Resumo: | The provision of qualified care to institutionalized elderly in Brazil still challenges managers, professionals, public policymakers and society itself. After 15 years of the promulgation of the guidelines for the operation of Long Stay Institutions for the Elderly (LSIE), by the Health Surveillance Agency, mismatches between the recommended and the offered by these services are still observed. Objective: To investigate, from the perspective of managers and technicians in charge of Brazilian LSIEs, what are the challenges to ensure the proper functioning of institutions, based on the Resolution of the Collegiate Board of Directors - RDC 502 2021, as well as to describe which proposals for institutions to achieve the provisions set out in the RDC and suggestions for reformulating this resolution. Method: This is a cross-sectional, exploratory and quantitative study, in which forms built and made available through the Google Forms tool were applied with questions related to the challenges, proposals and suggestions, faced by LSIEs in elderly care, based on the understanding of RDC 502 2021. Results: Of the 90 institutions accessed, more than half were located in the Southeast region of Brazil (51.1%) and most were characterized as private non-profit (34.4%). The biggest challenge of the referenced was the availability of financial resources to offer care and general expenses of the institution, followed by availability of human resources, Adequacy of the Property and the institution's facilities, Coverage and service of the Unified Health System, Medical and health follow-up and Offer of leisure activities. Other challenges permeated dimensions such as health, relationship with supervisory bodies and with the family. To achieve the provisions set out in the RDC, the following actions were mentioned: improvement and training of professionals working in the LSIEs, the review of the infrastructure for routine activities, the need for greater financial support from the State, and the application of realistic proposals of inspection and compatible with the reality of each LSIE. In this last topic, it is not clear which parameters of the RDC should be re-elaborated and how they should be operationalized. Regarding the actions to reform the RDC, most actions were identified as emerging, and the most prevalent was “to compose ancillary laws that regulate the minimum funding of institutionalized elderly care by municipalities and states”, “ make municipal and state managers co-responsible for actions to strengthen institutions" and "refer guidelines for pacting the care of the institutionalized elderly by the region's health units" Conclusion: Facing of the challenges and suggestions reported, it is observed that the debate on reformulations in the DRC must be accompanied by actions that provide better working and operating conditions for LSIEs, and this can permeate the domain of drawing up public policies that aim to reduce the probabilities of financial shortages and difficulties related to structural and human resources aspects. |

Após 15 anos da promulgação das diretrizes para o funcionamento das Instituições de Longa Permanência para Idosos (ILPIs), pela Agência de Vigilância Sanitária, ainda se observam descompassos entre o preconizado e o ofertado por esses serviços. Objetivo: Investigar, pela ótica dos gestores e responsáveis técnicos de ILPIs brasileiras, quais são os desafios para garantir o adequado funcionamento das instituições, com base na Resolução da Diretoria Colegiada - RDC 502 2021, bem como descrever quais as propostas para que as instituições alcancem as disposições previstas na RDC e sugestões para reformulação desta resolução. Método: Trata-se de estudo transversal, exploratório e quantitativo, em que foram aplicados formulários construídos e disponibilizados por meio da ferramenta do Google Forms com questões relativas aos desafios, propostas e sugestões frente aos cuidados dos idosos nas ILPIs, e a compreensão da RDC 502 2021. Resultados: Das 90 instituições acessadas, mais da metade localizava-se na região Sudeste do Brasil (51,1%) e a maioria caracterizava-se como privada sem fins lucrativos (34,4%). O maior desafio referenciado foi a disponibilidade de recursos financeiros para oferta dos cuidados e gastos gerais da instituição, seguido por disponibilidade de recursos humanos, Adequação do Imóvel e instalações da instituição, Cobertura e atendimento do Sistema Único de Saúde, Acompanhamento médico e de saúde e Oferta de atividades de lazer. Outros desafios permearam dimensões como saúde, relação com os órgãos de fiscalização e com a família. Para o alcance das disposições previstas na RDC foram citadas as ações: aperfeiçoamento e treinamento dos profissionais atuantes nas ILPIs, a revisão da infraestrutura para atividades rotineiras, a necessidade de maior apoio financeiro do Estado, e a aplicação de propostas realistas de fiscalização e compatível com a realidade de cada ILPI Nesse último tópico não fica claro quais parâmetros da RDC devem ser reelaborados e de que forma devem ser operacionalizados. No que se refere as ações para reformulação da RDC, a maioria das ações foi sinalizada como de implantação emergente, e as mais prevalentes foram “compor leis acessórias que regulamentem o financiamento mínimo dos cuidados aos idosos institucionalizados por parte dos municípios e estados”, “corresponsabilizar os gestores municipais e estaduais nas ações de fortalecimento das instituições” e “referenciar diretrizes para pactuação do atendimento dos idosos institucionalizados pelas unidades de saúde da região”.
