Resumo: This dissertation presents a study related to the reduction of industrial accidents, through the identification and treatment of unsafe acts and unsafe conditions in a petrochemical company. Through a case study, this work shows the importance of acting in the personal factor, with a view to the management of the performance of SHE becomes is effective. How, for example of this scenario, we can mention the ability to identify, analyze and minimize exposure to risks in order to ensure the productivity without prejudice to the health and physical integrity of persons. To achieve these SHE standards, it s required some important components as the behavior and attitude of employees at all levels and areas of expertise, and the closeness of the technical and operational knowledge and the human concepts, involving them in the daily lives of the organizations. The causes of work accidents in general are caused by human error; it shows that about 95% due to unsafe acts (OR-OSHA, 2006). Therefore, this work includes the analysis of some deviations from the field s activities, from a unit of a large petrochemical company. Also throughout this study, it is possible to show that the identification and treatment of unsafe acts and unsafe conditions are of paramount importance to the management on SHE, in addition to work on preventive of anomalies, operates in preserving worker s lives, which is the main goal.

Através de um estudo de caso, o presente trabalho mostra a importância da atuação no fator pessoal para que a gestão do desempenho de SMS seja eficaz. Como exemplo desta atuação cita-se a capacidade em identificar, analisar e minimizar a exposição aos riscos, a fim de garantir a produtividade sem prejuízo da saúde e da integridade física das pessoas. Para atingir estes padrões em SMS, são necessários componentes importantes como o comportamento e a atitude dos empregados em todos os níveis e áreas de atuação, bem como a aproximação do conhecimento técnico-operacional e do humano, associando-os no cotidiano das organizações. Conforme a literatura, os acidentes de trabalho em geral são causados por falhas humanas, dos quais cerca de 95% ocorrem devido a atos inseguros e condições inseguras (OR-OSHA, 2006). Portanto, este trabalho compreende uma análise de desvios identificados nas atividades de campo de uma Unidade referente a uma grande empresa petroquímica.
