Resumo: This study analyzed the socio-demographic factors that influence SUS users not to seek oral health services in Primary Health Care. Database of Brazilian Ministry of Health’s National Program for Improving Access and Quality in Basic Care (PMAQ - AB) 3rd Cycle (2017-2018) was analyzed. Data collection instruments were answered by users of primary health care units (PHCU) in 5.324 cities across the country. In each health unit four adult users were interviewed by trained researchers. Among 144,440 interviewed users, 117,570 were in PHCU which included family health strategy teams and family oral health teams (self/eSB). Study variables were obtained from PMAQAB module III - user interview: user Identification and access to care. It included independents variables on user’s sociodemographic profile (gender, age group, marital status, self-declared skin color, schooling, per capita income, and being beneficiary of basic income public programmer (PBF). Dependent variables were lack of demand or access to POHCS. Bivariate analysis, gross, and multiple logistic regression using SATA version 15.0 were performed to identify association between variables. Among the 117.570 participants, 48.82% (n = 57.403) reported being able and 4.01% (n = 4718) reported not being able to make a dental appointment. The remaining 47.16% (n = 55.449) had never sought POHCS in the PHCU. The highest chance of not seeking POHCS was observed among widowed individuals, the elderly, those with higher income and those with lower schooling. Users with lower chances of not seeking POHCS, were self-reported black, brown, yellow, and of indigenous skin color, presented higher schooling and were beneficiaries of PBF. Lack of demand for POHCS was associated with user’s socio-demographic characteristics. Dental health teams should target inequalities in services access, develop strategies to actively search missing users, and enhance POHCS access.

Foi analisado o banco de dados secundários do Programa Nacional para Melhoria do Acesso e Qualidade na Atenção Básica (PMAQ-AB) 3º Ciclo (2017-2018). O instrumento de avaliação externa foi aplicado nas Unidades Básicas de Saúde de 5.324 municípios, onde foram entrevistados quatro usuários adultos, por equipe de saúde, presentes nas Unidades Básicas de Saúde no dia da Avaliação Externa do PMAQ-AB - 3º Ciclo. Dos 144.440 usuários entrevistados, 117.570 estavam presentes em UBS com equipes de saúde da família e equipes de saúde bucal (eSF/eSB) e constituíram a amostra deste estudo. As variáveis independentes selecionadas para o estudo foram obtidas do Módulo III – Entrevista com Usuário, nos questionários: Identificação do Usuário (MIII.3) e Acesso ao Atendimento (MIII.5) e representaram o perfil sociodemográfico dos usuários (sexo, faixa etária, estado civil, cor de pele autodeclarada, escolaridade, renda per capita, participação no Programa Bolsa Família). A variável dependente foi a não procura por serviços de saúde bucal na Atenção Primária à Saúde, analisada a partir da pergunta “o Sr. consegue marcar consulta com o dentista? ”, presente no Instrumento de Avaliação Externa do PMAQ-AB 3º Ciclo, questionário - Saúde Bucal (MIII.18), que avaliou a satisfação do usuário quanto ao acesso aos serviços e qualidade da assistência. Foram realizadas análises bivariada e de regressão logística bruta e múltipla para determinar a associação entre as variáveis utilizando-se o programa estatístico STATA versão 15.0. Dos 117.570 usuários participantes do estudo, 48,82% (n = 57.403) relataram conseguir marcar a consulta com o dentista na Unidade Básica de Saúde e 4,01% (n = 4718), relataram não conseguir. Os 47,16% (n = 55.449) restantes relataram nunca ter procurado atendimento com o dentista naquela UBS. A maior chance de não procurar os serviços de saúde bucal na Atenção Primária foi observada entre os indivíduos viúvos, idosos, com maior renda e aqueles de menor escolaridade. Enquanto os usuários com menores chances de não procurar os serviços de saúde bucal na APS, se autodeclararam de cor de pele preta, parda, amarelos e indígenas, quando comparados aos brancos, os usuários de maior escolaridade e beneficiários do Programa Bolsa Família (PBF), quando comparados aos não beneficiários. A não procura por serviços de saúde bucal na APS foi associada aos fatores sociodemográficos da população usuária do Sistema Único de Saúde (SUS).
