Based on institutional clinical work with intersex subjects, their parents, physicians, healthcare team, and family members, we investigated the hypothesis that a discursive intervention in the Real of social ties has a consequence in the Real of the subject. The thesis puts the epistemology of sexual difference as binary and heteronormative, and the epistemology of the normalization process of genitals considered abnormal under discussion. We review in Freud and Lacan the subjective assumption of sex, on the one hand, and we also precise the genital dimension, understood as typically male or female, to articulate these dimensions to the clinic of the biological variation of sex. In Freud, we go through his peremptory formulation - anatomy is destiny -, and his concept with which we believe we can put the issue of the imposed norm into dialogue: the drive. There is no drive outside the field of speech and language, which necessarily implies the dimension of the Real. To develop it, it was also necessary to revisit the three registers of the psychic reality, according to Lacan. The denaturation operated by language means that the difference between the sexes only has subjective consequences when signified. In Lacan, we identify the phallus as a historical and structural reference simultaneously, it being a structure reference of human desire and a parameter of difference. Then we arrive at the logic of sexuation, which implies significant clinical consequences since it does not stop at what constitutes a man or woman but at the impossibility of the relationship between them. The fact that there is more than one sex is not enough to count two. Through the articulation with the clinic with children and intersex adolescents, we show that the sexual anatomy – highly valued in some discourses – is not the Real of the body – approached by psychoanalysis. The subjective issues that we face in the daily life of our clinic developed in the Urology Sector of the Hospital Universitário Pedro Ernesto, allowed and led us to study various discourses, the medical and the social ones, the LGBTTQI+ movements, to, with them, question some a priori and introduce a new dialogue with the psychoanalytic discourse itself, concerning the clinic of intersex.
