Estresse oxidativo na hepatotoxicidade aos medicamentos anti-tuberculose

Resumo: | O tratamento da tuberculose envolve uma associação de fármacos com ocorrência de interações entre si e outros fármacos. Seus efeitos adversos mais frequentes estão relacionados à hepatotoxicidade. A biotransformação de fármacos pode resultar na formação de metabólitos reativos capazes de produzir danos celulares, e tem sido considerada como um importante processo da patogênese de algumas formas de hepatotoxicidade. Objetivos: Avaliar a participação do estresse oxidativo em pacientes com hepatotoxicidade com medicamentos anti-tb. Metodologia: Este estudo é tipo Caso Controle e para a Revisão sobre hepatotoxicidade como reação adversa aos medicamentos anti-Tb, foi realizado uma ampla busca no Portal da Biblioteca Virtual da Saúde (BVS) visando a disponibilidade de literatura em língua portuguesa e inglesa para encontrar artigos publicados até dezembro de 2014. Para a análise das enzimas antioxidantes foram incluídos os pacientes atendidos no HUJBB no Ambulatório de Referência Secundária de Clínica de Pneumologia e internados na Clínica de Pneumologia, e os pacientes atendidos no Ambulatório da Unidade Básica de Saúde do Guamá em Belém do Pará. Resultados: Conforme a revisão da literatura sobre Reações adversas a medicamentos antituberculosos, realizada em 2015 foi constatado que o comprometimento hepático está entre as reações adversas de maior incidência associada aos medicamentos anti-tuberculose no cenário brasileiro, e que as reações adversas durante o tratamento da tuberculose são um dos principais fatores associa- dos ao abandono. Conforme as análises das enzimas antioxidantes, Glutationa nos grupos Controle, grupo com hepatotoxicidade (PCH) e o grupo sem hepatotoxicidade com medicamentos anti-TB (PCT) apresentaram medianas de níveis de glutationa com valores de 221 nmol/mL, 227 nmol/mL e 236 nmol/mL, respectivamente. As distri- buições da Catalase nos grupos controle, grupo com hepatotoxicidade (PCH) e o grupo sem hepatotoxicidade com medicamentos anti-TB (PCT), apresentaram medianas de atividade de catalase com valores de 213 nmol/mL, 319 nmol/mL e 2.035 nmol/mL, respectivamente. A mediana dos níveis de antioxidantes da glutationa para o grupo PCT foi a maior, e não se observou uma diferença estatisticamente significante ao aplicar o teste ANOVA. As distribuições da atividade da Catalase na população de pacientes com TB nos grupos que evoluíram com hepatotoxicidade (PCH) e os que evoluíram sem hepatotoxicidade (PCT) foram aumentadas quando comparados com os voluntários saudáveis. Particularmente, observou-se uma diferença estatisticamente significante da atividade da catalase no grupo (PCT) em relação aos demais grupos. Conclusão: Os resultados sugerem que a hepatotoxidade não está somente associada às enzimas antioxidantes e novas análises com mais variáveis explanatórias devem ser realizadas para entender este fenômeno. |

The most common side effects are related to hepatotoxicity. The biotransformation of drugs may result in the formation of reactive metabolites that can produce cellular damage which has been considered as an important process in the pathogenesis of hepatotoxicity. Goals: Evaluate the role of oxidative stress in patients with hepatotoxicity that have used anti-TB drugs. Methodology: To reach the first goal a Review of hepatotoxicity as an adverse reaction to anti-TB drugs was performed. Such an review was done through a wide search in the Portal of the Virtual Library of Health that targeted Portuguese and English literature to find papers pub- lished until December 2014. On the other hand, a Case Control study was performed to reach the second goal. For the analysis of antioxidant enzymes, patients treated at HUJBB in Secondary Reference Clinic of Pulmonology Clinic and admitted in línica of Pulmonology, and the patients seen at the Basic Health Unit Guamá in Belém were included. Results: As for the literature review, it was found that hepatic impairment is among the highest incidence of adverse reactions associated with anti-tuberculosis drugs in the Brazilian scene and adverse reactions during treatment of tuberculosis are one of the main factors associated with therapy abandonment. Regarding the analysis of antioxidant enzymes, the analysis of glutathione in the control group with hepatotoxicity (PCH) and the group without hepatotoxicity with anti-TB drugs (PCT) achieved median glutathione levels of 221 nmol / ml, 227 nmol / ml, and 236 nmol / ml, respectively. The distribution of catalase in the control group with hepatotoxicity (PCH) and the group without hepatotoxicity with anti-TB drugs (PCT) showed medians of catalase activity with values of 213 nmol / ml, 319 nmol / ml and 2035 nmol / ml, respectively. The median levels of antioxidants glutathione to the PCT group was the largest. However, glutathione levels were not statistically significant when applying the ANOVA test. Distributions of catalase activity in the population of TB patients in the group who developed hepatotoxicity (PCH) and that evolved without hepatotoxicity (PCT) were larger when compared to healthy volunteers. In particular, there was a statistically significant difference in catalase activity in the group (PCT) compared to the remaining groups.
