Resumo: Self-inflicted violence among children and adolescents is an important health issue. The objective of this study was to characterize and to describe patients with self-inflicted violence from Psychosocial Care Centers for children and adolescents (CAPSi) in São José do Rio Preto, SP, Brazil. Method: This is a descriptive, retrospective, documentary study with a quantitative approach. Data were obtained through the analysis of patients records between January 2019 and December 2021, specifically the cases identified by the CAPSi(s) teams as patients with self-inflicted violence. Data collection was carried out using an instrument built by the researcher and the SPSS Statistics software version 29 was used for the exploratory data analysis. Results: records of 225 patients were identified; most were female (n = 182), enrolled on fundamental school (6th to 9th years) (n =116). Patients’ referrals to CAPSi came mainly from Psychiatric Emergency (n = 69), due to spontaneous demand (n = 48), and from Health Care Unities (n = 38). Previously suffered violence was reported in 117 cases: sexual (n = 71), physical (n = 40), psychological (n = 47) and neglect/abandonment (n = 13). The use of legal and illegal drugs was observed in 47, the most frequent of which were alcohol (n = 32), marijuana (n = 21) and multiple drugs (n = 5). The diagnoses most frequently attributed to patients were mood disorders (n = 178), personality disorders (n = 66) and anxiety disorders (n = 57). Until the end of data collection, 110 were still receiving care through CAPSi; 115 had ended their care due to discharge from the team (n = 34), abandonment or requested discharge (n = 55), transfer to another service (n = 10), change of city (n = 10). The average treatment time at the CAPSi was 17.4 months (±9.7 months). There was no correlation between alcohol xix and drug use and self-inflicted violence, pointing to other predisposing factors. Conclusion: There was a predominance of single-parent families, with a family history of mental disorders and adolescent patients who were victims of previous violence, with self-mutilation (cuts) in the upper and lower limbs. The absence of mandatory notification for self-inflicted violence was identified by some services.

Este estudo teve como objetivo caracterizar e descrever pacientes de CAPS Infantis de São José do Rio Preto com violência autoprovocada. Método: estudo descritivo documental, retrospectivo, com abordagem quantitativa. Os dados foram obtidos por meio da análise dos prontuários de pacientes atendidos entre janeiro de 2019 a dezembro de 2021, identificados pelas equipes dos CAPSi(s) como pacientes com violência autoprovocada. A coleta foi realizada com instrumento construído pela pesquisadora e a análise exploratório dos dados foi realizada com o software IBM-SPSS Statistics versão 29. Resultados: foram identificados 225 prontuários, a maioria (n = 182) do sexo feminino e cursando ensino fundamental II - 6º ao 9º anos (n =116). Os encaminhamentos para o CAPSi foram realizados principalmente pela Emergência Psiquiátrica (n = 69), por demanda espontânea (n = 48) e pela Unidade de Saúde (n = 38). Havia antecedentes de transtornos mentais em familiares (n = 198), inclusive suicídios e tentativas, e a maioria residia com a genitora (famílias monoparentais). Automutilação foi a forma mais frequente de violência (n = 199) e ocorreu principalmente com cortes nos membros superiores (n = 184); em 148 dos casos analisados houve tentativa de suicídio, especialmente com a ingesta de medicamentos (n = 120); em 102 casos a presença de ideação suicida estava anotada no prontuário; foram notificados 146 casos, dos 225 casos identificados embora os profissionais sejam obrigados a notificar. Em 117 casos foi relatada violência sofrida anteriormente: sexual (n = 71), física (n = 40), psicológica (n = 47) e negligência/abandono (n = 13). O uso de drogas lícitas e ilícitas foi observado em 47 jovens, sendo as mais frequentes xvii álcool (n = 32), maconha (n = 21) e múltiplas drogas (n = 5). Os diagnósticos atribuídos com maior frequência aos jovens foram transtorno do humor (n = 178), transtorno da personalidade (n = 66) e transtornos da ansiedade (n = 57). Até o término da coleta de dados, 110 continuavam em atendimento pelo CAPSi; 115 haviam encerrado o atendimento por alta da equipe (n = 34), abandono ou alta solicitada (n = 55), transferência para outro serviço (n = 10), mudança de cidade (n = 10). O tempo médio de tratamento junto ao CAPSi foi 17,4 meses (±9,7 meses). Não houve correlação de uso de álcool e drogas com a violência autoprovocada, apontando para outros fatores predisponentes. Conclusão: Houve predominância de famílias monoparentais, com antecedentes familiares com transtornos mentais e pacientes adolescentes vítimas de violências anteriores, com automutilação (cortes) nos membros superiores e inferiores.
