Resumo: | INTRODUCTION: Obesity is considered a pathological condition in which excess adipose tissue replaces healthy tissue, causing functional changes in the body's most diverse organic systems. The respiratory system is mostly penalized with this condition since the resistance imposed by excess weight will cause changes in ventilatory mechanics, thus reducing lung volumes and capacities. Therefore, the spirometry exam aims to quantify ventilatory disorders, which is essential in clinical practice. The positioning in the test execution is a significant challenge to be evaluated by the examiner since it is possible to find changes in values quantified by spirometry with the change of positioning of individuals in the test OBJECTIVE: To propose a positioning protocol for the execution of the Spirometry in obese individuals, grade III. METHODOLOGY: This is a cross-sectional, descriptive study with a quantitative approach, and the measures of analysis are the spirometric variables of forced vital capacity (FVC) and forced expiratory volume in the first second (FEV1). The sample was composed of 30 grade III individuals (BMI> 40) who agreed to participate in the research and signed a Free and Informed Consent Term. All data obtained in this study were statistically evaluated. The experimental design adopted was completely randomized blocks. The results were analyzed by comparing the averages using the test and Tukey, with statistical significance at the level of 5% probability. Data analysis performed using the Sisvar software. RESULTS: the final sample of the study was 30 patients of both sexes, being the number of women (n = 16) and men (n = 14), the patients were submitted to the spirometric test in different positions to measure the FEV1 variables (volume expired in the first second) and FVC (forced vital capacity). They presented 73, 33% of the patients had values different from the standard value for the variable FEV1 in the different positions, and the variable FVC 86.66% of the participants diverged in absolute values from the value considered as normal. When assessing the ratio between these variables (FEV1 / FVC), 83.33% of the sample patients differed from the values considered normal. FINAL CONSIDERATIONS: Checking the position in the spirometric test's performance allows bringing more accuracy to the exam, thus providing which clinical and therapeutic treatment strategy is best suited to grade III obese individuals. |

O sistema respiratório é amplamente penalizado com essa condição uma vez a resistência imposta pelo excesso de peso ocasionará alteração na mecânica ventilatória, reduzindo desta forma os volumes e capacidades pulmonares. Portanto o exame de espirometria se propõe a quantificar os distúrbios ventilatórios, sendo este essencial na prática clínica. O posicionamento na execução do teste constitui desafio importante a ser avaliado pelo examinador, uma vez que é possível se encontrar alteração de valores quantificados pela espirometria com a mudança de posicionamento dos indivíduos no teste OBJETIVO: Propor qual o melhor posicionamento para a execução do Teste de Espirometria em indivíduos obesos, grau III. METODOLOGIA: Trata-se de um estudo transversal, descritivo com abordagem quantitativa e tem como medidas de análise as variáveis espirometricas de capacidade vital forçada (CVF) e volume expiratório forçado no primeiro segundo (VEF1), a amostra foi composta por 30 indivíduos grau III (IMC>40) que aceitaram participar da pesquisa e assinaram um Termo de Consentimento Livre e Esclarecido Todos os dados obtidos neste estudo foram avaliados estatisticamente. O delineamento experimental adotado foi o de blocos inteiramente casualizado. Os resultados foram analisados através da comparação das médias utilizando-se o teste e Tukey, com significância estatística ao nível de 5% de confiança. A análise dos dados foi realizada por meio do software Sisvar. RESULTADOS: a amostra final do estudo foi de 30 pacientes de ambos os sexos, sendo o quantitativo de mulheres (n=16) e homens (n=14), os pacientes foram submetidos ao teste espirométrico em posições distintas para mensurar as variáveis FEV1 (volume expirado no primeiro segundo) e FVC (capacidade vital forçada). Apresentaram resultado de 73, 33% dos pacientes tiveram valores distintos do valor padrão para a variável FEV1 nas diferentes posições, bem como para a variável FVC 86,66% dos participantes divergiram em valores absolutos do valor tido como normal. Ao avaliar a razão entre essas variáveis (FEV1/FVC) 83,33% dos pacientes constituintes da amostra divergiram dos valores tidos como padrão de normalidade.
