Resumo: This research aimed to evaluate the participation of civil society (CS) in the participatory processes of the national AIDS policy, from 2007 to 2019, combining a tested evaluation methodology with the theoretical-empirical proposition of an ‘evaluative practice friendly to the integrality' and with the analytical tracer 'woman', at CNAIDS. The variables selection procedures and agenda were used. The results of the first point to three participatory practices inside the integrality (PPII): election of civil society representatives in NGO meetings; election of the executive secretary by CNAIDS members, biannually; selection of CS representatives for Working Groups and events. The position of CNAIDS coordinator was established by Ordinance, considering it as a normative practice of a political nature. Choices by members of the technical-scientific community and the government followed the criterion of the existence of work in partnership with DHIVA, being considered selective collaborative practices. The late entry of women's representatives into CNAIDS stands out: the Special Secretariat for Policies for Women, in 2010, and the National Movement of Posithivas Citizens, in 2011. Little transparency was also identified about the composition of CNAIDS, with only two publications in Ordinances. In the agenda variable, the collective elaborations of agendas and annual calendars of meetings are pointed out as PPII. Themes about women were discussed six times – four times by the government, demonstrating its greater interest in the subject. It was observed that female condoms (FC) were not universally distributed, being excluded from campaigns until 2016, in order not to increase the demand for the input. From 2007 to 2019, the government provided 81 million FC, equivalent to 1.5% of the 5.3 billion male condoms (MC) offered. R$ 31 million were invested in a factory that only produced MC, even though there were only two suppliers of FC in Brazil. The accreditation of services for lipodystrophy was made more flexible, but from 2010 to 2011, only 11% of the R$ 4 million transferred to the states were used. As for the Integrated Plan to Combat the Feminization of Aids, launched in 2007, the lack of agreement in the Tripartite Intermanagers Commission made its capillarization in the states and municipalities unfeasible. There was little CS participation in the Action Plans and Goals, with low budget execution. Women with HIV had difficulty accessing the IUD, reproductive technologies and, in the interior of the country, prevention of vertical transmission (VT). Women, in general, had little access to PEP and restrictions on PrEP. There was a decline in CNAIDS meetings, from six in 2007 to two from 2014 onwards, demonstrating a reduction in dialogue between CS and government. CS's participation in the Commission produced little positive effect on the care of women, except on VT. It was inferred that the prioritization of actions for key populations weakened the principles of integrality and universality, making women a kind of pre-citizens, with their citizenship regulated by the concentrated epidemic, revealing that, in the 21st century, they still need to fight for right to have rights.

Utilizaram-se as variáveis procedimentos de seleção e agenda. Os resultados da primeira apontaram para três práticas participativas permeáveis à integralidade (PPPI): eleição de representantes da sociedade civil nos encontros de ONGs; eleição do secretário executivo por membros da CNAIDS, bianualmente; seleção de representantes da SC para Grupos de Trabalho e eventos. O cargo de coordenador da CNAIDS era estabelecido em Portaria, considerando-se como prática normativa de caráter político. Escolhas de membros da comunidade técnico-científica e do governo seguiam o critério de existência de trabalhos em parceria com o DHIVA, sendo consideradas práticas colaborativas seletivas. Destaca-se a entrada tardia de representantes de mulheres na CNAIDS: a Secretaria Especial de Políticas para Mulheres, em 2010, e o Movimento Nacional de Cidadãs Posithivas, em 2011. Também se identificou pouca transparência sobre a composição da CNAIDS, com apenas duas publicações em Portarias. Na variável agenda, apontam-se como PPPI as elaborações coletivas de pautas e calendários anuais de reuniões. Temas sobre as mulheres foram pautados seis vezes – quatro pelo governo, demonstrando seu maior interesse pelo assunto. Observou-se que os preservativos femininos (PF) não eram distribuídos universalmente, sendo excluídos de campanhas até 2016, para não aumentar a demanda pelo insumo. De 2007 a 2019, o governo forneceu 81 milhões de PF, o equivalente 1,5% dos 5,3 bilhões de preservativos masculinos (PM) ofertados. Foram investidos R$ 31 milhões em uma fábrica que só produziu PM, ainda que só houvesse dois fornecedores de PF no Brasil. Flexibilizou-se o credenciamento de serviços para lipodistrofia, mas, de 2010 a 2011, foram utilizados apenas 11% dos R$ 4 milhões repassados aos estados. Quanto ao Plano Integrado de Enfrentamento da Feminização da Aids, lançado em 2007, a falta de pactuação na Comissão Intergestores Tripartite inviabilizou sua capilarização nos estados e municípios. Houve pouca participação da SC nos Planos de Ações e Metas, com baixa execução orçamentária. Mulheres com HIV tiveram dificuldade de acesso ao DIU, às tecnologias de reprodução e, no interior do país, à prevenção da transmissão vertical (TV). Mulheres, em geral, tiveram pouco acesso à PEP e restrições à PrEP. Houve um declínio de reuniões da CNAIDS, de seis, em 2007, para duas a partir de 2014, demonstrando uma redução do diálogo entre governo e SC. A participação da SC na Comissão produziu pouco efeito positivo no cuidado às mulheres, salvo na TV. As for the Integrated Plan to Combat the Feminization of Aids, launched in 2007, the lack of agreement in the Tripartite Intermanagers Commission made its capillarization in the states and municipalities unfeasible. There was little CS participation in the Action Plans and Goals, with low budget execution. Women with HIV had difficulty accessing the IUD, reproductive technologies and, in the interior of the country, prevention of vertical transmission (VT). Women, in general, had little access to PEP and restrictions on PrEP. There was a decline in CNAIDS meetings, from six in 2007 to two from 2014 onwards, demonstrating a reduction in dialogue between CS and government. CS's participation in the Commission produced little positive effect on the care of women, except on VT. It was inferred that the prioritization of actions for key populations weakened the principles of integrality and universality, making women a kind of pre-citizens, with their citizenship regulated by the concentrated epidemic, revealing that, in the 21st century, they still need to fight for right to have rights.
