Resumo: | Background. Shear wave elastography is a noninvasive imaging modality for quantitative assessment of tissue stiffness. Objective: to evaluate the association between shear wave elastography parameters and renal alteration by color Doppler in renal transplant patients. Methods: prospective cross-sectional study. Renal transplant patients followed in the Nephrology Service, aged over 18 years and with indication for ultrasound. Multivariate analysis was performed using Binary Logistic Regression by Generalized Linear Models to identify the independent variables associated with the Doppler resistance index. Results. We evaluated 57 patients treated in the Nephrology Outpatient Clinic. The mean age of the patients was 47 ? 16.4 years, 40 (70.2%) males and 46 (80.7%) self-reported as white. The medians of glomerular filtration rate and creatinine were respectively 15.6 (7.8 - 28.3) and 3.57 (2.2 - 6.0) ?mol / l. There was a high prevalence of patients with severe to terminal renal function (n = 41, 72%). The average kidney was 11.7 ? 1.5cm. Most of the evaluated patients received deceased donor kidney (n = 54, 94.7%). The results of the analysis of agecontrolled generalized linear models (OR 1.377, 95% CI 1.182 to 1.603; p <0.001) identified the parameters of elastography, velocity and resistance (OR = 17.12, 95% CI 1.137 to 257.83), p = 0.040 and OR 1.204, 95% CI 1.020 to 1.422, p = 0.028, respectively) as independent factors associated with renal impairment. Conclusion. The parameters of elastography, velocity and resistance, when adjusted for age, showed a positive association with renal alteration, assessed by color Doppler. |

Objetivo: avaliar a associa??o entre os par?metros da elastografia shear wave e a altera??o renal por meio de doppler colorido em pacientes transplantados renais. Metodologia: estudo transversal, prospectivo. Foram inclu?dos nesse estudo pacientes transplantados renais acompanhados em Servi?o de Nefrologia, com idade superior a 18 anos e com indica??o de realiza??o de ultrassonografia. Foi realizada an?lise multivariada usando a Regress?o Log?stica Bin?ria por Modelos Lineares Generalizados para identificar as vari?veis independentes associadas ao ?ndice de resist?ncia do Doppler. Resultados: Foram avaliados 57 pacientes atendidos em ambulat?rio de Nefrologia. A m?dia de idade foi de 47 ? 16,4 anos, sendo 40 (70,2%) do sexo masculino, e 46 (80,7%) autodeclarados brancos. As medianas da taxa de filtra??o glomerular e creatinina foram respectivamente de 15,6 (7,8 ? 28,3) e 3,57 (2,2 ? 6,0) ?mol/l. Observou-se elevada preval?ncia de pacientes com fun??o renal grave a terminal (n = 41, 72%). O tamanho m?dio do rim foi de 11,7?1,5cm. A maioria dos pacientes avaliados receberam rim de doador falecido (n = 54, 94,7%). Os resultados da an?lise de modelos lineares generalizados, controlados para idade, identificaram os par?metros da elastografia, velocidade e resist?ncia (OR = 17,12, IC95% 1,137 a 257,83, p=0,040 e OR 1,204, IC95% 1,020 a 1,422, p = 0,028, respectivamente) como fatores independentes associados ? altera??o renal.
