The purpose of this dissertation aims to analyse how people with arterial hypertension and health care professionals of the Family Health Center Ibirajá, in the city of Guarulhos - SP build and experience the relationship with the ESF. The study sought to interpret the perceptions about the therapeutic relationship, looking for the meanings of both relationship unveiling for patient, as well as for health care professional, in relation health, illness, care and life production. Conceptual references of research were: Primary Health Care, Clinic, Family Health Strategy, and Health Policy, Relationship, found in contemporary authors of Healthcare and Social Work. It is a qualitative research approach was through the case study. The survey methodology was structured by bibliographic and field research. The data collection took as semi-structured interviews instrument. For the interpretation of the data content analysis was used. The subject of the research was: health care professionals, patient on regular monitoring, patient on irregular monitoring. The results of the analysis showed that the construction of relationship the between the subject and health care professionals occurs in a variety of ways. For users in the regular monitoring via relationship of humanized, interpersonal relationship and trusts that are established with health care professionals. We note the absence or fragility in building relationship of patient irregular monitoring, to the extent that exhibit resistance to assume the co-responsibility in health-disease process. The professionals identify the relationship building when they interfere in the production process of living and self-care of the subject and the deconstruction of the relationship when they fail to ensure the principle of completeness in the course of treatment
