Resumo: The present dissertation contributes to the studies related to the influences of the private hegemony apparatus of the representative capital of the imperialist countries , World Bank (WB) and the World Health Organization (WHO). The objective was to analyze the trends proposed by the devices deprived of BM / WHO hegemony, in the perspective of domination and resistance in the State's counter-reform process and its expressions in Brazilian health policy. This study proved to be relevant for unveiling some processes offensive to the Brazilian health policy, mediated by the contemporary capitalist crisis, by the new configurations of the imperialism and its relationship with dependent countries. For the countries with a dependent economy, such as the Brazilian one, there are harmful implications to working and living conditions of the working class, which affect the overexploitation and expropriation of social rights, with an emphasis on the right to universal health, as advocated by the Unified Health System (UHS). This research is based on the Critical Social Theory, being carried out through bibliographic review and document analysis. The study covers the period 2003 to 2018, and the governments of the Workers' Party and the return to neoliberal orthodoxy under Michel Temer. The results pointed to the facts that the private devices of hegemony, WB and WHO, direct their propositions towards health policy, management reform and health spending. These private devices of hegemony are offensive to the Brazilian National Health Policy, in the sense of commodification of health, as a niche for capital accumulation. They encourage the definancing and health spending reductions, as well as the privatization of the Unified Health System (SUS), through Social Organizations, Public-Private Partnerships (PPP) and the Brazilian Company of Hospital Services (BCHS), which has provided disruption of the public health network, by privileging the purchase of private services. In this way, the WB and the WHO present proposals for de-financing, privatization, targeting and selectivity of the health policy. With regard to resistance, it is denominated counter hegemonic apparatus the organization and mobilization of the National Front Against the Privatization of Health, which is contrary to the process of counter-reform of the State and its expressions in health policy. The organization is an anti-capitalist front and acts in defense of state public health.

The organization is an anti-capitalist front and acts in defense of state public health.A presente dissertação contribui com os estudos referentes às influências dos aparelhos privados de hegemonia do capital representativo dos países imperialistas, Banco Mundial (BM) e a Organização Mundial da Saúde (OMS), especificamente, sobre as políticas de saúde dos países dependentes; bem como, oferece substratos teórico-conceituais para a formação, atuação e militância de profissionais da área de Serviço Social e afins e outros sujeitos – individuais e coletivos – numa perspectiva de defesa da política pública de saúde e dos sistemas nacionais de saúde. O objetivo foi analisar as tendências propostas pelos aparelhos multilaterais de hegemonia BM/OMS, na perspectiva de dominação e resistência no processo de contrarreforma do Estado e as suas expressões na política de saúde brasileira. Este estudo mostrou-se relevante por desvelar alguns processos ofensivos à política de saúde brasileira, mediada pela crise capitalista contemporânea, pelas novas configurações do imperialismo e por sua relação com os países dependentes. Para os países de economia dependente, como a brasileira, existem implicações nefastas nas condições de vida e de trabalho da classe trabalhadora, que incidem na superexploração e expropriação dos direitos sociais, enfatizado o direito à saúde universal, como preconiza o Sistema Único de Saúde (SUS). Esta pesquisa se embasou na Teoria Social Crítica, sendo executada por meio da revisão bibliográfica e da análise documental. O período do estudo foi de 2003 a 2018, durante os governos do Partido dos Trabalhadores e do aprofundamento do neoliberalismo demarcado enquanto terceira fase neoliberal, no governo Michel Temer. Os resultados apontaram que os aparelhos privados de hegemonia, BM e a OMS, direcionam suas proposições à política de saúde, à reforma gerencial e aos gastos em saúde. Esses aparelhos privados de hegemonia encaminham ofensivas à Política Nacional de Saúde brasileira, no sentido da mercantilização da saúde, enquanto nicho de acumulação do capital. Estimulam o desfinanciamento e a diminuição dos gastos com a saúde; bem como a privatização do Sistema Único de Saúde (SUS), através das Organizações Sociais, das Parcerias Público-Privadas (PPP) e da Empresa Brasileira de Serviços Hospitalares (EBSERH), que tem propiciado a desestruturação da rede pública de saúde, ao privilegiar a compra de serviços privados. Desse modo, o BM e a OMS apresentam proposições de desfinanciamento, privatização, focalização e seletividade da política de Saúde. No que se refere à resistência, denomina-se de aparelhos contra-hegemônicos a organização e a mobilização da Frente Nacional contra a Privatização da Saúde, a qual é contrária ao processo de contrarreforma do Estado e às suas expressões na política de saúde.
