Resumo: This study aims to determine the possible association between family maltreatment and dissatisfaction with body image and obesity in adolescence. This research was carried out by two studies: the first, observational and cross-sectional in a sample of 201 adolescents, 10-19 years registered in the Bolsa Família Program and monitored by the Nutrition Service of a health unit in the municipality of Rio de Janeiro in 2008-2009; and the second, with 1628 students from 9th grade of elementary school of public schools in the municipality of Rio de Janeiro, in 2007. We investigated the prevalence of family abuse, dissatisfaction with body image and obese in adolescents. For this, we performed anthropometric measurements for the determination of obesity according to 3 indexes: the first was to assess excess weight using the Body Mass Index (BMI), the second was to estimate body fat from the formula proposed by Slaughter et al. for triceps and subscapular skinfolds and the third was to measure central obesity using the Waist-to-Height Ratio. Family maltreatment was investigated by means of instruments - Conflict Tactics Scales Form R (CTS1) to assess the domestic conflict in the relationship between parents and your children; the scale of psychological violence against adolescents and questions that characterized the types of abuse: physical aggression and verbal mistreatment. Body image was assessed by the scale of body satisfaction by areas for teens. Besides these, other covariates were investigated that could influence the investigation as sociodemographic information, sexual maturity, relationship with the family and the school, physical activity and sedentary activities. Associations between variables were expressed as odds ratios and their respective 95% confidence intervals estimated via logistic regression. All analyzes were conducted for boys and girls separately. The results were presented using two articles. The first article, regarding the association between physical and psychological violence and dissatisfaction with body image in adolescents showed that girls who have suffered psychological violence were 4.2 more likely to have dissatisfaction with body image than those who did not suffer violence psychological. For boys, there was no association between family abuse and dissatisfaction with body image. The second article was relevant to the relationship between family maltreatment and obesity and found that the was no association between family maltreatment and obesity for males. However, girls who have suffered physical aggression had lower prevalence of overweight than those who did not suffer physical aggression (OR = 0.449, CI = 0.212 to 0.951). It concludes that, in adolescence, girls victims of family maltreatment were dissatisfied with their body image and were less likely to have obesity

Essa investigação foi realizada por meio de dois estudos: o primeiro, observacional e de corte transversal, em uma amostra de 201 adolescentes, de 10 a 19 anos, cadastrados no Programa Bolsa Família e monitorados pelo Serviço de Nutrição de uma unidade de saúde do município do Rio de Janeiro no período de 2008 a 2009; e o segundo, com 1628 alunos, do 9º ano do ensino fundamental da rede pública de ensino do município do Rio de Janeiro, no ano de 2007. Foram averiguadas as prevalências de maus-tratos na família, de insatisfação com a imagem corporal e de obesidade dos adolescentes. Para isso, foi realizada avaliação antropométrica para a determinação da obesidade segundo 3 índices antropométricos: o primeiro avaliou o excesso de peso por meio do Índice de Massa Corporal (IMC), o segundo estimou a gordura corporal a partir da fórmula proposta por Slaughter et al. para dobras cutâneas tricipital e subescapular e o terceiro mediu a gordura central utilizando a razão cintura/altura. A violência familiar foi investigada por meio dos instrumentos Conflict Tactics Scales Form R (CTS1) para avaliar os conflitos intrafamiliares no relacionamento entre pais e filhos; escala de violência psicológica contra adolescentes e perguntas para caracterizar tipos de maus-tratos: a agressão física e o mau-trato verbal. A imagem corporal foi avaliada pela escala de satisfação corporal por áreas para os adolescentes. Além dessas, foram investigadas outras covariáveis como informações sociodemográficas, a maturação sexual, a relação com a família e com a escola, a atividade física e as atividades sedentárias. Para verificar as associações existentes entre os maus-tratos familiares e a insatisfação com a imagem corporal e a obesidade foram realizadas regressões logísticas e apresentadas as razões de chances e seus respectivos intervalos de 95% de confiança (IC95%). Todas as análises foram elaboradas para meninos e meninas separadamente. Os resultados foram apresentados por meio de dois artigos. O primeiro artigo, referente à associação entre as violências física e psicológica e à insatisfação com a imagem corporal nos adolescentes mostrou que as meninas que sofreram violência psicológica tinham 4,2 mais chances de terem insatisfação com a imagem corporal do que aquelas que não sofreram violência psicológica. Para os meninos, não foi encontrada associação entre a violência familiar e a insatisfação com a imagem corporal. O segundo artigo foi pertinente à relação entre os maus-tratos na família e a obesidade e verificou que não havia associação entre os maus-tratos na família e a obesidade para o sexo masculino. Porém, meninas que sofreram agressão física tiveram menor prevalência de excesso de peso do que aquelas que não sofreram agressão física (OR=0,449; IC 95% 0,212-0,951).
