Resumo: The cardiovascular diseases are those which promote the highest number of deaths in the world and Brazil follows this trajectory. The state of Pernambuco as well as the capital and Caruaru have in the last decade a growing and converging trend in the number of deaths. This signals to the inefficiency of health policy in the face of increasing and obvious needs of the population. On the other hand, studies that support the policy maker in policy are virtually nonexistent in the State hinterland. In this regard, from a critical perspective on the role of institutions in the current health system, this dissertation contributes from the methodology of arrangements and local production and innovation systems with a study on public policies in cardiovascular care in Central Caruaru Agreste region of Pernambuco, according to state health subsystem in dimension of target audience watched and infrastructure offered. In conclusion, attention is drawn to: a) there is in ASPIL a large number of local health-related institutions that require a dynamic directed to the extension of the benefits of a better functioning of ASPIL, based on innovation, cooperation and knowledge that translates into benefits for the target population policy, b) there is a commitment by the institutions of the perception of the potential for cooperation in ASPIL that inhibits policies to stimulate knowledge transfer of new initiatives, and through the activities already carried out between actors of ASPIL. Based on these observations , it is present the following suggestions for policy action: i) promote policies that encourage coordination between actors coordinating ASPIL with medical providers, ie, hospitals and public and private clinics; ii) improve the training of professionals both technical and higher level with the expansion of courses in health techniques in local schools and universities, focused on cardiovascular diseases; iii) increase and strengthen the infrastructure of higher education for specific training of medical professionals with specializations in the ASPIL preventive treatments, as well as medium and high complexity; iv) intensifying relations of buying and selling within the ASPIL between the government and local suppliers with a view to provide the supply of inputs and equipment, including specialized stimulate local production; v) include in the local analysis that precedes the development of the notion of political territory for the utilization of tacit knowledge and its transmission channels that feed back into cooperation and dissemination of innovation, as important for cost reduction actions and a closer relationship with the recipients of policy, population served, and, vi) financially stimulate research on ASPIL in universities , in order to have greater clarity of policy goals and ways to achieve them more efficiently.

O estado de Pernambuco bem como a capital e o município de Caruaru apresentaram na última década uma tendência crescente e convergente do número de óbitos, isto sinaliza para a ineficiência política da saúde diante das crescentes e evidentes necessidades da população. Por outro lado, pesquisas que subsidiem os policy makers na configuração das políticas são praticamente inexistentes no interior do Estado. Neste sentido, a partir de uma perspectiva crítica da atuação das instituições do atual sistema de saúde, a presente dissertação contribui a partir da metodologia de arranjos, sistemas produtivos e inovativos de locais com um estudo sobre as políticas públicas de atenção cardiovascular. Em Caruaru no Agreste Central de Pernambuco, segundo o subsistema de saúde do Estado, em dimensão do público alvo assistido e da infraestrutura ofertada em conclusão, destacam-se: a) Há no ASPIL um grande número de instituições local, relacionadas à saúde que requerem uma dinâmica direcionada à ampliação dos benefícios advindos de um melhor funcionamento do ASPIL, baseado na inovação, cooperação e conhecimento que se traduza em benefício da população alvo da política; b) Há um comprometimento da percepção e do potencial, pelas instituições de cooperação no ASPIL que inibe as políticas de estímulo à transmissão do conhecimento de novas iniciativas, através das atividades e experiências bem sucedidas já desenvolvidas entre atores do ASPIL. Com base nestas observações, são apresentadas as seguintes sugestões de ações e de políticas: i) promover políticas que estimulem a articulação entre os atores de coordenação do ASPIL com os prestadores de serviços médicos, ou seja, hospitais e clínicas públicos e privadas; ii) melhorar a formação de profissionais, tanto de nível técnico, quanto de nível superior com a ampliação dos cursos na área de saúde, nas escolas técnicas e universidades locais, voltados para as doenças cardiovasculares; iii) aumentar e reforçar a infraestrutura de ensino superior específica para formação de profissionais da área médica do ASPIL com especializações em tratamentos preventivos, bem como, os de média e alta complexidade; iv) intensificar as relações de compra e venda dentro do ASPIL entre o governo e os fornecedores locais, com vistas a prover o abastecimento de insumos e equipamentos, inclusive estimular a produção especializada local; v) incluir na análise do local que precede a elaboração da política a noção de território com vistas ao aproveitamento do conhecimento tácito e de seus canais de transmissão que realimentam a cooperação e a disseminação da inovação, que são importantes para a redução de custos das ações que permite a maior aproximação com os receptores da política, e da população beneficiada; vi) estimular financeiramente as pesquisas sobre o ASPIL nas universidades, para que se tenha maior clareza dos caminhos que possibilitem alcançar de forma mais eficiente os objetivos da política.Submitted by (edna.saturno@ufrpe.br) on 2016-06-09T13:16:48Z No. of bitstreams: 1 Rafaela Rodrigues Lins.pdf: 1434297 bytes, checksum: 3b05adafb35ac415439358528291c926 (MD5)Made available in DSpace on 2016-06-09T13:16:48Z (GMT).
