Sintomatologia depressiva em idosos residentes na comunidade: prevalência e associação com atividade física e o comportamento sedentário

Resumo: Com o aumento do número de idosos no Brasil e as consequências da sintomatologia depressiva nesse segmento populacional, o conhecimento da prevalência e o efeito dos mediadores na sua relação com os fatores comportamentais podem contribuir para o entendimento dos mecanismos explicativos da doença. Assim, foram realizadas duas pesquisas distintas: 1) uma revisão sistemática com metanálise para estimar a prevalência de sintomatologia depressiva em idosos brasileiros residentes na comunidade; 2) um estudo transversal de base populacional para: a) identificar a prevalência e os fatores associados à sintomatologia depressiva em idosos; b) avaliar as associações diretas e indiretas da atividade física e do comportamento sedentário com a sintomatologia depressiva em idosos; c) examinar os efeitos hipotéticos da substituição do tempo de sono, comportamento sedentário e atividades físicas moderadas a vigorosas (AFMV) na sintomatologia depressiva em idosos. Para a revisão sistemática, foram pesquisadas as bases de dados eletrônicas Medline (via PubMed), SciELO, Web of Science, Scopus e CINAHL e selecionados estudos com idosos brasileiros que identificaram a prevalência de sintomatologia depressiva. A metanálise foi realizada para estimar a prevalência de sintomatologia depressiva usando um modelo de efeito aleatório. A heterogeneidade dos resultados foi avaliada pelo teste do qui-quadrado (significância em p <0,10), enquanto a magnitude dos efeitos foi quantificada por I2. O segundo estudo foi realizado com 473 idosos cadastrados na Estratégia Saúde da Família e residentes na zona urbana do município de Alcobaça, BA. Os idosos que preencheram os critérios do estudo e deram o seu consentimento para participar, responderam a um questionário estruturado aplicado em forma de entrevista, contendo questões sobre fatores sociodemográficos, de saúde e comportamentais. A sintomatologia depressiva foi avaliada com a versão curta da Escala de Depressão Geriátrica. Para as análises dos dados, foram aplicadas as técnicas Análise de Correspondência Múltipla, Análise de Trajetórias e abordagem de Substituição Isotemporal em modelos de Regressão de Poisson. De acordo com a metanálise, a prevalência estimada de sintomatologia depressiva em idosos brasileiros foi de 21,0% (IC95%: 18,0-25,0; I2=98,3%). Por outro lado, o estudo transversal indicou uma prevalência de 11,8% (IC95%: 9,2-15,1) para sintomatologia depressiva em idosos de Alcobaça, BA. O perfil dos idosos comsintomatologia depressiva foi caracterizado principalmente por aspectos de saúde e comportamentais. A atividade física teve associação indireta com a sintomatologia depressiva, sendo mediada pela autoestima e capacidade funcional. A associação do comportamento sedentário com a sintomatologia depressiva foi mediada pela capacidade funcional e qualidade do sono. Nas análises dos modelos isotemporais, verificou-se que a substituição do tempo despendido sentado pelo mesmo tempo no sono ou AFMV pode trazer benefícios para a sintomatologia depressiva. Quanto maior o tempo substituído, maior o efeito protetor encontrado. A substituição do tempo sentado pela AFMV resultou em maiores reduções da sintomatologia depressiva. Os resultados do presente estudo indicam a necessidade de estratégias de intervenção para reduzir a prevalência de sintomatologia depressiva. Ao considerar intervenções relacionadas à atividade física e à redução do comportamento sedentário em relação aos sintomas depressivos, a capacidade funcional, a autoestima e os fatores do sono devem ser levados em conta como mediadores.

Thus, we developed the current project in two parts: 1) a systematic review with meta-analysis to estimate the prevalence of depressive symptoms for Brazilian community-dwelling older adults; and 2) a population-based cross-sectional study to: a) identify the prevalence and factors associated with depressive symptoms in the elderly; b) to evaluate the direct and indirect associations of physical activity and sedentary behavior with depressive symptoms in the elderly; c) to examine the hypothetical effects of substitution of sleep time, sedentary behavior and moderate to vigorous physical activities (MVPA) on depressive symptoms in the elderly. For the systematic review, we searched the electronic databases Medline (via PubMed), SciELO, Web of Science, Scopus and CINAHL, and also selected studies with Brazilian elderly people that identified the prevalence of depressive symptoms. The meta-analysis was performed to estimate the prevalence of depressive symptoms using a random effect model. The heterogeneity of results was assessed by the chi-square test (significance at p<0.10) while magnitude of effects was quantified by I2. The second study was conducted with 473 elderly enrolled in the Family Health Strategy and living in the urban area of the city of Alcobaça, State of Bahia. Older adults who met the study criteria and gave their consent to participate answered an interview-based structured questionnaire, containing questions on sociodemographic, health and behavioral issues. Depressive symptoms were assessed with the short version of the Geriatric Depression Scale. Multiple Correspondence Analysis, Path Analysis and Isotemporal Replacement Approach were applied in Poisson Regression models for analyzing the data. According to the meta-analysis, the estimated prevalence of depressive symptoms for Brazilian community-dwelling older adults was 21.0% (95%CI: 18.0-25.0, I2 = 98.3%). Conversely, our cross-sectional study indicated a prevalence of 11.8% (95%CI: 9.2- 15.1) for depressive symptom in older adults from Alcobaça, BA. The profile of the elderly with depressive symptoms was characterized mainly by health and behavioral aspects. Physical activity had an indirect association with depressive symptoms, beingmediated by self-esteem and functional capacity. The association of sedentary behavior with depressive symptoms was mediated by functional capacity and sleep quality. In the analyzes of the isotemporal models, it was verified that replacing the time spent sitting for the same amount of time in sleep or moderate and vigorous activities can lead to benefits for depressive symptoms. The longer the time replaced, the greater the protective effect found. The replacement of sitting time by MVPA resulted in greater reductions of depressive symptoms. The results of the present study indicate the need for intervention strategies to reduce the prevalence of depressive symptoms. description Com o aumento do número de idosos no Brasil e as consequências da sintomatologia depressiva nesse segmento populacional, o conhecimento da prevalência e o efeito dos mediadores na sua relação com os fatores comportamentais podem contribuir para o entendimento dos mecanismos explicativos da doença. Assim, foram realizadas duas pesquisas distintas: 1) uma revisão sistemática com metanálise para estimar a prevalência de sintomatologia depressiva em idosos brasileiros residentes na comunidade; 2) um estudo transversal de base populacional para: a) identificar a prevalência e os fatores associados à sintomatologia depressiva em idosos; b) avaliar as associações diretas e indiretas da atividade física e do comportamento sedentário com a sintomatologia depressiva em idosos; c) examinar os efeitos hipotéticos da substituição do tempo de sono, comportamento sedentário e atividades físicas moderadas a vigorosas (AFMV) na sintomatologia depressiva em idosos. Para a revisão sistemática, foram pesquisadas as bases de dados eletrônicas Medline (via PubMed), SciELO, Web of Science, Scopus e CINAHL e selecionados estudos com idosos brasileiros que identificaram a prevalência de sintomatologia depressiva. A metanálise foi realizada para estimar a prevalência de sintomatologia depressiva usando um modelo de efeito aleatório. A heterogeneidade dos resultados foi avaliada pelo teste do qui-quadrado (significância em p <0,10), enquanto a magnitude dos efeitos foi quantificada por I2. O segundo estudo foi realizado com 473 idosos cadastrados na Estratégia Saúde da Família e residentes na zona urbana do município de Alcobaça, BA. Os idosos que preencheram os critérios do estudo e deram o seu consentimento para participar, responderam a um questionário estruturado aplicado em forma de entrevista, contendo questões sobre fatores sociodemográficos, de saúde e comportamentais. A sintomatologia depressiva foi avaliada com a versão curta da Escala de Depressão Geriátrica. Para as análises dos dados, foram aplicadas as técnicas Análise de Correspondência Múltipla, Análise de Trajetórias e abordagem de Substituição Isotemporal em modelos de Regressão de Poisson. De acordo com a metanálise, a prevalência estimada de sintomatologia depressiva em idosos brasileiros foi de 21,0% (IC95%: 18,0-25,0; I2=98,3%). Por outro lado, o estudo transversal indicou uma prevalência de 11,8% (IC95%: 9,2-15,1) para sintomatologia depressiva em idosos de Alcobaça, BA. O perfil dos idosos comsintomatologia depressiva foi caracterizado principalmente por aspectos de saúde e comportamentais. A atividade física teve associação indireta com a sintomatologia depressiva, sendo mediada pela autoestima e capacidade funcional. A associação do comportamento sedentário com a sintomatologia depressiva foi mediada pela capacidade funcional e qualidade do sono. Nas análises dos modelos isotemporais, verificou-se que a substituição do tempo despendido sentado pelo mesmo tempo no sono ou AFMV pode trazer benefícios para a sintomatologia depressiva. Quanto maior o tempo substituído, maior o efeito protetor encontrado. A substituição do tempo sentado pela AFMV resultou em maiores reduções da sintomatologia depressiva. Os resultados do presente estudo indicam a necessidade de estratégias de intervenção para reduzir a prevalência de sintomatologia depressiva. Ao considerar intervenções relacionadas à atividade física e à redução do comportamento sedentário em relação aos sintomas depressivos, a capacidade funcional, a autoestima e os fatores do sono devem ser levados em conta como mediadores.
