Resumo: Even though it is a temporary and exclusiondiagnosis, the Atypical Squamous Cells (ASC) cytologies became the most frequent abnormality in smears, and both their characterization and the most appropriate management remain controversial. The aim of this study was to verify the correlation of ASC cytologies with the histopathological result and detection of HPV (human papillomavirus). For this purpose, a retrospective study was carried out analyzing cytology data in a cytopathology laboratory database of the city of Fortaleza between 2010 and 2016, comparing those data with the morphological findings of cytology, microbiology, high-risk HPV detection, histopathological findings, and evolution.Patients who were immunocompromised or pregnant were excluded.A total of 2457 patient was included, 2207 for ASC-undetermined significance (ASC-US) and 250 for ASC-cannot exclude high-grade squamous intraepithelial lesion (ASC-H). The results showed a higher association between ASC-H and a worst histological resultin most biopsies (56%) and a higher DNA-HPV test positivity than in the ASC-US group.From the cytomorphological findings that led to the diagnosis of ASC-US, the relation of nucleus size was highlighted when compared to a normal cell (78%). Hyperkeratosis correlates with worse histological prognosis (p=0,0439), unlike binucleation(p=0,0047). There was no statistical significance between a positive DNA-HPV test and the histopathological prognosis(p=0,0743).Other high-risk HPV were more prevalent than HPV 16 and HPV 18 in both groups (ASC-US 86%/ASC-H 100%) when real-time PCR was performed.

O objetivo deste estudo é verificar a correlação de citologias ASC com o resultado histopatológico e detecção de HPV. Para isso, realizou-se um estudo retrospectivo analisando dados de citologias em banco de dados de laboratório de citopatologia da cidade de Fortaleza, entre 2010 e 2016, comparando-os aos achados morfológicosda citologia, microbiologia, detecção do HPV de alto risco, histopatológicos e evolução. Gestantes e mulheres com imunoincompetência foram excluidas. A amostra obtida foi de 2457 pacientes, sendo 2207 casos de ASC de significado indeterminado (ASC-US) e 250 casos de ASC, não podendo descartar lesão de alto grau (ASC-H). Os resultados mostraram uma maior associação ASC-H com lesão de alto grau histológica (56%) e com uma maior positividade para um teste de HPV de alto risco em relação ao grupo ASC-US. Dos achados citomorfológicos que levaram ao diagnóstico de ASC-US se destacou a relação de tamanho do núcleo, quando comparada a uma célula normal (78%). Hiperqueratose, se correlaciona com um pior prognóstico histológico (p= 0,0439), ao contrário da binucleação (p=0,0047).
