Resumo: Introduction: The effects of Spinal Cord Injury (SCI) include impairment of mobility, which requires the use of manual wheelchairs. Assessing the performance of wheelchair skills is essential since it is related to the quality of life of the wheelchair user. With this in mind, the Wheelchair Mobility Activity Log (WC-MAL) instrument was developed, which assesses the actual use of a manual wheelchair in people who have suffered an SCI. For the instrument to be more accurate, COSMIN recommends that its structural validity be measured. Objective: To assess the structural validity of the WC-MAL questionnaire applied to individuals with SCI and manual wheelchair users. Methods: A descriptive and quantitative cross-sectional study with a non-probabilistic convenience sample included 60 people diagnosed with SCI who use a manual wheelchair. The WC-MAL instrument was used to assess the actual mobility of wheelchair users using video calls. To assess the structural validity of the instrument, Rasch analysis was carried out, which evaluates the unidimensionality of the instrument's items and the relationship between them. The data collected was analyzed using Winsteps version 3.91.0. Results: By hierarchically ordering the items according to difficulty, it was observed that the easiest items were aligned with individuals with higher and more complete injuries, as well as with those who reported suffering shoulder pain, indicating low wheelchair skills. The more complex items, on the other hand, were commonly performed by individuals who had greater skills, in this case, individuals classified with lumbar and thoracic injuries. The DIF analysis found that the variables gender, level, and type of injury and shoulder pain could interfere with the responses and results of the instrument, representing a risk of bias. Items, where erratic or predictable behavior was observed in the Frequency, Performance, and Assistance scales, were excluded from the new version. No ceiling effect was observed; reliability and structural validity were adequate. Conclusion: The WC-MAL is suitable for assessing wheelchair performance by people with MSDs, but it was necessary to exclude items that did not fit the model. WC-MAL 2.0 was therefore prepared for publication.

WC-MAL 2.0 was therefore prepared for publication.Introdução: Dentre os acometimentos da Lesão da Medula Espinal (LME), está o prejuízo à mobilidade ao indivíduo, que passa a ser realizada através do uso de cadeiras de rodas manual. Avaliar o desempenho de habilidades em cadeira de rodas é essencial, já que está relacionada à qualidade de vida do indivíduo cadeirante. Pensando nisso, foi desenvolvido o instrumento Whellchair Mobility Activity Log (WC-MAL), que avalia a percepção do uso real da cadeira de rodas manual em pessoas que sofreram LME. Para uma maior precisão do instrumento, o COSMIN recomenda que a validade estrutural seja mensurada. Objetivo: Avaliar a validade estrutural do questionário WC-MAL aplicada em indivíduos com LME e usuários de cadeira de rodas manual. Métodos: Estudo descritivo e quantitativo, num plano transversal com amostra não probabilística e por conveniência, incluiu 60 pessoas com diagnóstico de LME e que fazem uso de cadeira de rodas manual. Foi utilizado o instrumento WC-MAL para avaliar a mobilidade real dos indivíduos cadeirantes por meio de chamadas de vídeo. Para a avaliação da validade estrutural do instrumento, foi realizada a Análise de Rasch, que avalia a unidimensionalidade dos itens do instrumento e a relação entre eles. Os dados coletados foram analisados em programa específico Winsteps versão 3.91.0. Resultados: Pelo ordenamento hierárquico dos itens de acordo com a dificuldade, foi observado que os itens mais fáceis estavam alinhados com os indivíduos com lesões mais altas e completas, bem como, com aqueles que relataram sofrer dor no ombro, indicando baixa habilidade com a cadeira de rodas. Já os itens mais complexos, eram comumente realizados por indivíduos que possuíam maiores habilidades, no caso, indivíduos classificados com lesão lombar e torácica. Pela análise DIF, foi encontrado que as variáveis sexo, nível e tipo da lesão e dor no ombro podem apresentar algum risco para viés em itens específicos, exigido cautela da interpretação das mudanças em um item isolado.. Os itens onde se observou comportamento errático ou previsível nas escalas de Frequência, Desempenho e Assistência foram excluídos da nova versão. Não foi observado efeito teto; confiabilidade e validade estrutural se mostraram adequadas. Conclusão: A WC-MAL é adequada para avaliação do desempenho em cadeira de rodas por pessoas com LME, porém, foi necessária a exclusão de itens que não se encaixaram no modelo previsto.
