Resumo: The more elaborate identification of geronte cognitive disorders, according to current protocols, will validate the construction of solid bases for specific treatments with optimization of expenses and relevant results. In reference clinics in the Federal District we do not yet have an evaluation of subtypes of dementias with differential diagnosis, establishing a gap between the necessary assistance to the patients with the diseases, their families and public policies known. To establish a differential diagnosis of subtypes of dementias in a reference outpatient clinic in geriatrics in the Federal District, calculating their prevalence by primary source, prioritizing better accuracy through clinical examination and multifunctional evaluation. This is a longitudinal, retrospective, ecological and documentary study. Will be performed in an outpatient clinic specializing in Geriatrics. Population and sample comprised of people aged 60 years or over residing in the Federal District, who have cognitive deficits characterized by Major Neurocognitive Disorder (MND), registered during the years 2010 to 2018. Data collection will be performed in medical records. select and evaluate the profile of the elderly with diagnosis of MND followed by a broad geriatric evaluation and multifunctional evaluation. The data analysis will be performed with the calculation of prevalence, descriptive statistics and correlations through the coefficient V of Cramer. In response to the hypothesis and problems presented, a review of the literature was done, article 1, as well as a profile of the studied population and the subtypes of dementia found, article 2. Research on dementia in Brazil is still incipient, counting with few studies. In this study, 158 individuals were studied. 52.5% are 80 to 89 years old, 62.5% are women and 62.7% are Caucasian, 50.6% are widowers and 47.5% are illiterate. The initial prevalence of AD was 45.6%, decreasing to 35.4% after a follow-up period and that DV was initially 34.2% and 45.6% at the end. We used Cramer's V-Coefficient, where we found a weak or very weak relationship of risk factors with the diagnoses of dementias presented. Low schooling and AD (V 0.257), DV (V 0.052); arterial hypertension and AD (V 0.343), DV (V 0.259); obesity and AD (V 0.202), DV (V 0.263); hypoacusis and AD (V 0.027), DV (V 0.1777); smoking and AD (V 0.021), DV (V 0.076); depression and AD (V 0.060), DV (V 0.1119); sedentary lifestyle and AD (V 0.017), DV (V 0.020); social isolation and AD (V 0.160), DV (V 0.086), diabetes and AD (V 0.009), DV (V 0.0004). This dissertation contributes socially, as it disseminates and clarifies population profile, diagnostic subtypes, difficulties and complexity of evaluations, punctuating current gaps, opening possibilities for new research, developing a more critical and grounded view on reality. Tools derived from the theme, also greatly increase the aid in health education, one of the basic premises of SUS.

Em ambulatórios de referência do Distrito Federal, ainda não se conta com avaliação dos subtipos de demências com diagnóstico diferencial, estabelecendo-se uma lacuna entre a assistência necessária aos portadores das doenças, suas famílias e políticas públicas conhecidas. O objetivo deste trabalho foi estabelecer diagnóstico diferencial dos subtipos das demências em ambulatório de referência em geriatria no Distrito Federal, calculando-se sua prevalência por fonte primária, priorizando-se melhor acurácia através de exame clínico e avaliação multifuncional. Tratou-se de estudo longitudinal, retrospectivo, ecológico e documental realizado em ambulatório especializado em geriatria, com população e amostra composta por pessoas com 60 anos ou mais, residentes no Distrito Federal, portadores de déficit cognitivo caracterizado por Transtorno Neurocognitivo (TNC) Maior (demência), cadastradas durante os anos de 2010 a 2018. A coleta de dados foi realizada em prontuários para selecionar e avaliar o perfil do idoso com diagnóstico de TNC, seguida de avaliação geriátrica ampla e avaliação multifuncional. A análise de dados foi realizada com o cálculo da prevalência, estatística descritiva e correlações através do coeficiente V de Cramer. Em resposta à hipótese e problemas apresentados, efetuou-se revisão de literatura, Artigo 1, bem como estabeleceu-se perfil da população estudada e os subtipos de demência encontrados, Artigo 2. São ainda incipientes as pesquisas em demência no Brasil, contando-se com poucos estudos. Neste trabalho, estudou-se 158 indivíduos. 52,5% possuem de 80 a 89 anos, 62,5 % são mulheres e 62,7% caucasianos, 50,6% viúvos e 47,5% analfabetos. A prevalência inicial de Demência da Alzheimer (DA) foi de 45,6%, reduzindo-se para 35,4% após um período de acompanhamento e da Demência Vascular (DV) foi de 34,2% inicialmente e 45,6% ao final. Utilizamos o Coeficiente V de Cramer, onde encontramos uma relação fraca ou muito fraca de fatores de risco com os diagnósticos das demências apresentados que elucidamos a seguir: baixa escolaridade e DA (V 0,257), baixa escolaridade e DV (V 0,052); hipertensão arterial e DA (V 0,343), hipertensão arterial e DV (V 0,259); obesidade e DA (V 0,202), obesidade e DV (V 0,263); hipoacusia e DA (V 0,027), hipoacusia e DV (V 0,177); tabagismo e DA (V 0,021), tabagismo e DV (V 0,076); depressão e DA (V 0,060), depressão e DV (V 0,119); sedentarismo e DA (V 0,017), sedentarismo e DV (V 0,020); isolamento social e DA (V 0,160), isolamento social e DV (V 0,086), diabetes e DA (V0,009), diabetes e DV (V 0,0004). Esta dissertação contribui socialmente, à medida que divulga e esclarece perfil populacional, subtipos diagnósticos, dificuldades e complexidade de avaliações, pontuando hiatos atuais, abrindo possibilidades de novas pesquisas, desenvolvendo olhar mais crítico e fundamentado sobre a realidade.
