Resumo: | The characterization and diagnosis of childhood apraxia of speech (CAS) are often discussed in literature. The gold-standard for the diagnosis of this disorder is still the experts’ opinion since the evaluation instruments are limited and not all have established psychometric proprieties. An instrument that evaluates CAS is the Dynamic Evaluation of Speech Motor Skills (DEMSS), which is validated and used for speech pathologists in United State of America. The present research aimed to adapt for Brazilian Portuguese and search for reliability and validity evidence for the DEMSS. The adaptation process involved five stages: 1) translation and retranslation of the instrument; 2) New items (words) selection; 3) analysis by experts judges (speech therapists and linguist); 4) Analysis by non-specialist judges; 5) application of the instrument on a pilot sample. For reliability and criterion validity studies the sample was composed of 136 children between 3:0 and 6:7 years old (114 with typical language acquisition and 22 with disorders of speech sounds). Reliability was assessed by internal consistency (Cronbach's alpha), test-retest and inter e intraobserver analyses. Test-retest reliability was calculated using the Pearson correlation coefficient and the reliability inter and intra-observer Kendall and Kappa analysis were used. Comparisons of sociodemographic variables (type of speech development, sex, type of school and age) and performance on DEMSS-BR constituted the criterion validation data. Student t test for independent samples and univariate analysis were used. The test d Cohen was used to calculate the effect size between the significant results. Data were analyzed using SPSS software version 22, considered significant when p ≤ 0:05. The DEMSS translation was performed by two independent translators. After, the instrument was retranslated and it was sent to one of the authors of the original instrument. Two Speech therapists experts selected 269 words which 94 were considered adequate by experts’ judges. Finally, non-experts judges and pilot sample judged the 94 chosen words and 44 were selected for the instrument. The DEMSS-BR reliability study showed adequate accuracy and stability (> 0.8). The criterion validity showed that DEMSS-BR is able to discriminate children with impaired speech of those with typical development. In addition, girls performed better on the instrument. Type of school and age variables do not appear to influence the test results. DEMSS-BR came to fill a clinical and scientific gap in the Brazilian reality of quantitative instruments, whereas there are few instruments which intend to evaluate severe speech sounds disorder in children. The instrument demonstrated content validity, criterion and presented stable, consistent and accurate measurements, as demand a good test. |

O padrão-ouro para o diagnóstico deste distúrbio ainda é a opinião de especialistas, uma vez que os instrumentos de avaliação são escassos e nem todos apresentam propriedades psicométricas adequadas. Um instrumento que avalia a apraxia de fala infantil é o Dynamic Evaluation Motor of Speech Skills (DEMSS), que é validado e utilizado por fonoaudiólogos nos Estados Unidos. O objetivo deste trabalho foi adaptar para o Português Brasileiro (PB) e buscar evidências de validade e fidedignidade para o DEMSS. O processo de adaptação envolveu cinco etapas: 1) tradução e retradução do instrumento; 2) seleção de novos itens (palavras); 3) análise por juízes especialistas (fonoaudiólogos e linguísta); 4) análise por juízes não especialistas; 5) aplicação em amostra piloto. Nos estudos de fidedignidade e validade de critério, participaram 136 crianças com idades entre 3:0 e 6:7, sendo 114 com aquisição típica de linguagem e 22 com distúrbios dos sons da fala. A fidedignidade foi avaliada por meio da consistência interna (Alpha de Cronbach), teste-reteste e análise inter e intra-avaliadores. A análise teste-reteste foi calculada a partir do teste de Correlação de Pearson. Para as análises inter e intra-avaliadores foram utilizados os testes de Kendall e Kappa. Para validade de critério foram realizadas comparações entre as variáveis sociodemográficas (tipo de desenvolvimento de fala, sexo, tipo de escola e idade) e o desempenho no instrumento denominado para o PB de DEMSS-BR. Utilizou-se o Teste t de Student para amostras independentes e análise univariada. O teste d de Cohen foi utilizado para calcular o tamanho de efeito entre os resultados significativos. Os dados foram analisados através do software SPSS versão 22, considerando-se significativo p ≤ 0.05. A tradução do DEMSS foi realizada por dois tradutores independentes. Após, foi realizada a retradução, que foi enviada a uma das autoras do instrumento original. Foram selecionadas 269 palavras pelas fonoaudiólogas especialistas, das quais 94 palavras foram consideradas adequadas (RVC=1) pelos juízes especialistas. Por fim, os juízes não especialistas e a amostra piloto julgaram as 94 palavras escolhidas e, então, 44 foram selecionadas para o instrumento. O estudo de fidedignidade do DEMSS-BR apresentou precisão e estabilidade adequadas (>0,8). A validade de critério mostrou que DEMSS-BR é capaz de discriminar crianças com alteração de fala daquelas com desenvolvimento típico. Além disso, as meninas apresentaram melhor desempenho no instrumento. Ainda, nesta amostra, as variáveis tipo de escola e idade não influenciaram no desempenho do teste. O DEMSS-BR veio suprir uma importante lacuna clínica e científica na realidade brasileira de testes quantitativos, visto que ainda são escassos instrumentos que se proponham a avaliar os distúrbios dos sons da fala severos e auxiliem no diagnóstico diferencial.
