Privação auditiva unilateral de início tardio: avaliação comportamental e eletrofisiológica

Resumo: Introduction: There are numerous and proven clinical advantages in fitting bilateral hearing aids in cases of bilateral hearing loss: localization of sound, clarity, redundancy, binaural and stereo hearing addition effect. However, it still remains a significant number of unilateral fittings in cases of bilateral hearing loss, with long periods without proper auditory stimulation in the unaided ear. This indicates to be the basis of a phenomenon called "Late-onset auditory deprivation”. Objective: To characterize the auditory performance of individuals with bilaterally symmetrical sensorineural hearing loss fitted with unilateral hearing aid, through behavioral and electrophysiological tests. Methods: Thirty five adults (males and females) participated in the study. All subjects with bilateral symmetric sensorineural hearing loss acquired in adulthood, distributed into three groups: 1) Study Group: 15 unilaterally aided subjects; 2) Unaided Comparison Group: ten unaided subjects; 3) Bilaterally Aided Comparison Group: ten bilaterally aided subjects. All participants underwent the same evaluation protocol constituted of interviews, behavioral tests under headphones (pure-tone audiogram; suprathreshold speech-recognition score test; duration pattern test – DPT; Pediatric Speech Intelligibility Test – PSI; Sentences in Noise Test - LSP), electrophysiological testing (late auditory evoked potential - P300) and behavioral test in sound field (Sentences in Noise Test - LSP). All tests were performed without hearing aids. A descriptive analysis of all variables considered in the study was performed. The level of significance was p<0.05. Results: In the Unilateral Aided Study Group, the analysis of the results between the formal unaided and aided ear showed no differences in interaural performance for the following tests: suprathreshold speech-recognition score test, DPT, PSI and LSP. On the other hand, in the electrophysiological assessment, the P300 waveform revealed a significant (p<0.05) higher latency in the unaided ear compared to the aided ear of the Study Group. Amplitude of the P300 waveform revealed no difference between aided and unaided ears in this group. In both Bilateral Aided and Unaided Comparison Groups the differences were non-significant for the interaural performance (right ear vs. left ear) in all the tests applied. The evaluation of the Sentences in Noise Test (LSP) in sound field revealed a significant performance difference when comparing the Unilateral Aided Study Group to the Unaided Comparison Group and, showing yet, a trend to significance when compared to the Bilateral Aided Comparison Group. Conclusion: Subjects with bilateral hearing loss who are unilaterally aided showed a longer latency of the P300 potential at the unaided ear when compared to the aided ear. In the behavioral assessment under earphones, there were no interaural differences at the suprathreshold speech-recognition score test, DPT, PSI or LSP. In the behavioral assessment in sound field they showed a poorer performance in the Sentences in Noise Test (LSP).

No entanto, há ainda expressivo número de adaptações de próteses auditivas unilaterais, nos casos de perdas auditivas bilaterais. Nestes casos, longos períodos de ausência de estimulação auditiva na orelha não adaptada com prótese auditiva parecem ser a base de um fenômeno denominado “Privação Auditiva Unilateral de Início Tardio”. Objetivo: Caracterizar o desempenho auditivo de indivíduos com perda auditiva bilateral simétrica, adaptados com prótese auditiva unilateral, por meio de testes de avaliação comportamental e eletrofisiológica. MÉTODO: Foram selecionados 35 adultos, de ambos os gêneros, portadores de perda auditiva neurossensorial bilateral simétrica adquirida na fase adulta, distribuídos em três grupos: 1) Grupo Estudo (GE): composto por 15 participantes com adaptação unilateral de prótese auditiva; 2) Grupo Comparação Não Protetizado (GCNP): composto por dez participantes não usuários de prótese auditiva; 3) Grupo Comparação Protetizado (GCP): composto por dez usuários de prótese auditiva bilateral. Todos os participantes foram submetidos ao mesmo protocolo de avaliação composto por: entrevista, testes comportamentais por meio de fones auriculares (audiometria tonal liminar; Índice de Reconhecimento de Fala com palavras monossílabas – IRF; teste padrão de duração - TPD, Pediatric Speech Intelligibility Test – PSI; teste de sentenças no ruído: Listas de Sentenças em Português - LSP); teste eletrofisiológico (potencial evocado auditivo de longa latência - P300); e teste comportamental em campo livre (teste de sentenças no ruído: Listas de Sentenças em Português - LSP). Todos os testes foram aplicados nos três grupos, sem o uso da prótese auditiva. Foi realizada a análise descritiva de todas as variáveis consideradas no estudo. O nível de significância foi de p<0,05. RESULTADOS: No GE, a análise dos resultados entre a orelha que faz uso de prótese auditiva (OP) e a que não faz uso (ONP), não revelou diferenças no desempenho interaural nos testes IRF, TPD, PSI e LSP. Na avaliação eletrofisiológica, observaram-se diferenças (p<0,05) interaurais na latência do componente P300 que revelou ser maior na ONP quando comparada à OP. A amplitude média do componente P300 embora menor, não apresentou diferença. Não foram reveladas diferenças no desempenho interaural (orelha direita versus orelha esquerda), tanto no GCP como no GCNP nos testes aplicados. Os resultados no teste de Sentenças no Ruído (LSP) em campo livre revelaram diferenças (p<0,05) entre o desempenho do GE e o GCNP e tendência a significante entre o GE e o GCP. CONCLUSÃO: Indivíduos com perda auditiva bilateral que fazem uso de prótese auditiva unilateral apresentaram maior latência do potencial P300 na orelha que não faz uso de prótese auditiva quando comparada à orelha que faz uso. Na avaliação comportamental por meio de fones, não apresentaram diferenças interaurais nos testes IRF, TPD, PSI e LSP.
