Resumo: Introduction: Creatine Monohydrate supplementation has been extensively investigated in the scientific literature. The intake of this compound is justified by increases in intramuscular creatine and creatine phosphate (CP) concentrations, which increases ATP resynthesis, mainly during short term-high and high intensity exercises (<30 seconds). This, once repeated in resistance training exercises, would allow for the accumulation of muscle contractile proteins in the exercised muscle (muscle hypertrophy). According to previous studies by our group, BFR training proved to be superior in increasing muscle mass in the short term (<6 weeks), when compared to traditional hypertrophy training (TRAD). However, TRAD training proved to be superior in inducing gains in maximum muscle strength. Considering that creatine supplementation induces important gains in muscle performance, it is possible that such increased performance may also manifest in the BFR method, thus resulting in gains of muscle mass. Objective: To investigate the effects of 8-week creatine monohydrate or placebo supplementation on muscle strength and muscle thickness, employing different training paradigms (BFR vs TRAD). Materials and Methods: 18 healthy male subjects, between 18 and 30 years old, were randomized between the placebo (n = 9) or creatine (n = 9) group. The volunteers of both groups were trained unilaterally in the bicep curl exercise (each arm was allocated to a different training model, TRAD or BFR) for 8 weeks. In the present study, resistance training was performed employing the unilateral resistance training method for both paradigms, TRAD and BFR. Muscle strength was evaluated as follows: 1) Muscle dynamic strength was measured utilizing repetitions to failure, employing different 1 RM percentage (30%, 70% of 1RM); 2) Maximal dynamic strength was evaluated via 1 RM test; 3) Volume of training in each exercise session was calculated using the formula: number of sets x number of repetitions x load, in kilograms. Muscle hypertrophy was determined via muscle thickness measurements using ultrasound equipment and body composition was measured through skinfold caliper measurements. All measures of muscle thickness, circumference, strength, and body composition were evaluated at the moment 0, 2, 4, 6 and 8 weeks. Results: There were additional gains in the muscle thickness variable for both TRAD CR and BFR CR methods (17.85% and 14.35% respectively) when compared to the TRAD PL and BFR PL (9.53% vs 9, 08% respectively) (p = 0.011). Maximum strength (1RM) was increased in the TRAD CR group when compared to the BFR CR (35.89% vs 11.67% respectively) (p = 0.027). Repetition to failure test at 30% of 1RM was increased in the BFR CR group, when compared to TRAD CR (153.15% vs 90.04% respectively) (p = 0.025). Repetition to failure test at 70% 1RM was greater in BFR-CR than in placebo BFR (181.10% vs 79.19% respectively) (p = 0.007). There was an increase in total body weight in the group supplemented with creatine, when compared to the placebo group (2kg vs 0.01kg) (p = 0.002). Conclusion: Our data show that creatine supplementation favored increases in muscle mass in both groups (TRAD and BFR). Creatine was more effective at maximum strength for the TRAD group and repetitions to failure at 30% of 1RM for the BFR group, when compared to the placebo group. As already demonstrated in the literature, creatine supplementation was able to exert anabolic effects in the TRAD training modality. However, this is the first study to demonstrate that creatine supplementation also exerts anabolic and ergogenic effects on BFR training. The observed improvements in muscle performance and muscle hypertrophy, may have important implications not only in our study but also in other scenarios, such as rehabilitation programs or athletic programs, where BFR is also utilized.

A ingestão desse composto é justificada pelo aumento das concentrações intramusculares de creatina e creatina fosfato (CP), que aumentam a ressíntese de ATP, principalmente durante exercícios de alta intensidade e curta duração (<30 segundos). Isso, uma vez repetido em exercícios de treinamento de força, permitiria o acúmulo de proteínas contráteis musculares no músculo exercitado (hipertrofia muscular). De acordo com estudos anteriores do nosso grupo, o treinamento BFR se mostrou superior no aumento da massa muscular em curto prazo (<6 semanas), quando comparado ao treinamento tradicional de hipertrofia (TRAD). No entanto, o treinamento TRAD provou ser superior em induzir ganhos na força muscular máxima. Considerando que a suplementação de creatina induz ganhos importantes no desempenho muscular, é possível que tal aumento no desempenho também possa se manifestar no método BFR, resultando em ganhos de massa muscular. Objetivo: Investigar os efeitos da suplementação de creatina monohidratada ou placebo por 8 semanas na força muscular e na espessura muscular, empregando diferentes paradigmas de treinamento (BFR vs TRAD). Materiais e Métodos: 18 indivíduos saudáveis do sexo masculino, entre 18 e 30 anos, foram randomizados entre o grupo placebo (n = 9) ou creatina (n = 9). Os voluntários de ambos os grupos foram treinados unilateralmente no exercício rosca bíceps (cada braço foi alocado para um modelo de treinamento diferente, TRAD ou BFR) por 8 semanas. No presente estudo, o treinamento de força foi realizado empregando o método de treinamento de força unilateral para ambos os paradigmas, TRAD e BFR. A força muscular foi avaliada da seguinte forma: 1) A força dinâmica muscular foi medida utilizando repetições até a falha, empregando diferentes percentuais de 1 RM (30%, 70% de 1RM); 2) A força dinâmica máxima foi avaliada por meio do teste de 1 RM; 3) O volume de treinamento em cada sessão de exercício foi calculado pela fórmula: número de séries x número de repetições x carga, em quilogramas. A hipertrofia muscular foi determinada por meio de medidas da espessura muscular com equipamento de ultrassom e a composição corporal foi medida por meio de medidas do compasso de dobras cutâneas. Todas as medidas de espessura muscular, circunferência, força e composição corporal foram avaliadas nos momentos 0, 2, 4, 6 e 8 semanas. Resultados: Houve ganhos adicionais na variável de espessura muscular para ambos os métodos TRAD CR e BFR CR (17,85% e 14,35% respectivamente) quando comparados ao TRAD PL e BFR PL (9,53% vs 9,8% respectivamente) (p = 0,011) A força máxima (1RM) foi aumentada no grupo TRAD CR quando comparado ao BFR CR (35,89% vs 11,67% respectivamente) (p = 0,027). A repetição do teste de falha a 30% de 1RM foi aumentada no grupo BFR CR, quando comparado com TRAD CR (153,15% vs 90,04% respectivamente) (p = 0,025). A repetição do teste de falha a 70% 1RM foi maior no BFR-CR do que no BFR do placebo (181,10% vs 79,19% respectivamente) (p = 0,007). Houve aumento do peso corporal total no grupo suplementado com creatina, quando comparado ao grupo placebo (2kg vs 0,01kg) (p = 0,002). Conclusão: Nossos dados mostram que a suplementação de creatina favoreceu o aumento da massa muscular em ambos os grupos (TRAD e BFR). A creatina foi mais eficaz na força máxima para o grupo TRAD e nas repetições até a falha a 30% de 1RM para o grupo BFR, quando comparada ao grupo placebo. Conforme já demonstrado na literatura, a suplementação de creatina foi capaz de exercer efeitos anabólicos na modalidade de treinamento TRAD. No entanto, este é o primeiro estudo a demonstrar que a suplementação de creatina também exerce efeitos anabólicos e ergogênicos no treinamento de BFR. description Introduction: Creatine Monohydrate supplementation has been extensively investigated in the scientific literature. The intake of this compound is justified by increases in intramuscular creatine and creatine phosphate (CP) concentrations, which increases ATP resynthesis, mainly during short term-high and high intensity exercises (<30 seconds). This, once repeated in resistance training exercises, would allow for the accumulation of muscle contractile proteins in the exercised muscle (muscle hypertrophy). According to previous studies by our group, BFR training proved to be superior in increasing muscle mass in the short term (<6 weeks), when compared to traditional hypertrophy training (TRAD). However, TRAD training proved to be superior in inducing gains in maximum muscle strength. Considering that creatine supplementation induces important gains in muscle performance, it is possible that such increased performance may also manifest in the BFR method, thus resulting in gains of muscle mass. Objective: To investigate the effects of 8-week creatine monohydrate or placebo supplementation on muscle strength and muscle thickness, employing different training paradigms (BFR vs TRAD). Materials and Methods: 18 healthy male subjects, between 18 and 30 years old, were randomized between the placebo (n = 9) or creatine (n = 9) group. The volunteers of both groups were trained unilaterally in the bicep curl exercise (each arm was allocated to a different training model, TRAD or BFR) for 8 weeks. In the present study, resistance training was performed employing the unilateral resistance training method for both paradigms, TRAD and BFR. Muscle strength was evaluated as follows: 1) Muscle dynamic strength was measured utilizing repetitions to failure, employing different 1 RM percentage (30%, 70% of 1RM); 2) Maximal dynamic strength was evaluated via 1 RM test; 3) Volume of training in each exercise session was calculated using the formula: number of sets x number of repetitions x load, in kilograms. Muscle hypertrophy was determined via muscle thickness measurements using ultrasound equipment and body composition was measured through skinfold caliper measurements. All measures of muscle thickness, circumference, strength, and body composition were evaluated at the moment 0, 2, 4, 6 and 8 weeks. Results: There were additional gains in the muscle thickness variable for both TRAD CR and BFR CR methods (17.85% and 14.35% respectively) when compared to the TRAD PL and BFR PL (9.53% vs 9, 08% respectively) (p = 0.011). Maximum strength (1RM) was increased in the TRAD CR group when compared to the BFR CR (35.89% vs 11.67% respectively) (p = 0.027). Repetition to failure test at 30% of 1RM was increased in the BFR CR group, when compared to TRAD CR (153.15% vs 90.04% respectively) (p = 0.025). Repetition to failure test at 70% 1RM was greater in BFR-CR than in placebo BFR (181.10% vs 79.19% respectively) (p = 0.007). There was an increase in total body weight in the group supplemented with creatine, when compared to the placebo group (2kg vs 0.01kg) (p = 0.002). Conclusion: Our data show that creatine supplementation favored increases in muscle mass in both groups (TRAD and BFR). Creatine was more effective at maximum strength for the TRAD group and repetitions to failure at 30% of 1RM for the BFR group, when compared to the placebo group. As already demonstrated in the literature, creatine supplementation was able to exert anabolic effects in the TRAD training modality. However, this is the first study to demonstrate that creatine supplementation also exerts anabolic and ergogenic effects on BFR training. The observed improvements in muscle performance and muscle hypertrophy, may have important implications not only in our study but also in other scenarios, such as rehabilitation programs or athletic programs, where BFR is also utilized.
