Resumo: The cancer is considered a genetic disease once it results from alterations in genes involved with DNA repair or in genes controlling the cellular proliferation and differentiation. The oral cancer is preceded by precursor stages that induce some changes in cells of oral epithelium. The biomarkers uses are essential for monitoring of premalignant lesions. Epidemiological studies have shown that oral cancer and premalignant lesions are associated with many factors, emphasizing tobacco smoking and alcohol drinking, mainly when in concomitance. The aim of this study was to evaluate through a differentiated protocol of the Micronucleus Test, the frequencies of micronuclei and degenerative nuclear alterations indicative of apoptosis, in exfoliated cells from oral epithelium of individuals presenting premalignant and malignant lesions of this epithelium and possible interaction with smoking habits. The analysed sample consisted of 20 individuals with oral cancer; 11 individuals with premalignant lesions and 72 individuals without oral lesions. Cells were obtained from normal and lesion areas of the cheek by scrapping with a cervical brush. Smears were stained according to Feulgen/ Fast Green. Analysis was performed under light microscopy in a blind fashion in a minimum of 1,000 cells. The frequency of micronucleus was significantly higher in individuals with oral lesions (cells obtained from lesions area), as compared with the frequency observed in individuals without oral lesions and in normal area cells of individuals with lesions (p<0.00001). The frequency of micronucleus was significantly higher between women; between smokers and mouthwashes use. A significant stepwise increase in the frequency of micronuclei was observed in buccal epithelium exfoliated cells, from individuals without lesions to individuals with cancer. Considering the obtained in normal area cells of individuals with lesions e without lesions were not detected differences significant. The frequency of apoptosis was significantly lower in the individuals with oral lesions (as considering lesions exfoliated cells or normal exfoliated cells mucosa) than observed individuals without lesions (P<0.0001). These results show that: 1) the process of malignant transformation involves an increase in the frequency of chromosomal damage and the loss of cell ability in evolving to apoptosis; 2) the components of tobacco are effective to induce micronucleus.

O objetivo deste estudo foi avaliar, atrav?s de protocolo diferenciado do Teste de Micron?cleo, a ocorr?ncia de altera??es citol?gicas, indicativas de danos cromoss?micos e de apoptose em c?lulas esfoliadas do epit?lio oral de indiv?duos com les?es pr?-malignas e malignas deste epit?lio e a poss?vel intera??o com o h?bito de fumar. A amostra analisada incluiu 20 indiv?duos com les?es malignas, 11 indiv?duos com les?es pr?-malignas e 72 indiv?duos que n?o possuem les?es bucais. O material para an?lise foi coletado por raspagem da mucosa bucal, das regi?es com les?o e com aspecto normal, utilizando escova cytobrush. Com o material coletado foi feito esfrega?o posteriormente fixado em solu??o de metanol/?cido ac?tico (3:1). As prepara??es coradas pelo Fe?lgen/Fast Green, foram analisadas em teste cego sob microscopia ?ptica em um m?nimo de 1000 c?lulas. A ocorr?ncia de micron?cleos entre os indiv?duos com les?es bucais (?rea de les?o) foi significativamente maior do que o observado tanto nos indiv?duos sem les?es bucais quanto quando feita compara??o com a mucosa normal dos indiv?duos com les?es bucais (p<0,00001). N?o foram observadas diferen?as significantes na freq??ncia dessas estruturas quando comparadas c?lulas coletadas do epit?lio oral sem altera??o dos portadores de les?es e c?lulas obtidas dos indiv?duos sem les?es. O n?mero de micron?cleos foi significativamente maior entre as mulheres, entre fumantes e entre os indiv?duos que faziam uso de antiss?ptico. Considerando a ocorr?ncia de micron?cleos entre os grupos em fun??o do diagn?stico (material obtido das ?reas de les?o), observa-se que a ocorr?ncia de micron?cleos entre os portadores de c?ncer foi significativamente maior do que a observada para os indiv?duos com les?es pr?-malignas e indiv?duos sem les?es orais (p<0,01). N?o h? diferen?a significante na ocorr?ncia de micron?cleos quando comparadas c?lulas coletadas da ?rea normal dos portadores de les?es malignas, pr?-malignas e dos indiv?duos sem les?es (p=0,1618). A ocorr?ncia de apoptose foi significativamente menor nas c?lulas coletadas tanto das ?reas de les?es malignas e pr?-malignas, quanto das ?reas de mucosa normal dos indiv?duos portadores de les?es quando comparados ?s c?lulas dos indiv?duos sem les?es (p< 0,0001). These results show that: 1) the process of malignant transformation involves an increase in the frequency of chromosomal damage and the loss of cell ability in evolving to apoptosis; 2) the components of tobacco are effective to induce micronucleus.
