Resumo: | This thesis addresses the lifestyle change in people with end-stage renal failure (ESRF) undergoing hemodialysis (HD) and used a mindfulness-based intervention (MBI) to manage stress and quality of life, called the Hemomindful Program as an adjunct the usual treatment. In a first moment, it is presented a contextualization about the process of choosing the theme, theoretical basis and gaps that justify the realization of the study. Therefore, prevalence data, implications of ESRF and HD treatment on health and lifestyle conditions that influence the low quality of life of this population are presented. MBI are presented as a promising intervention for improving the health of the general and clinical population, as well as for the treatment of chronic pain. Finally, research using mind-body interventions, especially mindfulness, are presented with the aim of testing non-drug interventions that contribute to reducing stressors, improving the quality of life and adherence of people with ESRF to treatment programs. Thus, the main objective of this thesis is to evaluate the effect of an 8week MBI adapted to the context of HD on the pain and health conditions of people with ESRF. In order to answer this question, a randomized controleed trial of mixed methods was designed comparing the Hemomindful Program combined with the usual treatment (UT), with UT alone. 32 adults with ESRF, mean age 55 years (SD = 15.6), participated in an experiment lasting five months. To address the research problems that arose from the main objective, the thesis is divided into three articles. As main results, positive indicators of viability and safety of the intervention protocol stand out and the perceived pain intensity was significantly lower in the 3-month follow-up for the participants in the experimental group compared to the control. The Hemomindful Program presents itself as a promising adjuvant treatment to HD to reduce pain-related suffering and promote improvement in the health conditions of people with ESRF. In addition, the results of this thesis may contribute to expand investigations with this approach in the search for comprehensive health care based on evidence for this population. |

In addition, the results of this thesis may contribute to expand investigations with this approach in the search for comprehensive health care based on evidence for this population.Esta tese aborda a mudança do estilo de vida em pessoas com insuficiência renal de estágio terminal (IRET) submetidas à hemodiálise (HD) e utilizou uma intervenção baseada em mindfulness (MBI) para manejo do estresse e qualidade de vida, denominada Programa Hemomindful como adjuvante ao tratamento usual. Em um primeiro momento, é apresentada uma contextualização sobre o processo de escolha do tema, embasamento teórico e lacunas que justificam a realização do estudo. Logo, são apresentados dados de prevalência, implicações da IRET e do tratamento de HD sobre as condições de saúde e estilo de vida que influenciam na baixa qualidade de vida dessa população. As Intervenções baseadas em mindfulness (MBI) são apresentadas como uma possibilidade de intervenção promissora para melhora da saúde da população geral e clínica, bem como no tratamento da dor crônica. Por fim, são apresentadas pesquisas que utilizam intervenções mente-corpo, especialmente o mindfulness, com o objetivo de testar intervenções não medicamentosas que contribuam para a redução de estressores, melhora da qualidade de vida e adesão das pessoas com IRET aos programas de tratamento. Assim, o objetivo principal dessa tese é avaliar o efeito de uma MBI de 8 semanas adaptada para o contexto de HD sobre a dor e condições de saúde de pessoas com IRET. Para responder essa questão, desenhou-se um ensaio clínico randomizado de métodos mistos comparando o Programa Hemomindful combinado com o tratamento usual (TAU), com o TAU sozinho. Participaram 32 adultos com IRET, média de idade de 55 anos (DP=15,6), de um experimento com duração de cinco meses. Para endereçar os problemas de pesquisa que decorreram do objetivo principal, a tese desdobra-se em três artigos. Como principais resultados, destacam-se indicadores positivos de viabilidade e segurança do protocolo de intervenção e a intensidade da dor percebida ter sido significativamente menor no acompanhamento de 3 meses para os participantes do grupo experimental em comparação ao controle. O Programa Hemomindful apresenta-se como um tratamento adjuvante à HD promissor para reduzir o sofrimento relacionado à dor e promover melhora de condições de saúde de pessoas com IRET.
