Introduction: Amyotrophic Lateral Sclerosis is a neurodegenerative disease characterized by progressive loss of upper and lower motor neurons. In recent years, studies have shown that in addition to muscle weakness, difficulties in speech, swallowing or breathing, the patient may also experience language difficulties and behavioral changes. For an appropriate cognitive-behavioral evaluation of these patients, screening tests adapted to their motor difficulties were created and in Brazil, to date, there are no specific evaluations for this population. Objective: To translate and adapt the “Edinburgh Cognitive and Behavioral Amyotrophic Lateral Sclerosis Screen” (ECAS) screening test to the Portuguese language. Methodology: The instrument changes were based on the International Test Commission standards, the Translation and Adaptation Guide provided by the protocol authors and national references. After the translation stages, some subtests underwent cross-cultural adaptations, with content validation, verbal comprehension analysis and semantic equivalence. For the cultural adaptation stage, a pilot group of 46 individuals, 20 men and 26 women, with no history of neurological or psychiatric problems was created. Results: The pilot group had a mean age of 52.63 ± 13.88 years and a mean educational level of 13.83 ± 5.10 years. Nine of the fifteen subtests of the instrument were transculturally adapted: subtest of Naming, Spelling, Alternation, Inhibition, Verbal Fluency, Memory (Immediate recall, Delayed recall, Delayed recognition). Conclusion: The protocol was translated and adapted to the Brazilian Portuguese with the denomination Edimburgo – Rastreio cognitivo-comportamental em Esclerose Lateral Amiotrófica (ECAS- BR).
