Repercussão da gastrectomia vertical com interposição ileal laparoscópica na função sexual e erétil de portadores de diabetes mellitus tipo 2

In recent decades, the diabetes mellitus became an important clinical and public health problem throughout the world. Currently, there are about 150 million people with diabetes mellitus in the world and that number is expected to double in the year 2025, 90% cases of type 2 diabetes. Until recently, erectile dysfunction was one of the most neglected complications of diabetes mellitus, committing various aspects of life of patients, including the quality of life and interpersonal relationships. It is estimated that the prevalence of erectile dysfunction in diabetic patient sejs of 35 to 75% and that diabetic men to develop 5-10 years before the patients without diabetes mellitus in the corresponding age. This prospective observational study and assessed 50 men with type 2 diabetes mellitus and with body mass index less than 35 kg/m², submitted to laparoscopic vertical gastrectomy with ileal interposition, with the following objectives, in order: 1-identify the incidence of erectile dysfunction in men with diabetes mellitus type 2 and BMI 35 kg/m², before and after 12 months of laparoscopic vertical gastrectomy with ileal interposition; 2-establish if there has been improvement in sexual function within 12 months after the surgical procedure; 3-describe whether the diagnosis of diabetes mellitus interferes in the improvement of sexual function of patients after the surgical procedure. The age ofthe patients ranged from 37 to 69 years (average of 54.4 years). After 12 months of operation there was significant improvement of glycemic control, dyslipidemia, hypertension, in addition to reducing the use of insulin and oral antidiabetics,all with significant difference (p < 0.05). The same occurred with erectile function, with satisfaction with sexual intercourse, orgasmic function, sexual desire and overallsatisfaction with sex life (p < 0.05). It was concluded that: the incidence of erectile dysfunction was 78% before the surgical procedure and 46% after 12 months;within 12 months after surgery there was significant improvement in erectile function, satisfaction with sexual intercourse, orgasmic function, sexual desire and sexual life of patients; patients with the disease more than 15 years have lower response to patients with less disease in relation to the control of diabetes mellitus and sexual dysfunction.
