Resumo: Promoting interoperability among systems of organizations involved in an ecosystem enables better performance, and reduction of costs and operational errors. University Hospitals (UH) are complex nature organizations because there are several specificities related to their processes, protocols, legislation, systems, among others. These factors make fundamental the interoperability among these elements, being a distant and diligent goal. Software Ecosystem (SECO) Architecture can facilitate the management of complex environments, characterized by the high heterogeneity of systems and process.In this scenario, the objective of this work is propose a SECO Architecture, to promote future improvements in the interoperability of the developed systems, from the point of view of the various actors in the context of the UH Network of the Brazilian Hospital Services Company (EBSERH). Initially, a Systematic Mapping Study was performed, with the assumption of identifying applications of ecosystem architectures in the field of health. Then, a survey was applied to hospital managers of the EBSERH network, to characterize the current scenario of UH about the aspects of SECO.Finally, a SECO architecture and a deployment guide were developed, followed by two SECO health assessments in two hospitals of the EBSERH network. For the state of the art, it was identified that the proposed architectures have as main objective the semantic interoperability, being the message exchange the main approach.The standards or technologies are HL7, XML, and SNOMED. The results of the survey showed low interoperability between systems and organizations, because of high system fragmentation, insufficient use of standards and a high discrepancy among hospitals, especially concerning the number of employees, the installed Information Technology infrastructure, and often lack of software development. The proposed SECO Architecture has presented potential to promote SECO interoperability of EBSERH network hospitals, considering the current legislation and scenario of hospitals. However, overcoming barriers to the implementation of this architecture is essential, requiring the commitment of actors involved.

Os Hospitais Universitários (HU) são organizações de natureza complexa, devido à existência de diversas especificidades em relação aos seus processos, protocolos, legislações, sistemas, entre outros. Esses fatores tornam fundamental a interoperabilidade entre os elementos, sendo um objetivo distante e laborioso. Uma Arquitetura de Ecossistema de Software (ECOS) pode facilitar o gerenciamento de ambientes complexos, caracterizados por alta heterogeneidade de sistemas e processos. Nesse cenário, o objetivo deste trabalho é propor uma Arquitetura de ECOS, com a finalidade de fomentar futuras melhorias na interoperabilidade dos sistemas desenvolvidos, do ponto de vista dos diversos atores no contexto da Rede de HU da Empresa Brasileira de Serviços Hospitalares (EBSERH). Inicialmente, foi realizado um Mapeamento Sistemático da Literatura, com a premissa de identificar as aplicações de arquiteturas de ecossistemas na área da saúde. Em seguida, um survey foi aplicado aos gestores dos hospitais da rede EBSERH, para caracterização do cenário atual dos HU em relação aos aspectos de ECOS. Por fim, uma arquitetura de ECOS e um guia de implantação foram elaborados, sendo seguidos de duas avaliações da saúde do ECOS em dois hospitais da rede. Para o estado da arte, identificou-se que as arquiteturas propostas têm como principal objetivo a interoperabilidade semântica, sendo a troca de mensagens a principal abordagem. Entre os principais padrões ou tecnologias estão: HL7, XML e SNOMED. Quanto ao survey, os resultados apontaram baixa interoperabilidade entre sistemas e organizações, devido a fatores como alta fragmentação dos sistemas, uso insuficiente de padrões e alta discrepância entre os hospitais, principalmente em relação ao número de funcionários, a infraestrutura de Tecnologia da Informação instalada e, muitas vezes, ausência de desenvolvimento de software. A Arquitetura de ECOS proposta, apresentou potencial para fomentar a interoperabilidade no ECOS dos hospitais da rede EBSERH, considerando a legislação e cenário atual dos hospitais.
