Resumo: Obesity is one of the biggest public health problems nowadays and has expressively affected children. Childhood obesity has several consequences, including an increased risk of chronic diseases development during adolescence and adulthood. One of the factors strongly associated with weight gain in children is the excessive sugar ingestion due to the high consumption of products target at children, which present high energy content and low nutritional value. Sugary drinks (fruit drinks, soft drinks) are pointed out as the main source of sugar consumed by children. In this sense, several public policies have been proposed to reduce sugar consumption. The aim of this thesis was to evaluate the effect of the front of pack (FOP) labeling schemes and the food reformulation as strategies for reducing the consumption of sugar by children. For that, five studies were carried out between October/2016 and April/2018, with 6-12 years old children, and in three of them adults were also involved. All the studies were approved by the Brazilian Committee of Ethics in Research (Plataforma Brasil - CAAE 55023416.0.0000.5285). The first two studies evaluated the inclusion of FOP schemes (non-directive scheme: guideline daily amounts (GDA), semi-directive: traffic light system (TLS) and directive: nutritional warnings) as a strategy to reduce the healthfulness perception and modify positive emotional response related to unhealthy foods. The studies 3 and 4 evaluated the food reformulation (sugar reduction) as a strategy to reduce children sugar consumption, by determining the sugar reduction threshold and comparing children hedonic and sensory perception on sugar reduced drinks using different methods (gradual and sequential reduction). The last study evaluated the combined effect of FOP schemes and sugar reduction on children and adults? food choice under different experimental conditions. The results revealed that for parents, food products containing nutritional warnings were significantly classified as less healthy. On the other hand, FOP schemes had a limited effect on the children healthfulness perception and the emotional response. However, the directive and semi-directive FOP schemes had higher effect compared to the non-directive (GDA) in both studies. The results of the sugar reduction threshold determination showed that children were less sensitive to changes in sugar concentrations than adults. The gradual reduction was the most appropriate reformulation strategy because it caused less changes in children? hedonic and sensory perception, suggesting that this is a viable policy for reducing sugar consumption. Taking into account the combined effect of sugar reduction and FOP schemes, the results indicated that nutritional warnings had a higher effect on children and adults? expectation; however, after tasting the product, the flavor was determinant for the product choice. The low choice of products with higher levels of sugar reduction suggests the combination of gradual sugar reduction and the nutritional warnings as appropriate strategy, because they are complementary actions. The inclusion of directive FOP schemes would facilitate the identification of unhealthy foods by children and adults, and the gradual reduction of sugar would reduce the sugar content of products available in the market, contributing to the reduction of consumption, and modification of the population food habits in long-term.

A obesidade infantil acarreta consequ?ncias como o aumento do risco de desenvolvimento de doen?as cr?nicas durante a adolesc?ncia e vida adulta. Um dos fatores fortemente associados ao ganho de peso em crian?as ? o consumo excessivo de a??car decorrente da elevada ingest?o de produtos com apelo infantil, os quais apresentam alto teor energ?tico e baixo valor nutricional. As bebidas a??caradas (bebidas de fruta, refrigerantes) s?o apontadas como a principal fonte de a??car consumida pelas crian?as. Nesse sentido, diversas pol?ticas p?blicas tem sido propostas para a redu??o do consumo de a??car. O objetivo da tese foi avaliar o efeito da inclus?o da rotulagem nutricional frontal (FOP) e da reformula??o de alimentos como estrat?gias para a redu??o do consumo de a??car pelo p?blico infantil. Para tal, cinco estudos foram realizados no per?odo entre outubro/2016 e abril/2018 com crian?as na faixa et?ria de 6-12 anos, sendo que em tr?s destes estudos houve a participa??o de adultos. Todos os estudos foram aprovados pelo comit? de ?tica em pesquisa (Plataforma Brasil - CAAE 55023416.0.0000.5285). Os dois primeiros estudos avaliaram a inclus?o de modelos FOP (modelo n?o diretivo: valor di?rio (VD%), semi-diretivo: sem?foro nutricional (SN) e diretivo: alertas nutricionais) como estrat?gia para reduzir a percep??o de saudabilidade e modificar a resposta emocional em rela??o aos alimentos n?o saud?veis. Os estudos 3 e 4 avaliaram a reformula??o de alimentos (redu??o de a??car) como estrat?gia para reduzir o consumo de a??car pelo p?blico infantil, a partir da determina??o do limiar de redu??o e da compara??o da resposta hed?nica e sensorial de crian?as em rela??o ?s bebidas reduzidas em a??car por diferentes m?todos (redu??o gradual e sequencial). O ?ltimo estudo, avaliou o efeito combinado da inclus?o de modelos FOP e da redu??o do teor de a??car na escolha de alimentos sob diferentes condi??es experimentais. Os resultados mostraram que para os pais, os alimentos contendo alertas nutricionais foram classificados como menos saud?veis (p<0.05). J? entre as crian?as, os diferentes modelos FOP apresentaram efeito limitado na percep??o de saudabilidade e da resposta emocional. No entanto, os modelos diretivo e semi-diretivo apresentaram efeito superior ao n?o diretivo (VD%) em ambos os estudos. Os resultados da determina??o do limiar de redu??o de a??car revelaram que as crian?as tiveram menor sensibilidade ?s modifica??es nas concentra??es de a??car do que os adultos. A redu??o gradual foi a estrat?gia de reformula??o mais adequada, pois causou menor modifica??o na percep??o hed?nica e sensorial das crian?as, sugerindo que esta ? uma pol?tica vi?vel para a redu??o do consumo de a??car. Em rela??o ? avalia??o do efeito combinado da redu??o de a??car e da inclus?o de modelos FOP, os resultados indicaram que os alertas nutricionais apresentaram maior efeito na expectativa de adultos e crian?as em rela??o ? sele??o dos produtos; por?m, durante o consumo, o sabor teve papel determinante na escolha dos participantes do estudo. A baixa escolha dos produtos com maiores n?veis de redu??o de a??car sugere que a combina??o da redu??o gradual de a??car e a inclus?o dos alertas nutricionais pode ser a estrat?gia adequada, uma vez que s?o a??es complementares. description Obesity is one of the biggest public health problems nowadays and has expressively affected children. Childhood obesity has several consequences, including an increased risk of chronic diseases development during adolescence and adulthood. One of the factors strongly associated with weight gain in children is the excessive sugar ingestion due to the high consumption of products target at children, which present high energy content and low nutritional value. Sugary drinks (fruit drinks, soft drinks) are pointed out as the main source of sugar consumed by children. In this sense, several public policies have been proposed to reduce sugar consumption. The aim of this thesis was to evaluate the effect of the front of pack (FOP) labeling schemes and the food reformulation as strategies for reducing the consumption of sugar by children. For that, five studies were carried out between October/2016 and April/2018, with 6-12 years old children, and in three of them adults were also involved. All the studies were approved by the Brazilian Committee of Ethics in Research (Plataforma Brasil - CAAE 55023416.0.0000.5285). The first two studies evaluated the inclusion of FOP schemes (non-directive scheme: guideline daily amounts (GDA), semi-directive: traffic light system (TLS) and directive: nutritional warnings) as a strategy to reduce the healthfulness perception and modify positive emotional response related to unhealthy foods. The studies 3 and 4 evaluated the food reformulation (sugar reduction) as a strategy to reduce children sugar consumption, by determining the sugar reduction threshold and comparing children hedonic and sensory perception on sugar reduced drinks using different methods (gradual and sequential reduction). The last study evaluated the combined effect of FOP schemes and sugar reduction on children and adults? food choice under different experimental conditions. The results revealed that for parents, food products containing nutritional warnings were significantly classified as less healthy. On the other hand, FOP schemes had a limited effect on the children healthfulness perception and the emotional response. However, the directive and semi-directive FOP schemes had higher effect compared to the non-directive (GDA) in both studies. The results of the sugar reduction threshold determination showed that children were less sensitive to changes in sugar concentrations than adults. The gradual reduction was the most appropriate reformulation strategy because it caused less changes in children? hedonic and sensory perception, suggesting that this is a viable policy for reducing sugar consumption. Taking into account the combined effect of sugar reduction and FOP schemes, the results indicated that nutritional warnings had a higher effect on children and adults? expectation; however, after tasting the product, the flavor was determinant for the product choice. The low choice of products with higher levels of sugar reduction suggests the combination of gradual sugar reduction and the nutritional warnings as appropriate strategy, because they are complementary actions. The inclusion of directive FOP schemes would facilitate the identification of unhealthy foods by children and adults, and the gradual reduction of sugar would reduce the sugar content of products available in the market, contributing to the reduction of consumption, and modification of the population food habits in long-term.
