"De peito aberto" : vulnerabilidade de consumidoras dos serviços de saúde relativos ao câncer de mama pelo Sistema Único de Saúde

Resumo: Breast cancer, or malignant neoplasm, has been assuming an increasingly worrying position among the diseases that affect the female population, portraying in Brazil and in the world as a significant cause of death, being the second most frequent type of cancer in the world and the first among women. In this context, as consumers of the health services of the Serviço Único de Saúde (SUS), women with breast cancer may experience vulnerable consumption, as this type of disease is configured as a trigger event in which they are unable to cope. to advocate for their best interest, especially as it is an exchange relationship based on highly complex medical knowledge. In addition, the consumption of SUS services, in general, is performed by patients who are unable to access other providers, according to their preference, which subjects them to a scenario of restricted choice. Therefore, the objective of this research is to analyze the experiences of the vulnerability of consumers of health services related to breast cancer by the Serviço Único de Saúde. Therefore, we carried out qualitative research, through a narrative interview, based on a semi-structured script applied to 12 women affected by breast cancer and consumers of SUS health services. As the main results, we found that women experience vulnerabilities even before being diagnosed with breast cancer, especially during the consumption of health center services, before being referred to cancer referral hospitals. In addition, situations of vulnerable consumption are very common in relation to structural resources, due to the lack of availability of services, such as performing tests and scheduling a return to the doctor in the postoperative period, but we observed that, as the treatment progresses, the patients improve their interpersonal resources, especially in terms of social capital and social support. As a result, they are also better able to deal with their resources, especially in terms of knowledge and emotions. In this way, the research signals the need for improvement in the health service, mainly in the Basic Units, with emphasis on the early detection of breast cancer cases and more welcoming care by the teams that work in this provider entity. In reference hospitals, the difficulty with equipment and medication is highlighted, which generates impotence in women to obtain from the health system what they need to guarantee their well-being. Finally, we analyzed that the perception of women concerning the reception received from professionals is something of paramount importance during their exchange relationships, but that, even understanding the deficiencies in the delivery of health services, such as delays in exams and lack of medication, responses to vulnerability experiences are mostly non-defensive, probably because they feel unable to bring about major changes in the system. This reveals the need to analyze health services related to breast cancer from a systemic perspective, so that there are improvements in delivery at all levels, micro, meso, and macro, for the well-being of these women.

Nesse contexto, enquanto consumidoras dos serviços de saúde do Sistema Único de Saúde (SUS), as mulheres com câncer de mama podem experienciar consumos vulneráveis, à medida que esse tipo de doença se configura como um evento gatilho diante do qual elas se encontram incapazes de advogar em prol do seu melhor interesse, sobretudo por se tratar de uma relação de troca pautada por um saber médico, de alta complexidade. Complementarmente, o consumo dos serviços do SUS, em geral, é realizado por pacientes que são impossibilitados de acessar outros provedores, de acordo com sua preferência, o que as submete a um cenário de escolha restrita. Diante disso, o objetivo dessa pesquisa consiste em analisar as experiências de vulnerabilidade de consumidoras dos serviços de saúde relativos ao câncer de mama pelo Sistema Único de Saúde. Para tanto, realizamos uma pesquisa de caráter qualitativo, por meio da entrevista narrativa, com base em um roteiro semiestruturado aplicado com 12 mulheres acometidas pelo câncer de mama e consumidoras dos serviços de saúde do SUS. Como principais resultados, constatamos que as mulheres vivenciam experiências de vulnerabilidades antes mesmo de serem diagnosticadas com câncer de mama, principalmente durante o consumo dos serviços dos postos de saúde, antes de serem encaminhadas para os hospitais referência em câncer. Além disso, situações de consumo vulnerável são muito comuns em relação aos recursos estruturais, pela falta de disponibilidade dos serviços, como realização dos exames e marcação do retorno ao médico no pós-operatório, mas observamos que, à medida que avançam no tratamento, as pacientes melhoram seus recursos interpessoais, principalmente em termos de capital social e suporte social. Com isso, também conseguem lidar melhor com seus recursos individuais, sobretudo em termos de conhecimento e emoções. Desse modo, a pesquisa sinaliza a necessidade de melhoria no serviço de saúde, principalmente, das Unidades Básicas, com ênfase para a detecção precoce dos casos de câncer de mama e atendimento mais acolhedor por parte das equipes que atuam nessa entidade provedora. Nos hospitais referência, destaca-se a dificuldade com equipamentos e medicações, o que gera impotência nas mulheres para conseguirem do sistema de saúde o que precisam para garantir seu bem-estar. Por fim, analisamos que a percepção das mulheres em relação ao acolhimento recebido dos profissionais é algo de suma importância durante suas relações de troca, mas que, mesmo compreendendo as deficiências de entrega dos serviços de saúde, como atrasos nos exames e falta de medicação, as respostas às experiências de vulnerabilidade são, em sua maioria, não defensivas, provavelmente por se sentirem incapazes de conseguir grandes mudanças no sistema.
