Resumo: Among the new cytokine Family with Sequence Similarity 3 (FAM3), a subfamily of proteins similar to cytokines known as FAM3D was identified. FAM3D is predominantly expressed in the gastrointestinal tract of healthy individuals. Plasma concentrations of FAM3D range according to the nutritional status presenting a postprandial increase and a reduction in the later post-absorptive period. It might be speculated that FAM3D is an inhibitor of insulin secretion, as it has been observed a significant increase in patients with pancreatic adenocarcinoma associated with T2D. Our aim was to quantitatively determine the plasma concentration of FAM3D, in a group of patients with T2D and comparing with patients with pre-Type 2 Diabetes Mellitus (pT2D) and Control (CTRL) individuals paired according to gender, age and body mass index in order to better elucidate the action of FAM3D as well as its pathophysiological role in T2D. We selected 90 patients comprising 15 male and 14 females diagnosed with T2D; 12 male and 15 female diagnosed pT2D; and 7 male and 9 female CTRL. Seventeen patients were excluded according to the inclusion and exclusion criteria. In total, 73 patients were included. All were submitted to measurement of anthropometric and biochemical tests, including HOMA-IR calculation and the determination of plasma concentrations of FAM3D by ELISA. The Kruskal-Wallis tests were used in the comparison of study groups, and the regression coefficient  of Spearman was calculated on correlation between plasma concentrations of FAM3D and other anthropometric and biochemical variables. The level of statistical significance was set at p <0.05. Among the groups there were no significant difference in the concentration of FAM3D, and the median±Q for the CTRL group (24.92ng/mL ± 9.11); pT2D (14.65ng/mL ± 6.02) and T2D (19.87ng/mL ± 5.39). The group of DM2 patients on sulfonylurea (23.42ng/mL ± 6.1) had higher values compared to the group treated with other oral antidiabetic drugs (14.9 ng/mL ± 7.30), p <0.05. There was no correlation of FAM3D between groups. Patients with T2D on sulfonylurea presented a higher concentration of FAM3D when compared with T2D patients treated with other oral antidiabetic drugs.

As concentrações plasmáticas de FAM3D variam de acordo com o estado nutricional apresentando uma elevação pós-prandial e uma redução no período pós-absortivo tardio. Aventa-se a hipótese de que o FAM3D seria um inibidor da secreção insulínica, já que foi observada sua elevação expressiva em tumores de pacientes com adenocarcinoma de pâncreas associado ao Diabetes Mellitus (DM). Nosso objetivo foi o de determinar quantitativamente a concentração plasmática do FAM3D, pelo método de ELISA, em grupo de indivíduos portadores de DM2 e compará-las com as de indivíduos com Pré-DM2 (PDM2) e Controle (CTRL) não portadores de DM. Foram pareados de acordo com gênero, idade e índice de massa corpórea a fim de elucidar melhor a ação do FAM3D bem como sua relação fisiopatológica com o DM2. Selecionamos 90 pacientes, que, após a realização dos exames, 17 foram excluídos de acordo com os critérios de inclusão e exclusão. A distribuição entre os grupos por gênero ficou desta forma: DM2 com mais de 5 anos do diagnóstico, sem uso de Insulina e sem complicações ou outras doenças grave – 15 masculinos e 14 femininos; PDM2 – 13 masculinos e 15 femininos e grupo controle – 7 masculinos e 9 femininos, totalizando 73 pacientes. Todos foram submetidos à mensuração das medidas antropométricas e exames bioquímicos mais o cálculo do HOMA-IR e a determinação da concentração plasmática do FAM3D pelo método de ELISA. Foram utilizadas as provas de Kruskal-Wallis, na comparação dos grupos de estudo, e o coeficiente de regressão  de Spearman foi calculado nas análises de correlação entre as concentrações plasmáticas de FAM3D e outras variáveis antropométricas ou bioquímicas. Os cálculos estatísticos foram realizados pelo programa JMP (SAS Campus Drive. Building T. Cary, USA). O nível de significância estatística foi fixado em p<0,05. Entres os grupos não houve diferença significante da concentração do FAM3D, sendo a Mediana ± Q para o grupo CTRL (24,92 ng/mL ± 9,11); PDM2 (14,65 ng/mL ± 6,02) e DM2 (19,87 ng/mL ± 5,39). A concentração plasmática do FAM3D foi mais alta nos pacientes com DM2 em uso de Sulfoniluréia (p=0,0001).
