Resumo: The present research had as general objective to understand construction of motherhood was given to women who were in their first gestation with diagnosis of high risk pregnancy. Issues related to the constitution of motherhood in women who were in their first gestation were discussed, as well as the building of the mother-baby bonding in pregnancies which were considered high risk.It was carried out a qualitativelongitudinalresearch, delineated by the study of multiple cases. Four women who were in their first pregnancy, hospitalized in a Public Hospital in the interior of Rio Grande do Sul, due to the diagnosis of high risk participated in the study. The participants were interviewed at three times, two of the interviews occurred in the last trimester of the gestation, during the hospitalization, and the other one month after the birth of the baby. The qualitative analysis of the datawas through psychoanalytic clinical report. The results indicated that the first gestation was experienced with mixed feelings, such as joy, sadness, desire, fear, anguish and insecurity.The high-risk diagnosis potentiated the feelings of the pregnant women, mainly because of the fear of preterm birth and the possibility of malformation, but the diagnosis did not change the way the participants imagined their babies.The infant aspects of the participants' life histories, as well as the primary object identifications, appeared in this study demonstrating how intimately they are connected with the construction of motherhood and the affective space for both gestation and the baby.The care the participants received from their mothers was related to the care they could provide to their babies. In a second moment, it could be verified that the relationship with the baby was being constructed satisfactorily for all mothers, who realized that the relationship needed to be constructed daily, already demonstrating the challenges that motherhood establishes. The results of this research point to the need for increased attention to the mothers by the team in order to accompany them in this singular moment.

Foram abordadas questões referentes à constituição da maternidade em mulheres que estavam na sua primeira gestação, bem como a construção da vinculação mãe e bebê em gravidezes consideradas de alto risco. Foi realizado um estudo qualitativo, longitudinal, cujo delineamento foi estudo casos múltiplos. Participaram quatro mulheres que estavam na sua primeira gestação, internadas em um Hospital Público no interior do Rio Grande do Sul, devido ao diagnóstico de alto risco. As participantes foram entrevistadas em três momentos, duas entrevistas no último trimestre da gestação durante a internação e uma um mês após o nascimento bebê. A análise qualitativa dos dados foi por meio do relato clínico psicanalítico. Os resultados apontaram que a primeira gestação foi vivenciada com misto de sentimentos, como alegria, tristeza, desejo, medo, angústia e insegurança. O diagnóstico de alto risco potencializou os sentimentos das gestantes, principalmente pelo medo do parto prematuro e pela possibilidade de má formação no bebê, mas o diagnóstico não modificou a maneira como as participantes imaginavam os seus bebês. Os aspectos infantis das histórias de vida das participantes, assim como as identificações objetais primárias apareceram nesse estudo demonstrando o quanto estão intimamente ligadas com a construção da maternidade, e do espaço afetivo tanto para a gestação quanto para o bebê. Os cuidados que as participantes receberam de suas mães estavam relacionados com os cuidados que puderam disponibilizar aos seus bebês. No segundo momento, pode-se constatar que a relação com o bebê estava sendo construída de forma satisfatória para todas as mães, as quais perceberam que ela necessitava de construção diária, já demonstrando os desafios que a maternidade estabelece.
