Análise metavirômica de pacientes com doença febril aguda e suspeita de arbovirose em Mato Grosso, 2018-2019

Resumo: | Arboviruses (arthropod-borne viruses) correspond to a heterogeneous group of viruses transmitted to vertebrates by arthropods (mainly mosquitoes and ticks), which occur mainly in tropical and subtropical areas of the world. Of these, the most important for Brazilian public health are dengue, yellow fever, zika and chikungunya viruses. Previous studies revealed that an important portion of the patients with acute febrile illness suspected of arboviral disease in Mato Grosso (MT) negative for a panel of 16 arboviruses belonging to the genera Alphavirus, Flavivirus and Orthobunyavirus, which represent more than 95% of the casuistry of arboviruses in Brazil. This study aimed to detect viruses present in samples from patients with acute febrile illness suspected of arbovirus infection by viral metagenomics in the years 2018 and 2019. For this, 65 serum samples were allocated in seven pools during the year 2018. The analysis revealed the presence of chikungunya virus (CHIKV) genotype East-Central-South African (ECSA) in all pools, human parvovirus B19 (B19V) genotype I in six pools, pegivirus C genotypes 1 and 2 in two pools, and three anelloviruses, being two Alphatorquevirus in two pools and one Betatorquevirus in one pool. Among the 92 serum samples allocated in 10 pools concerning the year 2019, dengue virus serotype 2, Asian American genotype (genotype III) was found in all pools, pegivirus C genotype 2 in four pools, B19V genotype 1 in one pool, Coxsackievirus A in one pool and 13 anelloviruses in five pools. In both studies, nonsynonymous mutations were detected in CHIKV, B19V and DENV-2, the agents responsible for acute febrile illness detected in MT in the study years. This study revealed the importance of genomic surveillance for the identification of viral agents in the population, as well as the profile and viral diversity found in patients with suspected acute febrile illness caused by arboviruses in subsequent years. |

Destes, os mais importantes para a saúde pública brasileira são o vírus dengue (DENV), febre amarela, zika (ZIKV) e chikungunya (CHIKV). Estudos anteriores revelaram que uma parcela importante dos pacientes com doença febril aguda suspeita de arboviroses em Mato Grosso (MT) negativas para um painel de 16 arbovírus pertencentes aos gêneros Alphavirus, Flavivirus e Orthobunyavirus, aos quais se atribui mais de 95% da casuística de arboviroses no Brasil. Este estudo objetivou detectar vírus presentes em amostras de pacientes com doença febril aguda suspeita de arbovirose por metagenômica viral, em 2018 e 2019. Para isso, foram alocadas 65 amostras de soro em sete pools durante o ano de 2018. A análise revelou a presença do CHIKV genótipo East-CentralSouth African (ECSA) em todos os pools, parvovirus humano B19 (B19V) genótipo I em seis pools, pegivirus C genótipos 1 e 2 em dois pools, e três anellovirus, sendo dois Alphatorquevirus em dois pools e um Betatorquevirus em um pool. Dentre as 92 amostras de soro alocadas em 10 pools referentes ao ano de 2019, foram encontrados o vírus da Dengue sorotipo 2, genótipo Asiático-Americano (genótipo III) em todos os pools, pegivirus C, genótipo 2 em quatro pools, B19V genótipo 1 em um pool, Alphatorquevirus em três pools, Coxsackievirus A em um pool e 13 anellovirus em cinco pools.
