Percepção da equipe interprofissional sobre o uso da telemedicina em Unidades de Terapia Intensiva durante a pandemia do COVID-19

Resumo: The COVID-19 pandemic has boosted the use of telemedicine as a means of communication, dissemination of knowledge and sharing of behaviors. The effectiveness results of the use of telemedicine in intensive care in pre-pandemic periods were heterogeneous and possibly influenced by the sovereignty of the human factor over technology. Understanding real-world experiences, through the lens of daily practice patterns across the multiple facets of interprofessional interaction in intensive care, in a unique period such as the COVID-19 pandemic, can help identify local structures and processes that will facilitate effective implementation of the use of telemedicine in the ICU. The objectives of the research were to characterize attitudes and perceptions of the interprofessional team about the usefulness and effectiveness of using Telemedicine in ICUs in times of pandemic and, based on this knowledge, to outline proposals that optimize its use. The Participants: Interprofessional health team from Itapetininga who had contact with Telemedicine in the ICU during the period of the COVID-19 Pandemic. The methodology used was a cross-sectional, quali-quantitative, exploratory study carried out through online questionnaires and interviews in focus groups. Quantitative response analysis was performed using IBM SPSS Statistics 21 software. Quantitative data were described based on their main measures of position (mean, median and standard deviation) and frequency. To analyze the relationships between the categorical variables gender, public or private, age group, professional category with the observed perceptions, the Pearson test for contingency tables was performed. In cases of significant results (p-values <0.05) the analyzes were supplemented by the analysis of Chi-square residuals (χ2). The Content Analysis of the FGs was of the thematic type and construction of Bardin's non-a priori categories. After analyzing the data, the qualitative and quantitative aspects were grouped and for the final analysis they were structured under insights from Moscovici's theory of social representations. In the Discussion, in general, the results obtained show the team's satisfaction with the use of telemedicine and highlight the driving role of the pandemic in its implementation. From different perspectives of the interdisciplinary team, the use of telemedicine was unveiled and potentially modifiable aspects emerged that can contribute to the improvement of its use. In this study, the consistency dimension was the biggest obstacle to be overcome and integration was the facilitator of implementation. It was concluded that the work carried out makes a significant contribution to the health sector, as it shows an overview of the use of telemedicine from the perspective of its main actors, the interprofessional team. And this scenario, although positive, revealed several potentially modifiable factors that will support a better use of technology

Os resultados de efetividade do uso de telemedicina em terapia intensiva em períodos pré-pandêmicos foram heterogêneos e possivelmente influenciados pela soberania do fator humano a tecnologia. Entender as experiências do mundo real, por meio das lentes de padrões de prática diária nas múltiplas facetas de interação interprofissional em terapia intensiva, em um período único como o da pandemia do COVID-19 pode ajudar a identificar estruturas e processos locais que facilitarão a efetiva implementação do uso da telemedicina na UTI. Os Objetivos da pesquisa foram caracterizar atitudes e percepções da equipe interprofissional sobre a utilidade e efetividade do uso da Telemedicina em UTIs em tempos de pandemia e a partir destes conhecimentos delinear propostas que otimizem seu uso. Os Participantes: Equipe interprofissional de saúde de Itapetininga que teve contato com a Telemedicina na UTI durante o período da Pandemia do COVID-19. A Metodologia utilizada foi estudo transversal, quali-quantitativo, de caráter exploratório realizado por meio de questionários online e entrevistas em grupos focais. A análise das respostas quantitativas foi efetuada usando o software IBM SPSS Statistcs 21. Os dados quantitativos foram descritos a partir de suas principais medidas de posição (média, mediana e desvio padrão) e frequência. Para análise das relações entre as variáveis categóricas sexo, público ou privado, faixa etária, categoria profissional com as percepções observadas foi realizado o teste de Pearson para tabelas de contingência. Nos casos de resultados significativos (p-valores <0,05) as análises foram suplementadas pela análise dos resíduos de Qui-quadrado (χ2). A Análise de Conteúdo dos GF foi do tipo temática e de construção das categorias não a priori de Bardin. Após a análise dos dados os aspectos qualitativos e quantitativos foram agrupados e para a análise final foram estruturadas sob insights da teoria das representações sociais de Moscovici. Na Discussão, de forma geral, os resultados obtidos mostram uma satisfação da equipe com o uso da telemedicina e destacam o papel propulsor da pandemia em sua implementação. Sob diferentes óticas de equipe interdisciplinar o uso da telemedicina foi desvendado e surgiram aspectos potencialmente modificáveis e que podem contribuir para a melhora de sua utilização. Nesse estudo a dimensão consistência foi o maior obstáculo a ser superado e a integração foi o facilitador à implantação. Concluiu-se que o trabalho realizado apresenta contribuição significativa para o setor da saúde, pois mostra um panorama do uso da telemedicina sob a ótica de seus principais atores, a equipe interprofissional.
