Introduction: In Brazil, the Family Health Strategy (ESF) should be the preferred contact of SUS users, functioning as the main gateway and communication center of the entire health care network. Evaluation has become an important tool for change so that health services meet minimum standards of quality and promote a renewal of practices and work culture. In this perspective, the National Program for the Improvement of Access and Quality of Primary Care (PMAQ-AB) has the objective of inducing the expansion of access and improvement of the quality of Primary Care (AB), to allow greater transparency and effectiveness of actions governmental organizations directed to the service. Objective: To evaluate the organization of the service and work process of the Family Health Strategy in Mato Grosso through the results of the second PMAQ-AB cycle. Methodology: This is a cross-sectional descriptive study, based on secondary data from the external evaluation phase - organization of the service and work process of the FHS teams - module II, of the second PMAQ-AB cycle developed in Mato Grosso in the year 2014. Participated 490 teams of the Family Health Strategy distributed in the 137/141 municipalities that joined the second cycle of PMAQ-AB. The descriptive analysis was based on absolute and relative frequencies of the variables selected and consolidated in quality parameters. The results were presented by the 16 health regions. Results: The Middle Araguaia region stood out as the best evaluated region. On the other hand, the majority (13/16) of the health regions were most frequently classified as "poor" quality - Southwest Matogrossense, Araguaia Xingu, Matogrossense West, Araguaia Norte Karajá, Norte Norte, Centro Norte, Alto Tapajós, Sul Matogrossense, Arinos Valley, Peixoto Valley, North Matogrossense, Matogrossense Northwest, Araguaia Herons. The subdimensions that achieved the best results were: Territorialization and reference population of the primary care team, Child Health Care and Health in School Program. Conclusion: The analysis of some subdimensions makes explicit the need for improvements that compete with the management, however, there are decisions and strategic actions that the teams have autonomy in the decision process. The unsatisfactory performance of most regions of the state and the large quality difference between them suggests that resources are not used adequately and / or are insufficient for the proper organization and work process of AB teams.
