Análise comparativa de técnicas de apicectomia, retrocavitação e retrobturação: estudo in vitro

Resumo: Different techniques of apicectomy, retrograde preparation and root obturation were evaluated through an experimental in vitro study of apical sealing capability. Sixtyfour permanent single-rooted teeth were endodontically treated. The roots were then submitted to a root-end resection at a distance of 3 mm from the dental apex. The roots were randomly divided into two groups: 1) root-end resection in 45°; 2) root-end resection in 90°. Two types of instruments to retrograde preparation were used, nº 2 carbide bur and ultrasonic retro-tip S12/90D. Then, the entire external root surfaces of the specimens were covered with two-coats of nail polish except for the sectioned surface. Two types of retrograde filling materials were used: I) mineral three-oxide aggregate (MTA) and II) amalgam (AA). After the root-end fillings were made, the specimens were immediately immersed in a 0. 2% rhodamine B solution during 24 hours. After that, the specimens were washed in tap water for the same time and dried in environment temperature. Slices 2mm far from apex were cut and then photographed to analyze apical dye penetration and staining area (mm2) using a software (Image Tool 3. 0) in a personal computer. The results were analyzed as a ratio of dye penetration, to eliminate possible influence of differences between specimens root sizes. Data were submitted to statistical analysis using ANOVA test and post-hock LSD test, with significance of 95% (α = 0. 05).Correlation between dependent and independent variables, as between the the two examiners, Pearson’s test were assessed. None group was capable to completely seal apical stainig. Some combinations of the techniques used showed better results (p<0. 05), generally when the apicectomy was made at 90º by ultrasonic retro-tip and using MTA. Within the limitations of this study, it was concluded that the most important factor in dye penetration seems to be the retrobturator material, followed by the angle of apicectomy and retrograded instrument, respectively.

A amostra foi constituída por 64 dentes unirradiculares tratados de maneira endodôntica, sendo metade deles aleatoriamente apicectomizados a 3mm da extremidade apical em 45º e os remanescentes com apicectomia em 90º. Para cada tipo de apicectomia foram realizados dois tipos de retrocavidade: com broca de aço esférica nº 2 ou com retroponta ultra-sônica S12/90D. Ao concluir esses procedimentos, os dentes foram isolados com duas camadas de esmalte de unhas, excetuando-se a retrocavidade e as suas margens. Para conclusão dos tratamentos, em cada subgrupo (apicectomia/retrocavidade) dois tipos de materiais odontológicos foram empregados: retrobturação com MTA ou com amálgama, sendo, então, os espécimes imediatamente imersos no corante (Rodamina B a 0,2%) por 24 horas. A infiltração apical foi avaliada, com auxílio do programa ImageTool 3. 0, a partir de fatias transversais a 2mm da extremidade apical, de cada unidade amostral, determinando-se a área de dentina infiltrada pelo corante ao redor das retrobturações, em milímetros quadrados. Este resultado, em área, foi transformado em uma medida relativa do percentual de área infiltrada, para ser avaliado sem influência de variações nas áreas das raízes nos diferentes grupos. Os dados foram submetidos à análise estatística (ANOVA e pós-teste LSD), com nível de significância de 95%. Para constatar a correlação entre a variável dependente e as independentes, bem como entre os dois avaliadores, utilizou-se o teste de correlação de Pearson. Como resultados, observou-se que nenhum dos grupos foi capaz de selar completamente a interface da retrocavidade.O tipo de seccionamento apical, de preparo retrocavitário e o material odontológico usado para a retrobturação mostraram-se fatores significantes em algumas combinações (p<0. 05), salientando-se que, em geral, menores valores de infiltração foram observados para a apicectomia a 90º, retrocavitação com retroponta ultra-sônica e quando a retrocavidade foi obturada com MTA. Assim, com base na metodologia empregada, foi possível concluir que as diferentes formas de seccionamento apical, preparo retrocavitário e material odontológico utilizado para a retrobturação influenciam no grau de infiltração, sendo que o material mostrou-se como o fator mais influente, seguido da angulação da apicectomia e do instrumento utilizado na retrocavitação.Made available in DSpace on 2011-12-27T14:14:08Z (GMT).
