The well-known difficulties that the Unified Health System (SUS) has to provide comprehensive care to all its potential users cause inconvenience in daily life of families who need it. Thus, this study aimed to analyze the decision making process of the families who had some of its members in a situation of waiting for surgery through SUS, considering the possibility to look for alternatives outside the public health system. Specifically, we sought to examine the perceptions of users and managers of SUS on the functioning of the system, analyzing the social networks activated by patients waiting for surgery and identify the steps and roles of the family through the decision process. We performed a qualitative study that interviewed 45 patients who were on the waiting list, as well as six institutional leaders linked to the health of the municipality of Viçosa, MG. Patients and leadership demonstrated dissatisfaction with the functioning of the SUS, to the extent that it failed to meet its fundamental principles of comprehensiveness and universality, and access is not equal, mainly because of insufficient resources invested in health by the federal and state. The profile of users queued showed that mostly consisted of women, low literacy and limited income, indicating a state of poverty. Social networks activated by these patients were focused on close relatives, where they came from the main aids, which were expressive content, governed by rules based on the obligations of a family. The family decision-making process was more influenced by relatives who have had most of the roles, predominantly, however, the opinions and judgments of patients themselves. It was concluded that the SUS, when it can not provide the service expected by users, because changes in everyday personal and family, creating a feeling of powerlessness in those who depend on the public health system.
