Introduction: Questionnaires are commonly used to quantify the level of physical activity in daily life. However they may underestimate it when compared to the accelerometer becoming unreliable in the accurate evaluation of these parameters in their respective individuals, mainly the asthmatic ones that has as characteristic the decrease of the physical and functional capacity. Objective: To evaluate the accuracy of the International Physical Activity Questionnaire (IPAQ-SF short version) in identifying the level of physical activity compared to the accelerometer in children and adolescents with asthma. Method: Cross-sectional study in asthmatic patients aged 6-18 years with diagnosis of asthma. Spirometry, the Modified Shuttle Walk Test (SWTI), IPAQ-SF and accelerometry were performed to evaluate the daily life activity in the last seven days (the comparisons between the activity levels in the two instruments were made by the time spent at the level: light, moderate, vigorous, sedentary). Results: A total of 43 volunteers from the asthma group (GA) with mean age of 10 (8-13) years and eutrophic (25) were male (58%). Regarding the severity of the disease, the group was classified as mild to moderate asthma Step 3 (2-4) according to GINA. Regarding functional capacity, on average the patients presented 83% ± 15% of predicted (782 ± 183m). There was a difference between the time measured at all levels of IPAQ activity compared to the accelerometer: light activity 60 min (0-120) in the IPAQ vs 1482 min (846-2032min) in the accelerometer, p = 0.001; moderate activity: 60 min (10-120) in the IPAQ vs 30 min (11-68) in the accelerometer, p = 0.034; vigorous activity: 40 min (0-135) in the IPAQ vs 0 min (0-10) in the accelerometer, p = 0.001; time in sedentarism: 3000 min (2010-4320) in the IPAQ vs 2680 min (1407-3504) in the accelerometer, p = 0.030. Additionally, there was no good reliability between questionnaires and the accelerometer to identify the intensity of physical activity. The ICC value ranged from 0.01 to 0.03 for all activity levels. Conclusions: The IPAQ did not prove to be an accurate method to identify the level of physical activity in asthmatic children and adolescents compared to the accelerometer.
