The implementation of this model can contribute to the standardization and agility of notification processes, providing a more solid foundation for the analysis and prevention of incidents and adverse events in the outpatient context. The functional implementation of an incident and adverse event reporting system is essential to ensure patient safety, promote continuous improvement in the quality of health services, and strengthen the culture of safety and learning in healthcare institutions. These systems are essential tools for managing quality and patient safety, widely used to enhance the care provided and minimize risks. This work aimed to provide support for the development and implementation of an incident and adverse event notification system in outpatient specialty clinics. The focus was on creating an appropriate notification form, based on a comparative analysis of various models available online, from both public and private institutions in Brazil. The method used involved a comparative analysis of notification forms collected online, comparing them with the notification standards established by the World Health Organization (WHO) and the National Health Surveillance Agency (ANVISA). The comparison was made based on the notification models proposed by the WHO and ANVISA's NOTIVISA, organizing the items into similar blocks found in the forms of the institutions studied. The results identified three fundamental blocks that make up an incident notification system. The first block includes user identification elements; the second block provides a detailed analysis of data related to the context of the incident; and the third block covers the necessary components for analyzing the process in which the incident occurred. The adoption of this model can significantly contribute to the standardization and efficiency of notification processes, providing a solid foundation for the analysis and prevention of incidents and adverse events in the outpatient context.
