Impacto da dieta dissociada no índice internacional de qualidade da dieta e nos biomarcadores de estresse oxidativo

Resumo: Obesity is a chronic disease that contributes to the development of oxidative stress, increased risk of developing cardiovascular disease and metabolic syndrome. Alternatives for the treatment of obesity various types of low-calorie diets have been used, among these, the dissociated diet. This diet recommends that carbohydrate and protein should not be consumed in a meal together in order to promote greater efficiency and accelerate the metabolic process of weight reduction. The aim of this study was to investigate the effect of dietary carbohydrate and protein in dissociated diets on biomarkers of oxidative stress represented by circulating levels of oxidized low density lipoprotein (oxLDL) and plasma total antioxidant capacity (TAC) and their associations with anthropometric, biochemical, and dietary oxidative in men in conditions of overweight and obesity. The study included 31 healthy men with BMI between 26.5 and 35.2 kg/m2, who followed the diet for 8 weeks with caloric restriction the 250 kcal. The subjects were randomly divided into three groups according to the type of diet. For the control group (GCT), was prescribed a diet balanced in nutrients, the group of day-carbohydrate (GCD) was prescribed a high carbohydrate / low-protein lunch and dinner, while in the night- carbohydrate group (GCN) the distribution of macronutrients in the meals was the reverse of the GCD. At the beginning and end of this study were analyzed anthropometric measurements, body composition using the deuterium and the serum glucose, insulin, cholesterol and fractions, triglycerides, uric acid, ceruloplasmin, TAC and oxLDL. Diet composition and the overall quality of the usual diet and during the intervention were analyzed using food diaries and Dietary Quality Index- International (IDQI). At the end of eight weeks, the volunteers had an average weight loss of 4.26 ± 1.47% in GCT, 3.29 ± 2.46 in the GCD, and 4.85 ± 3.01% at GCN and there was a reduction in BMI, WC and HC in the significant when compared to the initial period. There were no differences between groups in the observed changes in anthropometric measurements. The average caloric restriction was 26.01 ± 22.03% in GCT, 31.70 ± 30.28% in the GCN and 16.87 ± 26.67% in the GCD and there was no difference between groups. The quality of the overall diet and nutritional composition showed no significant variations between groups, with the exception of sodium and selenium, which showed a greater reduction in BCM when compared to the other groups (p <0.05). According IDQI the diet of the volunteers was characterized as low quality both in the evaluation of habitual intake and during the intervention. The oxidative and biochemical variables analyzed have not changed significantly between the groups during the study with the exception of serum glucose rose in the GCD and HDL-C which is elevated and insulin decreased in the GCT group (p <0.05), but remained within the normal range. Despite dietary and endogenous variables that could influence serum concentrations of biomarkers of oxidative stress have not changed substantially, TAC rose at the GCT and GCD (p <0.05). While the serum concentration of oxLDL was higher in GCT compared to other groups (p <0.05) and its concentration decreased not significantly only in GCD (p> 0.05). There were no associations between variations in TAC and oxLDL and quantity supplied of carbohydrate or protein meals in there that the dissociation of the diet (p> 0.05). The same analysis was performed for IDQI and their partial scores with respect to TAC and oxLDL and no correlation was observed between these variables, even after adjustment (p> 0.05). We conclude that although the dissociated diet have promoted weight loss and reduction of other anthropometric variables, no relationship was observed between TAC and oxLDL with biochemical variables, and dietary oxidative studied.

Na busca de alternativas para o tratamento da obesidade vários tipos de dietas hipocalóricas tem sido utilizada, dentre estas, encontra-se a dieta dissociada. Esta dieta preconiza que carboidratos e proteínas não devem ser consumidos juntos em uma refeição, a fim de promover uma maior eficiência metabólica e acelerar o processo de redução ponderal. Dessa forma, o objetivo deste estudo foi investigar o efeito da dieta dissociada em carboidrato e proteína sobre os biomarcadores do estresse oxidativo representados pelos níveis circulantes de lipoproteína de baixa densidade oxidada (LDLox) e a capacidade antioxidante total do plasma (CAT) e suas associações com variáveis antropométricas, bioquímicas, oxidativa e dietéticas, em homens em condição de sobrepeso e obesidade. Participaram deste estudo 31 homens saudáveis com IMC entre 26,5 e 35,2 kg/m2, que seguiram a dieta por 8 semanas com uma restrição calórica de 250 kcal. Os indivíduos foram distribuídos aleatoriamente em três grupos de acordo com o tipo de dieta. Para o grupo controle (GCT), foi prescrita uma dieta equilibrada em macronutrientes; para o grupo de carboidrato diurno (GCD), foi prescrito almoço hiperglicídico/hipoprotéico e jantar hipoglicídico/hiperprotéico, enquanto que no grupo do carboidrato noturno (GCN), a distribuição dos carboidratos e proteínas nas grandes refeições foi o inverso do GCD. No início e no final deste estudo foram aferidas as medidas antropométricas, de composição corporal utilizando-se o deutério e a concentração sérica de glicose, insulina, colesterol e suas frações, triglicerídeos, acido úrico, ceruloplasmina, CAT e LDLox. A composição da dieta e a qualidade global da dieta habitual e durante a intervenção, foram analisadas por meio de diários alimentares e do Índice Internacional de Qualidade da Dieta (IQDI). Ao final das 8 semanas, verificou-se que os voluntários deste estudo tiveram perda de peso média de 4,26 ± 1,47 % no GCT, 3,29 ± 2,46 no GCD e 4,85 ± 3,01 % no GCN e também houve redução no IMC, CC e CQ significativa quando comparado ao período inicial. Não houve diferenças entre os grupos nas mudanças observadas nas medidas antropométricas. A média de restrição calórica foi de 26,01±22,03% no GCT, 31,70±30,28% no GCN e 16,87±26,67% no GCD e não houve diferença entre os grupos. A qualidade da dieta global da dieta e sua composição nutricional não apresentaram variações significativas entre os grupos, com exceção do sódio e selênio, que apresentaram redução maior no GCN quando comparados aos outros grupos (p<0,05). De acordo com IQDI, a dieta dos voluntários foi caracterizada como de baixa qualidade, tanto na avaliação da ingestão habitual quanto durante a intervenção. As variáveis bioquímicas e oxidativas não sofreram modificações significativas entre os grupos durante o estudo, com exceção da glicose sérica que aumentou no GCD. No GCT, as concentrações de HDL-c subiram enquanto as de insulina foram reduzidas (p<0,05), mas se mantiveram no dentro da faixa de normalidade. Apesar das variáveis endógenas e dietéticas que poderiam influenciar as concentrações séricas dos biomarcadores do estresse oxidativo não terem sofrido variações significativas, a CAT se elevou no GCT e GCD (p<0,05). A concentração sérica de LDLox, no entanto,foi mais elevada no GCT, quando comparada aos demais grupos (p<0,05) e a sua concentração diminuiu de forma não significativa exclusivamente no GCD (p>0,05). Não foram observadas associações entre as variações da CAT e da LDLox e a quantidade ofertada de carboidrato ou proteína nas refeições em que houve a dissociação da dieta (p>0,05). As mesmas análises foram realizadas para o IQDI e seus escores parciais com relação à CAT e LDLox e não constatou-se correlação entre essas variáveis, mesmo após ajustes (p>0,05).
