Resumo: | In recent years, although the literature has made progress in investigating the quality of care in Long-Term Care Facilities for the Elderly (LTCFs), the humanization of care in these services still faces numerous challenges. Among them are the lack of funding, human resources, government subsidies, and mismatches between the health needs of residents and the care offered. In Brazil, the implementation of this type of care requires short-, medium-, and long term interventions, with a view to training professionals who work with elderly people, raising awareness in society and government representatives for the creation of public policies, offering educational interventions to care providers, and increasing research and studies on the subject. Based on this, the objectives of this study were: a) to investigate the difficulties and challenges of care in LTCFs from the perspective of professionals working in Brazilian institutions; b) to verify possible relationships between measures of well-being, attitudes towards old age, job satisfaction and individual-centered care practices in professionals working in LTCFs; and c) to verify the possible contributions of an educational intervention in the scenario of care offered. To this end, professionals working in Brazilian public, philanthropic and private LTCFs were investigated during the first and second half of 2022. Participants were assessed using semi structured questionnaires, an inventory of attitudes towards old age, an inventory of practices related to individual-centered care, and satisfaction and work-related overload. To assess the difficulties and challenges associated with care, data from a focus group conducted via web conference were analyzed, followed by a training and updating cycle on the care offered in LTCFs, consisting of eight meetings lasting 120 minutes. Qualitative data were transcribed and analyzed based on the thematic analysis proposed by Minayo (2010), and quantitative data were analyzed using parametric and nonparametric statistical tests, with a significance value <0.05. Regarding the challenges related to the quality of care in LTCFs in Brazil, professionals highlighted factors such as social and health demands, lack of resources, inadequate training of professionals, and stigmas associated with institutionalization that directly impact care. To overcome these challenges, participants suggested that LTCFs should act as mediators, promoting qualified listening and integrating the family into care. In the field of public policies, ways to overcome violations and shortages stand out, while in education, it is vital to train professionals capable of dealing with the needs of older adults. The focus group analysis showed the limits and advances in care during the 2022 pandemic, reflecting the need to reconfigure both work in institutions and public policies and financing. Regarding possible associations between variables, associations were observed between negative attitudes towards old age and symptoms of Burnout; and an association between lower levels of job satisfaction (except in the “Policy and strategy” domain) and poorer quality of person-centered care in LTCFs. In addition, the length of time working with older people, and not the length of time in LTCFs, is also related to job dissatisfaction. The results reinforce the need for educational interventions to qualify professionals and improve working conditions, which may result in an increase in the supply of qualified care. Regarding the educational intervention, there were no differences between measures of job satisfaction, attitudes towards old age, Burnout and person-centered care among participants who remained in the intervention and among those who discontinued the continuing education program. However, the training cycle proved useful in promoting a collaborative environment and in reflecting on professional practices, although the direct impact on working conditions and the quality of care provided was not statistically significant. The continuity of such programs, with a focus on collaboration between different social actors, is essential to improve working conditions and the quality of care in LTCFs. In Brazil, there are few programs for care and continued training for professionals working in LTCFs. It is expected that this work will contribute to the articulation; construction and strengthening of a line of continued care for institutionalized elderly people in Brazil. |

Dentre eles, estão a escassez de financiamento, a oferta de recursos humanos, os subsídios governamentais e os descompassos entre as necessidades de saúde dos residentes e as ofertas assistenciais. No Brasil, a implantação dessa modalidade de cuidados solicita intervenções a curto, médio e longo prazo, com vistas a formar profissionais que atuem com pessoas idosas, sensibilizando a sociedade e os representantes do poder público para a criação de políticas públicas, oferecendo intervenções educacionais aos provedores de cuidados e aumentando as pesquisas e estudos sobre o tema. Com base nisso, os objetivos deste estudo foram: a) investigar as dificuldades e os desafios dos cuidados em ILPIs na perspectiva dos profissionais que atuavam nas instituições brasileiras; b) verificar possíveis relações entre medidas de bem-estar, atitudes em relação à velhice, satisfação em relação ao trabalho e práticas de cuidados centrados no indivíduo, em profissionais que atuam nas ILPIs; e c) verificar as possíveis contribuições de uma intervenção educacional no cenário dos cuidados ofertados. Para isso, foram investigados profissionais que atuavam em ILPIs brasileiras — públicas, filantrópicas e privadas — durante o primeiro e o segundo semestre de 2022. Os participantes foram avaliados por questionários semiestruturados, inventário de atitudes sobre a velhice, inventário de práticas relacionadas aos cuidados centrados no indivíduo, satisfação e sobrecarga relacionada ao trabalho. Para levantamento das dificuldades e desafios associados aos cuidados, foram analisados dados de grupo focal conduzido via web conferência, seguido por ciclo de formação e atualização sobre os cuidados ofertados nas ILPIs, composto por oito encontros com duração de 120 minutos. Os dados qualitativos foram transcritos e analisados com base na análise temática proposta por Minayo (2010), e os dados quantitativos foram analisados por meio de testes estatísticos paramétricos e não paramétricos, com valor de significância < 0,05. No que tange aos desafios relacionados à qualidade dos cuidados nas ILPIs no Brasil, os profissionais destacaram fatores como as demandas sociais e de saúde, falta de recursos, formação inadequada dos profissionais e estigmas associados à institucionalização, que impactaram diretamente no atendimento. Para superar esses desafios, os participantes sugeriram que as ILPIs devem atuar como mediadoras, promovendo uma escuta qualificada e integrando a família nos cuidados. No campo das políticas públicas, destacam-se caminhos para superar violações e escassez, enquanto, na educação, é vital formar profissionais capacitados para lidar com as necessidades das pessoas idosas. A análise do grupo focal mostrou os limites e avanços dos cuidados durante a pandemia de 2022, refletindo a necessidade de reconfigurar tanto o trabalho nas instituições quanto as políticas públicas e o financiamento. No que se refere às possíveis associações entre as variáveis, observou-se associações entre atitudes negativas em relação à velhice e sintomas de Burnout; e associação entre menores níveis de satisfação no trabalho (exceto no domínio “Política e estratégia”) e a pior qualidade de cuidados centrados no indivíduo em ILPIs. Além disso, o tempo de trabalho com pessoas idosas, e não o tempo nas ILPIs, também está relacionado à insatisfação profissional. Os resultados reforçam a necessidade de intervenções educacionais para qualificação dos profissionais e de melhores condições de trabalho, o que pode resultar no aumento da oferta de cuidados qualificados. No que tange à intervenção educacional, não houve diferenças entre as medidas de satisfação em relação ao trabalho, atitudes em relação à velhice, Burnout e cuidados centrados no indivíduo entre os participantes que permaneceram na intervenção e entre os que descontinuaram o programa de formação continuada. Entretanto, o ciclo de formação revelou se útil na promoção de um ambiente colaborativo e na reflexão sobre práticas profissionais, embora o impacto direto nas condições de trabalho e na qualidade dos cuidados oferecidos não tenha sido significativo do ponto de vista estatístico. A continuidade de tais programas, com foco na colaboração entre diversos atores sociais, é essencial para melhorar as condições de trabalho e a qualidade dos cuidados em ILPIs. No Brasil, são escassos programas de atenção e formação continuada de profissionais que atuam nas ILPIs. Participants were assessed using semi structured questionnaires, an inventory of attitudes towards old age, an inventory of practices related to individual-centered care, and satisfaction and work-related overload. To assess the difficulties and challenges associated with care, data from a focus group conducted via web conference were analyzed, followed by a training and updating cycle on the care offered in LTCFs, consisting of eight meetings lasting 120 minutes. Qualitative data were transcribed and analyzed based on the thematic analysis proposed by Minayo (2010), and quantitative data were analyzed using parametric and nonparametric statistical tests, with a significance value <0.05. Regarding the challenges related to the quality of care in LTCFs in Brazil, professionals highlighted factors such as social and health demands, lack of resources, inadequate training of professionals, and stigmas associated with institutionalization that directly impact care. To overcome these challenges, participants suggested that LTCFs should act as mediators, promoting qualified listening and integrating the family into care. In the field of public policies, ways to overcome violations and shortages stand out, while in education, it is vital to train professionals capable of dealing with the needs of older adults. The focus group analysis showed the limits and advances in care during the 2022 pandemic, reflecting the need to reconfigure both work in institutions and public policies and financing. Regarding possible associations between variables, associations were observed between negative attitudes towards old age and symptoms of Burnout; and an association between lower levels of job satisfaction (except in the “Policy and strategy” domain) and poorer quality of person-centered care in LTCFs. In addition, the length of time working with older people, and not the length of time in LTCFs, is also related to job dissatisfaction. The results reinforce the need for educational interventions to qualify professionals and improve working conditions, which may result in an increase in the supply of qualified care. Regarding the educational intervention, there were no differences between measures of job satisfaction, attitudes towards old age, Burnout and person-centered care among participants who remained in the intervention and among those who discontinued the continuing education program. However, the training cycle proved useful in promoting a collaborative environment and in reflecting on professional practices, although the direct impact on working conditions and the quality of care provided was not statistically significant. The continuity of such programs, with a focus on collaboration between different social actors, is essential to improve working conditions and the quality of care in LTCFs. In Brazil, there are few programs for care and continued training for professionals working in LTCFs. It is expected that this work will contribute to the articulation; construction and strengthening of a line of continued care for institutionalized elderly people in Brazil. |
