Autorrelato de higiene oral e sinais de doença periodontal para o diagnóstico de periodontite severa

Resumo: The objective of this study was to evaluate the performance of self-report questions related to oral hygiene and to the periodontal condition to estimate the occurrence of severe periodontitis. Records of individuals diagnosed with severe periodontitis or gingivitis were included in the study. The periodontal condition was determined by the periodontal screening recording exam (PSR). Severe periodontitis were patients identified with PSR code 4 and healthy/gingivitis codes 0, 1 or 2. Self-report of conditions related to signs and symptoms of periodontal diseases, gingival bleeding (OS) and dental mobility (MOB) frequency of brushing (EF), interdental cleaning (ID) and oral antiseptic use (AO) were evaluated for the ability to predict the diagnosis of severe periodontitis when compared to patients with gingivitis. A total of 267 patients' charts were included, 80 of them with severe periodontitis (29.9%). Results: This group had a statistically significant (p <0.05) mean age (40 ± 12.1 x 34.2 ± 12.7 years), a higher proportion of men (47.5% vs. 29,9%) and lower AO (27.8% x 45.1%). On the other hand, a higher proportion of SG (61% x 32.6%) and MOB (17% x 4.3%). No differences were found for MOB, FE and ID. Logistic regression including the statistically significant variables besides age and sex generated values of SS = 31.1%, SP = 95.3% and receiver operating characteristic (ROC) = 75.4%. Self-report measures related to periodontal condition present moderate predictability to identify patients who do not present severe periodontitis.

O objetivo deste estudo foi avaliar o desempenho de questões de autorrelato relacionadas à higiene oral e à condição periodontal para estimar a ocorrência de periodontite severa. Prontuários de indivíduos diagnosticados com periodontite severa ou gengivite foram incluídos no estudo. A condição periodontal foi determinada pelo exame periodontal simplificado (PSR). Periodontite severa, pela presença de código 4; Pacientes saudáveis, código 0 e gengivite, pela presença dos códigos 1 ou 2. O autorrelato de condições relacionadas aos sinais de doenças periodontais, sangramento gengival (SG) e mobilidade dentária (MOB) e de medidas de higiene oral, frequência de escovação (FE), limpeza interdental (ID) e uso de antissépticos orais (AO) foram avaliadas quanto à capacidade de predizer o diagnóstico de periodontite severa quando comparados a pacientes com gengivite. Foram incluídos 267 prontuários de pacientes, sendo 80 de pacientes com periodontite severa (SEV) e 187 saudáveis ou com gengivite (SAUD/GENG). Resultados: Quando comparados os dois grupos, os pacientes com periodontite severa apresentaram, de forma estatisticamente significante (p<0,05), maior média de idade (40±12,1 x 34,2±12,7 anos) assim como maior proporção de homens (47,5% x 29,9%). Apresentaram menor AO (27,8% x 45,1%), mas por outro lado, maior proporção de SG (61% x 32,6%) e MOB (17% x 4,3%). Não foram encontradas diferenças para FE e ID. Dentre os pacientes do grupo SEV 61,3% apresentaram pelo menos um sextante com código 4 do PSR. Dentre os do grupo SAUD/GENG 38,5% apresentaram pelo menos um sextante com código 2. Regressão logística, incluindo as variáveis estatisticamente significantes além de idade e sexo, gerou valores de SS = 31,1%, SP = 95,3% e receiver operating characteristic (ROC) = 75.4%.
