Resumo: The host’s protective mechanisms are mediated by the ability of immunonutrients in defense mechanisms, such as the inflammatory response. Among the most investigated are arginine and polyunsaturated fatty acids derived from fish oil, anti-inflammatory agents which act on the synthesis of eicosanoids and regulate the immune response. This study is randomized, double-blind, paired, whose aim was to identify whether there is pre-conditioning effect in nutraceutical supplement containing arginine and oil mix with ω3 fatty acids (EPA, DHA and ALA) on the inflammatory mediators in surgical trauma. The supplement was administered in two doses of 200ml/day during the preoperative period (5 days before surgery) in patients undergoing radical prostatectomy (PTR). Laboratory evaluations were performed preoperatively (PRE), before administration of the supplement (5 days before surgery), before the anesthetic induction (IND) and in the 2nd postoperative day (POS). The sample (26 patients) was divided into 3 groups: G1 - skim milk (0% fat), G2 – Arginine group: Supplement with ω6: ω3 ratio of 8:1 and Arginine (10g/day) and G3 – Arginine + Oil group: Supplement with high ω9:ω6 ratio, of 3,2:1, and low ω6:ω3 ratio (of 1,4:1) and Arginine (10g/day). Inflammatory (IL-1β, IL-6, IL-8, IL-10 and TNF-α) and preconditioning (HSP-27) parameters were analyzed. There were no significant differences between groups and times. Therefore, supplementation using arginine and oil mix, with high ω9: ω6 ratio and low ω6: ω3 ratio, has no effect as a nutraceutical pre-conditioner on the inflammatory mediators in surgical trauma patients undergoing radical prostatectomy (PTR).

Dentre os mais investigados estão a arginina e os ácidos graxos poliinsaturados derivados do óleo de peixe, agentes anti-inflamatórios que atuam na síntese de eicosanóides e regulam a resposta imune. O presente estudo é clínico randomizado, duplo-cego e emparelhado, cujo objetivo foi identificar se existe algum efeito decorrente do pré-condicionamento nutracêutico em um suplemento contendo arginina e mistura de óleos contendo ácidos graxos ω3 (EPA, ALA e DHA) sobre mediadores inflamatórios no trauma cirúrgico. O suplemento foi administrado em duas doses de 200ml/dia, durante o período pré-operatório (5 dias antes da cirurgia), em pacientes a serem submetidos à prostatectomia radical (PTR). As avaliações laboratoriais foram realizadas no pré-cirúrgico (PRE), antes da administração do suplemento (5 dias antes da cirurgia), antes da indução anestésica (IND) e no 2º pós-operatório (POS). A amostra (26 pacientes) foi distribuída em 3 grupos: G1 - leite desnatado (0% de gordura), G2 - suplemento com relação ω6:ω3 de 8:1 e Arginina (10g) e G3 - suplemento com elevada relação ω9:ω6:, de 3,2:1 e baixa relação ω6:ω3, de 1,4:1 e Arginina (10g). Foram analisados parâmentros inflamatórios (IL-1β, IL-6, IL-8, IL-10 e TNF-α) e de Pré-condicionamento (HSP-27). Não foi evidenciada diferença estatisticamente significante entre os grupos e momentos.
