Resumo: Aids is a stigmatizing chronic disease with no cure. Despite there is no effective vaccine, progresses in antiretroviral therapy have improved life quality of people with HIV, decreasing morbidity and mortality. The high variability of HIV, triggers virological failures that jeopardize all medical treatment action, and the fast emergence of mutation enables the resistance to antiviral, and favors the development of opportunistic infections. The aim of this study was to characterize patients with HIV-1 who are under treatment in a reference hospital of infectious diseases who`s HIV has showed or not resistance to antiretroviral drugs in the term 2011 to 2013 in Ceará. This is a transversal and retrospective study, including 250 resistant patients, and 67 non-resistant patients treated in HIV/AIDS clinic between August 2014 and December 2015. Data was compiled from survey genotyping, medical records and interview, with statistical descriptive data analysis, with Shapiro – Wilk test, Student, Fisher and Chi-square with value p< 0.05. The population with virus resistant to antiretroviral drugs showed a predominance of males (62%), mean age 43.6 years, single (70%), low level of education (54%) and income (68.6%), resident in Fortaleza (75.2%), self-employed (48%) and Catholic (89.2%). In the clinical aspect, it was found the time of serology and use of HAART in the range of 12 to 17 years (42 %), with medication changes from 1 to 3 times and therapeutic failures due to lack of adhesion (33.2%), with opportunistic infection due to tuberculosis (18.8%) and those ones with prolonged use of HAART had dyslipidemia (33.2%). At the time of genotyping survey, most of them was symptomatic with several hospitalization. In the immunology aspect, low CD4+ TL (84.4%), and high viral load (76%). The significant mutations that happened for theNucleoside reverse transcriptase inhibitors (NRTIs)were 184V, for the Non-nucleoside reverse transcriptase inhibitors (NNRTIs) were 103N and for the Protease inhibitors (PIs) were 63P and 36I. The drugs that lost activity were Lamivudine, Efavirenz e Indinavir associated with Ritonavir. In the population with non-resistant virus, the prevalent gender was female, aged between 18 and 28 years with higher levels of education and income, and HAART use time between 0 and 5 years, and high level of CD4+TL with low viral load. It was found that people without the virus resistant to antiretroviral drugs has improved quality of life compared to the population with resistant virus.

Apesar de não existir uma vacina eficaz, os avanços na terapia dos antirretrovirais melhoraram a qualidade de vida do indivíduo com HIV, reduzindo a morbidade e a mortalidade. A alta variabilidade do vírus HIV deflagra falhas virológicas que comprometem toda a ação da medicação e o surgimento rápido de mutação, que possibilita a resistência aos antivirais e favorece o surgimentode infecções oportunistas. O objetivo desse estudo foi caracterizar os pacientes portadores de HIV-1 que fazem tratamento em hospital de referência em doenças infectocontagiosas, que apresentaram ou não resistências aos antirretrovirais, no período de 2011 a 2013, no Ceará. Trata-se de um estudo transversal e retrospectivo, com a inclusão de 250 pacientes com vírus resistente aos antirretrovirais e 67 pacientes com vírus não resistente aos antirretrovirais, atendidos no ambulatório de HIV/AIDS, entre agosto de 2014 e dezembro de 2015. Os dados foram compilados a partir do exame de genotipagem, prontuário médico e entrevista, com análise estatística descritiva dos dados e análise inferencial, com o teste Shapiro – Wilk, Student, Fisher e Qui – quadrado,sendo considerados os resultados diferentes estatisticamente significantes quando o valor de p< 0,05. A população com vírus resistente aos antirretrovirais apresentou predominância no sexo masculino (62%), com média de idade 43,6 anos, solteiro (70%), baixo nível de escolaridade (54%) e de renda (68,6%), residente em Fortaleza (75,2%), autônomo (48%) e católico (89,2%). No aspecto clínico, encontrou-se o tempo de sorologia e uso da TARV no intervalo de 12 a 17 anos (42,8%), com trocas da medicaçãode 1 a 3 vezes e falhas terapêuticas em decorrência da adesão (33,2%), com infecção oportunista por tuberculose (18,8%) e aqueles com uso prolongado da TARV apresentaram dislipidemia (33,2%). No momento da realização do exame de genotipagem a maioria era sintomático com várias intercorrências. No aspecto imunológico, baixo LT CD4+ (84,4%) e carga viral elevada (76%). As mutações significantes que ocorreram para os ITRN foram 184V, para os ITRNN foi 103N e na IP foram 63P e 36I. As medicações que perderam atividadeforam a Lamivudina, Efavirenz e Indinavir associado a Ritonavir. Na população sem o vírus resistente aos antirretrovirais, o sexo prevalente foi o feminino, a faixa etária entre 18 – 28 anos com maiores níveis de escolaridade e renda, o tempo de uso da TARV entre 0 e 5 anos, e o nível de LT CD4+ elevado com baixa carga viral.
