Resumo: | Myelomeningocele is a neural tube defect that occurs during embryonic development due to the incomplete closure of the spinal neural tube. It ultimately leads to an exposed neural tissue or meninges with a fluid-filled sac that protrudes at the affected vertebral level. The unprotected neural tissues suffer progressive harm due to exposure to chemical and mechanical factors of the intrauterine environment. These impairments may impact the child's functionality, placing them at risk for decreased social participation. Physiotherapy is an important feature of the rehabilitation of individuals with MMC. However, knowledge of which physiotherapeutic strategies should be used or the best frequency and intensity of the sessions are still lacking. of photobiomodulation (PBM) as a supplemental therapeutical tool has demonstrated positive effects in sensory and motor recovery patients with spinal cord injuries when combined with physiotherapy or alone in in vivo spinal cord injury models. Purpose: Evaluate the effects of PBM associated with physiotherapy on the functional performance of children with myelomeningocele. Methods: Twenty-five children were randomly allocated to either Active PBM+physiotherapy (n = 13) or PBM sham+physiotherapy (n = 12). PBM was carried out with a LED device (850 nm, 25 J, 50s per point and 200 mW) at four points over the area with absence of a spiny process. Both groups completed twelve-week supervised program with two weekly 45 – 60 min sessions. Pre-training and post-training assessments involved the Pediatric Evaluation of Disability Inventory (PEDI score). Muscle activity was assessed using portable electromyography (BTS Engineering) and the electrodes were positioned on the: lateral gastrocnemius, anterior tibialis and rectus femoris muscles. Results: After 24 sessions of the treatment protocol, improvements were found in the PEDI score in both evaluated groups. The participants presented greater independence in performing the tasks, requiring less assistance from their caregivers. More significant electrical activity was found in the three muscles evaluated between the rest period and execution of the sit-to-stand tasks, both in the more compromised or less compromised lower limbs. Conclusion: Neurofunctional physiotherapy with or without PBM improved functional mobility and electrical muscle active in children with myelomeningocele. |

Essa condição afeta drasticamente varias estruturas incluindo a medula espinhal, as raízes nervosas e as meninges são expostas durante a gravidez, acarretando em deficiências. Como consequência, essas crianças apresentam diminuição geral da mobilidade funcional e da participação social. A fisioterapia é uma parte importante da reabilitação em crianças com MMC, mas não há consenso ou ensaios disponíveis sobre a frequência, intensidade ou quais estratégias fisioterapêuticas devem ser utilizadas nas diferentes idades. A literatura atual demonstra que recursos como fotobiomodulação (FBM) podem ser uma ferramenta terapêutica auxiliar na reabilitação de condições neurológicas. Objetivo: Avaliar os efeitos da FBM associada à fisioterapia no desempenho funcional e na atividade muscular eletétrica de crianças com mielomeningocele. Metodologia: Após a aprovação do comitê de ética, quarenta e dois participantes foram avaliados para elegibilidade. Dezessete crianças foram excluídas. Vinte e cinco crianças foram randomizadas em dois grupos. O grupo A foi submetido a FBM ativa + exercícios fisioterapêuticos. O grupo B foi submetido a FBM sham + exercícios fisioterapêuticos. A irradiação foi realizada com um LED com comprimento de onda de 850 nm, energia por ponto de 25 J, 50 segundos por ponto e potência de 200 mW. O protocolo de fisioterapia foi realizado através de atividades funcionais, movimentos ativos com repetições. O protocolo de tratamento foi realizado duas vezes na semana por 24 sessões. As avaliações foram executadas antes do tratamento e após a última sessão. Para avaliar a independência funcional das crianças, foi utilizado o Inventário de Avaliação Pediátrica da Incapacidade (escala PEDI). A atividade muscular foi avaliada usando um eletromiógrafo portátil (BTS Engineering). Os eletrodos foram posicionados sobre os músculos: Gastrocnêmio lateral, tibial anterior e reto femoral. A avaliação sensorial foi realizada com o kit Semmes-Weinstein (Smiles®). Os dados foram analisados com o auxílio do GraphPad PRISM versão 8.0. Resultados: Após 24 sessões do protocolo de tratamento, foram encontradas melhoras no escore PEDI em ambos os grupos avaliados. Os participantes apresentaram maior independência na realização das tarefas, exigindo menos assistência de seus cuidadores. A melhora da atividade elétrica foi encontrada nos três músculos avaliados entre o período de repouso e a execução das tarefas de sentar e levantar, apresentada tanto na avaliação dos membros inferiores mais comprometidos quanto dos menos comprometidos.
