Sistematização da assistência de enfermagem: instrumento para o processo de trabalho do enfermeiro na gerência da assistência

Resumo: | This study contemplates Care System Nursing (NCS) as a clinical tool for the working process of the nurse in the management of assistance. Has the general objective to analyze the process of managerial work of nurses face the implementation of the NCS and specific, identifying facilitators and challenging mechanisms for implementation of NCS in the practice of nursing work, as well as analyze the managerial models and care that guide the praxis experienced by nurses and characterize their knowledge, analyzing their academic preparations and training. This is a qualitative approach, grounded in the theoretical and methodological framework of Hermeneutics-Dialectics. This study consisted of 32 nurses working in the profession Hospital three institutions, public education and public private legal natures of Alfenas-MG. Was approved by the Research Ethics Unifal-MG with Opinion under n. 139.518. Was used for data collection technique by focus group participants nurses. Content analysis, according to Minayo, applied to the discourses identified three major empirical categories. In class facilities and challenges for SAE in the daily work of nurses is encompassed subcategories: - teamwork; operationalization of SAE and people management. In the category experienced conservative models, we identified two subcategories: - centralization of power and standoff in care: X comprehensive care disease. And finally, in the category conformation of knowledge of nurses for practice in SAE, we identified two subcategories: - dichotomy between theory and practice and management knowledge of nurses. The analytical categories were based on the three theoretical axes, and, in the structural dimension, we consider the proximity of the empirical context of the transformations determined by process management and care nurses' work in relation to the planning and organization of the data and their actions existing management and care models. In particular dimension, the teacher praxis that outlines the training of nurses for their professional practice, trying to adjust to the managerial practice knowledge in a manufacturing perspective, meeting the demands of professional and life. In singular dimension as space intercessor between education and the reality of health institutions, between theory and practice, between the subjects of this investigation, the nurses, to the conformation of the Professional Practice Law and their managerial skills and solidification of existing. The results showed that the successful operation of SAE is through a mutual development, in which people are valued by the organization to the extent that contribute effectively to its development, as well as organizations are valued by people in that offer them concrete conditions for its growth. It is therefore necessary to adjust the SAE according to the reality of each hospital, contemplating resistance by some nurses who refuse the same as legal support profession, breaking the taboo that this instrument came to add and invigorating autonomy professional. |

Tem como objetivo geral analisar o processo de trabalho gerencial do enfermeiro face à implementação da SAE, e específicos, identificar mecanismos facilitadores e desafiadores para a implementação da SAE na prática de trabalho do enfermeiro, bem como analisar os modelos gerencial e assistencial que norteiam à práxis vivenciada pelos enfermeiros e caracterizar seus conhecimentos, analisando seus preparos e formação acadêmica. Trata-se de uma abordagem qualitativa, ancorada no referencial teórico-metodológico da Hermenêutica-Dialética. Este estudo constituiu-se de 32 enfermeiros que atuam na profissão de três Instituições Hospitalares de naturezas jurídicas pública, pública de ensino e privada do município de Alfenas-MG. Foi aprovado pelo Comitê de Ética em Pesquisa da Unifal-MG com o Parecer sob o n. 139.518. Utilizou-se na coleta de dados a técnica por grupo focal com os enfermeiros participantes. A análise de conteúdo, de acordo com Minayo, aplicada sobre os discursos identificou três grandes categorias empíricas. Na categoria facilidades e desafios para a SAE no cotidiano de trabalho do enfermeiro, englobaram-se as subcategorias: - trabalho em equipe; operacionalização da SAE e gestão de pessoas. Na categoria modelos conservadores vivenciados, identificaram-se duas subcategorias: - centralização de poder e impasse na assistência: doença X integralidade do cuidado. E, por último, na categoria conformação de saberes do enfermeiro para a SAE na práxis, identificaram-se duas subcategorias: - dicotomia entre teoria e prática e saberes gerenciais do enfermeiro. As categorias analíticas foram fundamentadas nos três eixos teóricos, sendo que, na dimensão estrutural, considera-se a proximidade dos dados empíricos ao contexto das transformações determinadas pelo processo de trabalho gerencial e assistencial do enfermeiro em relação ao planejamento e à organização de suas ações e aos modelos gerenciais e assistenciais existentes. Na dimensão particular, a práxis formadora que delineia a formação do enfermeiro para sua prática profissional, procurando ajustar os saberes gerenciais à práxis, numa perspectiva transformadora, atendendo às demandas dos profissionais e da vida. Na dimensão singular, como espaço intercessor entre o ensino e a realidade das instituições de saúde, entre a teoria e a prática, entre os sujeitos desta investigação, os enfermeiros, para a conformação da Lei do Exercício Profissional e de suas competências gerenciais e para a solidificação das já existentes. Os resultados apontaram que o sucesso da operacionalização da SAE se dá por meio de um desenvolvimento mútuo, em que as pessoas são valorizadas pela organização na medida em que contribuem efetivamente para o seu desenvolvimento, assim como as organizações são valorizadas pelas pessoas na medida em que lhes oferecem condições concretas para o seu crescimento.
