais) foram buscados por avós e netos para lidar com a situação estressora provocada pelo processo de envelhecimento, adoecimento e a paliação; 4) a geração dos pais, majoritariamente, ofereceu suporte e estimulou para que tais relações se dessem satisfatoriamente, exceto nas famílias em que havia triangulações e coalisões familiares; 5) houve reciprocidade no cuidado entre as duas gerações em análise, sendo fator importante para os netos a cuidarem dos avós. Considerações finais: percebemos que, seja por afinidade, proximidade, gratidão, solidariedade, reciprocidade ou conveniência, os netos demonstraram satisfação na tarefa de cuidar dos avós e também por eles, sentiram-se cuidados. Observamos que a geração intermediária favoreceu para que os netos se aproximassem ainda mais de seus avós diante da condição especial de cuidado. Ainda que a perda de autonomia tenha sido apontada como o principal fator estressor vivenciado pelos avós, a oportunidade de estar ao lado dos netos se mostrou como mais importante que a inversão hierárquica provocada pela mudança nos papéis de cuidado.
