O Programa de Saúde da Família no Ceará : uma análise de sua estrutura e funcionamento

Resumo: The objective of this study was to contribute for the improvement of the Family Health Program ( FHP) developed in the State of Ceará, northeast Brazil, in terms of program functioning and impact. Based on the results, recomendations on interventions aimed to the main problems found were prepared. Self-administered questionnaires were answered by 82 (78%) secretaries of health of the municipalities where the program is being carried out, 230 (67,4%) physicians, 278 (81,5%) nurses and 2.507 (73,5%) community health agents working in the program. The FHP teams (composed mainly by physicians, nurses and health agents) also answered together, an specific questionnaire, with a response of 149 (43,6%). Many positive aspects of the program were found such as : many health activities being developed in some villages where, in the past, there was no regular assistance, including activities to reach groups like youth and elders; more frequently follow up of patients with chronic diseases like diabetes, and hypertension; almost all physicians and nurses working full time in the program; many health agents reporting that they were receiving technical support and refreshing courses from the other members of the team. Also a better organized referral system for patients in need of a more complex level of health care was found. According to secretaries, physicians and nurses there was a significant improvement of the quality of the assistance, that now is supposed to be more comprehensive. The problems found were: The majority of doctors and nurses had no labor rights guaranteed; an important number of health professionals were working in the program suitable qualification; a great diversity of salaries leading to a considerable turnover of doctors; lack of physicians willing to work in the poorest municipalities; an excessive number of families to follow by most of the teams; poor infrastructure of many local health facilities; teams providing assistance in many communities; insufficient provision of medicines; few activities of health control, just few intersetorial initiatives and lack of community participation. The conclusions of the study indicate that it is necessary a greater discussion among the federal, state and municipal health managers, including the effective participation of the local professionals, about the adjustments that need to be done in FHP. This discussion must happen before the important increase of number of health teams planned for the coming years. This study is expected to be also useful for other brazilian states that are developing, or planning to develop the program, as well as to the national coordination of FHP ,hoping that such strategy bring solutions for many health problems faced by the public health sector in Brazil.

O estudo foi baseado em questionários sobre vários aspectos do programa respondidos por 82 (78%) secretários de saúde dos municípios onde o mesmo está implantado; 230 (67,4%) médicos; 278 ( 81,5%) enfermeiros e 2.507 (73,5%) agentes de saúde envolvidos no PSF. Além disso, 149 (43,6%) equipes responderam, conjuntamente, outro tipo de questionário. Os resultados encontrados evidenciaram pontos bastante positivos do programa, tais como: a realização de diversas ações de saúde em localidades anteriormente não assistidas com uma atenção regular, inclusive contemplando atividades voltadas para grupos muitas vezes esquecidos pelos sistemas de saúde, como adolescentes e idosos; um acompanhamento mais freqüente de pacientes com patologias crônicas, como diabéticos e hipertensos; praticamente todos os médicos e enfermeiros dedicando tempo integral ao programa; muitos agentes de saúde sentindo-se bem apoiados e recebendo treinamento por intermédio de outros membros das respectivas equipes; médicos e enfermeiros acompanhando os agentes durante as visitas domiciliares, analisando dados produzidos pelos mesmos e encaminhando providencias para os problemas encontrados. Verificou-se também um encaminhamento mais organizado dos pacientes que necessitam de atendimento nos níveis de maior complexidade de assistência. Além disso, pelo relato dos diversos respondentes, parece ter havido uma melhoria significativa na qualidade da atenção, passando a ser mais contínua, integral e humanizada. Também foram encontrados problemas, dentre os quais destacamos: a maioria dos médicos e enfermeiros sem a garantia dos direitos trabalhistas; um número significativo de profissionais sem a qualificação adequada para a atuação no programa; uma grande variação salarial entre os municípios e falta do profissional médico em algumas equipes, decorrente da dificuldade de fixação do mesmo em municípios mais carentes; número excessivo de famílias sob a responsabilidade de algumas equipes; más instalações físicas das unidades de saúde e atendimento sendo prestado em várias localidades, quase sempre em situação precária; quantidade de medicamentos insatisfatória; poucas ações de vigilância sanitária e somente uma incipiente tentativa de ação intersetorial. Constatou-se, igualmente, que muitas ações básicas de saúde, como o exame ginecológico e pequenas cirurgias, infelizmente não estão sendo ainda realizadas em todas as comunidades atendidas pelo programa. Percebe-se, também, que não há uma grande ênfase nas ações de promoção da saúde e nem tampouco de participação da comunidade no planejamento e acompanhamento das atividades realizadas. As conclusões do estudo indicam a necessidade de uma ampla discussão, entre os gestores dos níveis federal, estadual e municipal de saúde, com a participação efetiva dos profissionais do PSF sobre os ajustes que são necessários e redefinições de vários aspectos relacionados ao programa. Essa discussão deveria ser realizada antes da ampliação do número de equipes, prevista para os próximos anos.
