This research emerged on the clinic practiced by us on a CAPSad (Alcohol and drug psychosocial care center), aiming on the verification of the possible treatment for psychotic alcohol and other drugs users subjects. This thesis unfolds in two parts: the first part deals with the constitution of our work agenda on the CAPSad (Alcohol and drug psychosocial care center), titled by a teammate as "psychosis agenda"; the second part is dedicated to the public health agenda for users of alcohol and other drugs. What can be seen is that an agenda does not occur independent of the other, that is, the public guidelines on the alcohol and other drugs field directs the kinds of offers possible for the treatment of drugs users, being them psychotics or not. In such a way that we raised the hypothesis of that precisely the Harm Reduction the political guideline for the field of alcohol and other drugs supported by the psychosocial paradigm allowed the entry of the mental health analyst's discourse. Since 2009, with the widespread "epidemic" of crack, and, especially from the Crack plan, it is possible to win in 2011, we have been watching an increasing prohibitionist paradigm, and the growing demand for public funding from the Therapeutic Communities (CTs), with the concomitant disinvestment in mental health and its equipment aimed at the psychosocial attention of drug users. In this thesis we point out the harmful consequences of this change of direction that has been taking place in the field of alcohol and other drugs, and we discuss the alliance between the discourse of science and religious morality as an offer of treatment to drug users, which raises ethical questions. Regarding the treatment of subjects who are psychotics drug users, having as reference the suppletions clinic proposed by Lacan, we support the hypothesize that the direction of the treatment of the enjoyment of drugs on the psychosis also goes, as in neurosis, in the direction of desire, only it is a desire arising from the invention of a unique mooring for each subject. A desire that is not supported by the phallus, but which, analogously to the desire in neurosis, restrains the death drive
