Resumo: Health Education (HE) is a well studied aspect of health promotion and disease prevention and medical formation and practice are key factors to its consolidation. The objective of this work was to investigate medical formation and HE practices performed by doctors in Primary Care System (PCS) context. The study was descriptive and interpretative, with a quanti-qualitative approach, based on the interviews of 6 doctors who work in the Brazilian Family-Centered Health Strategy and their 2 coordinators, focusing in their perception about formation in HE and educative practices performed. The interviews were analyzed through Bardin?s Content Analyses. We identified three categories: Formation in HE, with the subcategories Graduation, Post-Graduation and Permanent or Continuous Education; the category Practice in HE, with the subcategories Level of Practice, Multidisciplinary and Issue Selection; and the category Factors that Affect HE Effectiveness, with the subcategories Health Professional, Management and PCS Users. Regarding Formation, curriculum changes showed differences among the participants? speech, as well did Family and Community residency and other specific specializations for PCS practitioners. Regarding Practice, all participants reported HE practices, in both individual and collective levels. HE was a multidisciplinary practice present in all heath units (HU) researched. HE effectiveness was influenced by professionals? formation and profile, public department management, professional valorization, intersectionality, co-responsibility and culture. The results of groups? observation were triangulated with the interviews and professionals? data. Regarding the observed practices, all professionals used the lecture as method, with vertical transmission of information from doctor to patient, aiming individual changes in health behavior. Professionals with specific formation for PCS practice showed no innovative experiences, and neither the youngest did. Long-last acting in the same HU also showed no difference among HE practices. Moreover, the way of selection of the practice-guiding issues were not based in a local community diagnosis. We concluded that HE was present in professionals? speech and practices in different ways. While formation reflected important transformations that took place in graduation and postgraduation, practice demonstrated the constancy of inappropriate and updated performances. Multidisciplinary was a positive aspect found. Professionals? speech demonstrated the complexity of HE effectiveness if it is taken in its entirety. This study pointed out the need of strategies to translate research into practice in PCS, implementing scientific recommendations to heath care, considering that HE effectiveness is related to multiple interventions in multiples levels.

O objetivo do presente trabalho foi a investiga??o da forma??o e das pr?ticas de ES realizadas por m?dicos no contexto da Aten??o Prim?ria em Sa?de. O estudo qualitativo, de car?ter explorat?rio-descritivo e interpretativo, complementado com elementos quantitativos, baseou-se na entrevista de 6 m?dicos que atuam na Estrat?gia de Sa?de da Fam?lia e seus 2 respectivos coordenadores, com enfoque na percep??o a respeito da forma??o em ES e pr?ticas educativas realizadas. Investigou-se tamb?m a percep??o dos entrevistados sobre a efetividade da ES. Al?m disso, realizou-se a observa??o dos profissionais durante a??es educativas do tipo grupo e sala de espera. As entrevistas foram analisadas atrav?s da An?lise de Conte?do de Bardin. Identificaram-se tr?s categorias: Forma??o em ES, com as subcategorias Gradua??o, P?s-Gradua??o e Educa??o Permanente ou Continuada; categoria Atua??o em ES, com as subcategorias N?vel de Atua??o, Multidisciplinaridade e Sele??o Tem?tica; e a categoria Fatores que Influenciam a Efetividade da ES, com as subcategorias Profissional de Sa?de, Gest?o e Usu?rio. Na primeira, relatos sobre as mudan?as curriculares demonstraram diferen?a nos discursos dos entrevistados, bem como a resid?ncia em MFC e as especializa??es espec?ficas para atua??o em APS. Na segunda, quanto ? Atua??o, todos referiram realizar pr?ticas de ES, tanto no n?vel individual quanto coletivo; e em todas as unidades de sa?de (US) pesquisadas, a ES ocorre como uma pr?tica de car?ter multidisciplinar. Na terceira, a efetividade da ES mostrou-se influenciada pela forma??o e perfil do profissional, a administra??o do setor p?blico, a valoriza??o profissional, a intersetorialidade, a coresponsabiliza??o e a cultura. Os resultados da observa??o dos grupos foram triangulados com os das entrevistas e com os dados dos profissionais: nas pr?ticas educativas observadas, todos os profissionais utilizaram a metodologia da palestra, com a transmiss?o vertical das informa??es do m?dico para os usu?rios, objetivando a mudan?a individual do comportamento em sa?de. Os profissionais com forma??o espec?fica para atua??o em APS n?o mostraram experi?ncias inovadoras, nem tampouco os com forma??o mais recente. O tempo de atua??o na mesma US tamb?m n?o evidenciou diferen?a em rela??o ? pr?tica de ES. Al?m disso, o modo de sele??o dos temas para as a??es em ES n?o se seguiu ? execu??o de um diagn?stico em sa?de da comunidade local. Concluiu-se que a ES esteve presente no discurso e na pr?tica dos profissionais de formas diferentes: enquanto a forma??o refletiu transforma??es importantes ocorridas na gradua??o e na p?s-gradua??o, a atua??o demonstrou a perman?ncia de pr?ticas homogeneamente inadequadas e ultrapassadas. A multidisciplinaridade presente nas pr?ticas educativas foi um aspecto positivo encontrado. Se considerado no seu conjunto, o discurso dos profissionais demonstrou a complexidade que envolve a efetividade da ES.
