Busca analisar os dilemas humanos sob a ótica das teorias éticas, entre as quais a principialista é a mais divulgada na área da saúde. É considerada uma ética aplicada, normativa, prática e produtiva, que auxilia na análise e na reflexão sobre problemas na prática biomédica; entretanto sua inserção na práxis da fisioterapia é recente. O objetivo geral deste estudo foi analisar a inserção da ética e da bioética na práxis dos fisioterapeutas, buscando compreender os dilemas éticos existentes sob a ótica dos quatro princípios – autonomia, beneficência, não-maleficência e justiça. Utilizaram-se a pesquisa bibliográfica e a investigação de campo, por meio de abordagens quantitativa e qualitativa. A pesquisa bibliográfica consistiu em um estudo exploratório, através da revisão sistemática dos textos publicados a partir de 2000, em bases de dados eletrônicos e em sites de revistas de fisioterapia. Isso permitiu verificar como os temas sobre ética e bioética foram incorporados à prática e à pesquisa em fisioterapia Constatou-se que a evolução da pesquisa bioética na área é crescente no cenário internacional, mas há carência desses estudos no âmbito nacional. A pesquisa do tipo quantitativa, com estudo transversal e descritivo, foi realizada com 167 fisioterapeutas que atuavam em Santa Maria (RS), no final de 2006. Os dados foram coletados por meio de instrumento semi-estruturado, codificados e analisados utilizando-se o banco de dados Microsoft Excel 2002 e processados pelo pacote estatístico SAS (Statistical Analysis System). Esses profissionais eram, em sua maioria, adultos entre 22 e 35 anos (60%), mulheres (83%) e atuavam predominantemente como autônomos. A pesquisa qualitativa foi realizada por meio de entrevistas em duplas com 26 fisioterapeutas, selecionados intencionalmente de acordo com a área, o local e os níveis de atuação. Utilizou-se a técnica de análise de conteúdo temática para a interpretação e discussão dos dados. Na prática profissional, os fisioterapeutas demonstraram respeito pelos aspectos éticos normativos da profissão, usados em benefício do paciente. A tendência a ações paternalistas ainda prevalece, embora as tentativas de superação desse modelo sejam crescentes, o que é evidenciado pelo respeito aos direitos e às decisões autônomas dos pacientes. Quanto ao pleno exercício da autonomia, observou-se que a estrutura do sistema de saúde, as relações de trabalho e a formação acadêmica impõem a esses profissionais diversas limitações caracterizadas por dilemas nas relações intra e interprofissionais e terapeuta–paciente. Isso demonstra a necessidade premente de inserir as questões éticas e bioéticas tanto na formação acadêmica quanto na prática e capacitação dos fisioterapeutas, como forma de contribuir para as tomadas de decisões em situações conflituosas. _________________________________________________________________________________________ ABSTRACTBioethics emerged as an alternative to promote reflections on the moral conflicts related to man, to their well-being, and to their inter-relation with the environment. It tries to analyze the human dilemmas according to ethical theories, being the principlism theory the most known in the health field. It is considered applied, normative, practical, and productive ethics, helping the analysis and the reflections on problems in the biomedical practice; however, it has just been included in physical therapy praxis. The general objective of this study was to analyze the inclusion of ethics and bioethics in the physical therapists‟ practice, aiming at understanding the existent ethical dilemmas according to the four principles – autonomy, beneficence, non-maleficence and justice. Bibliography and field research through quantitative and qualitative approaches were used. The bibliography research consisted of an exploratory study through a systematic review of texts published on electronic databases and on websites of physical therapy magazines from the year 2000 on. This made it possible to verify how the issues about ethics and bioethics were incorporated into the practice and the research in physical therapy. It was observed that there is an ever-increasing evolution of bioethics research in the international scenario, but there is a lack of such studies in the national scenario. The quantitative research with a transversal and descriptive study was carried out with 167 physical therapists who worked in Santa Maria (RS) late in 2006. Data were collected through a semi-structured instrument, coded and analyzed using Microsoft Excel 2002 database and were processed by the statistics package SAS (Statistical Analysis System). Most of these professionals were adults between 22 and 35 years old (60%), women (83%) and predominantly self-employed. The qualitative research was carried out through interviews in pairs with 26 physical therapists, selected deliberately, according to the clinical area, the place and levels of performance. The technique thematic content analysis was used to interpret and discuss the results. In the professional practice, physical therapists showed respect for the normative ethical aspects of the profession, used to benefit the patient. There is still a tendency to paternalist actions, although there are increasing attempts to overcome this model, which is shown by the respect for the patients‟ rights and autonomous decisions. Regarding the full exercise of autonomy, it was observed that the structure of the health system, the work relationships and the academic education impose a lot of limitation on these professionals, characterized by dilemmas in the intra and interprofessional relationships as well as in the therapist-patient ones.
