Resumo: The study aims to discuss the participation process in the Brazilian Public Health System. The research fieldwork has been based on attending the Health Councils’ meetings in the primary health care level and in the Health Conferences in the Municipality and State levels in the State of Sergipe. The methodological approach was inspired by an ethnographic process of research. The research examines two kinds of democratic processes, the first being the policies and rules established that set up procedures and processes for participation and the second, the everyday processes of participation through people’s participation in their local communities. We have collected interviews, photos and videos. Participation in these institutions was a locus of interests of power and political control. On the other hand, in everyday practices, it seems that people are reinventing ways to articulate forms of participation to increase the value of health care service in their community. The study has shown that women are more participative, especially in the primary health care levels, which deals with their local needs. The research also has shown that the institutional places for participations is where it less happens effectively. Participation process happens much more in everyday lives then in those places organized by the government managements._________________________________________________________________________________________

Nous avons suivi cette participation aux conseils de santé locaux d’Aracaju et aux conférences municipales et d’État. Le travail de terrain a été mené de manière plus intensive au cours de l'année 2015, considérée comme «l'année des conférences». La participation est directement liée au contexte historique démocratique. Selon plusieurs auteurs cités dans cette étude, le système de santé unifié reste public grâce au processus de participation dans ce domaine. Dans cette recherche, nous travaillons dans la perspective de deux types de participation: une participation institutionnalisée et réglementée à travers les Conseils et Conférences. Et un autre type de participation quotidienne, ayant pour objet les mêmes intérêts et besoins qui amènent les gens à rechercher des moyens de participer et de changer les formes politiques existantes institutionnalisées. Cette deuxième forme de participation est surtout possible au niveau des soins primaires, où le système de santé est censé être lié à la communauté locale et à son environnement. C’est aussi dans cette forme de participation que nous retrouvons la majorité des femmes, intimement liées à des pratiques de soins de santé qui surpassent leur environnement domestique. D'une certaine manière, les deux formes de participation dépendent l'une de l'autre. Nous avons constaté que c’est dans les espaces destinés à la participation que celle-ci se produit le moins. Cela se produit avant et de manière plus articulée dans la vie quotidienne des utilisateurs des services de santé que lorsqu'il est administré par l’État.Este trabalho procura estudar o processo de participação na saúde pública brasileira, através de uma perspectiva etnográfica. Acompanhamos os processos de participação nos Conselhos de Saúde locais de Aracaju e nas Conferências Municipal e Estadual. A pesquisa de campo foi realizada mais intensamente durante o ano de 2015, considerado este, “o ano das Conferências”. A participação está diretamente relacionada ao contexto histórico da democracia. De acordo com diversos autores citados neste trabalho, o processo de participação contribui fortemente para que Sistema Único de Saúde continue sendo um serviço público e gratuito. Nesta pesquisa, trabalhamos sob a ótica de dois tipos de participação: uma, que é a participação institucionalizada e regulamentada através dos Conselhos e Conferências; e outra, que é caracterizada como um tipo de participação germinada cotidianamente, tendo como engrenagem, os mesmos interesses e necessidades que envolvem as pessoas a buscarem maneiras de participar e driblar às formas institucionalizadas já existentes. Esta segunda forma de participação é mais possível principalmente no nível de atendimento primário, no qual o sistema de saúde supostamente deve estar relacionado à comunidade local e seu meio ambiente. É nesta forma de participação que encontramos as mulheres, em sua maioria, interligadas às práticas de cuidado à saúde que ultrapassam seu ambiente doméstico. De certa maneira, ambas as formas de participação dependem uma da outra. Vemos que os espaços destinados para a participação são os espaços onde menos acontece.
